Purpose: Idiopathic hypertrophic cranial pachymeningitis (IHCP) is a rare disase, which causes chronic progressive inflammation and thickening of the basal dura mater. We report a case of IHCP associated with superior oblique paralysis and present a review of the literature. Methods: An 8-year-old boy presented with binocular diplopia and left side head tilting. Suspecting right superior oblique muscle paralysis, an alternating prism test, head tilt test, fundus examinations, neurologic examination, and brain MRI were performed. Results: The brain MRI revealed abnormal enhancement of the right tentorium in the course of the right fourth cranial nerve, leading to a diagnosis of IHCP with paralytic strabismus and the patient was treated with systemic steroid therapy. Conclusions: We report a case of IHCP with right superior oblique paralysis. Patients with recent onset paralytic strabismus require appropriate neurolgic and neuroimaging examinations.

최근 금융위기와 불황은 인간의 가치관에 변화를 주어 옛것을 그리워하는 복고적 경향이 사회현상으로 나타났다. 패션에서도 복고가 새로운 트랜드로 제안되면서 창의적인 디자인으로 표현되어지고 있다. 본 연구의 목적은 과거의 슈트디자인의 특성을 파악하여, 최근 트렌드로 나타난 복고풍 슈트 디자인의 표현 특성을 알아보는데 있다. 연구결과는 다음과 같다. 첫째, 형태의 절충성은 남성적인 재킷의 형태와 여성적인 형태를 절충하여 새로운 이미지의 형태로 변화시켰다. 둘째, 하이터치 질감의 창조는 복고적 스타일에 발달된 직조, 가술로 만들어낸 하이터치 질감을 조합하여 과거의 시대적 감성을 새로운 창의성으로 표현하였다. 셋째, 선의 창의성은 슈트 내부의 구성 선으로 새로운 면 분할과 어깨와 소매 등의 연결선 이동은 새로운 비례를 만들어 독창적인 디자인을 창조하였다. 넷째, 장식적 요소의 결합은 슈트에 시각적인 리듬감으로 부여하여 흥미로운 디자인을 만들어내었다. 이처럼 21세기 여성 슈트는 과거 시대를 풍미했던 대표적 스타일을 현대적 재질, 현대적인 테일러링 기술과 디테일의 접목으로 과거와는 다른 새로운 라인과 새로운 비례를 만들어 슈트 디자인의 형태를 재창조하고 있어 미래의 슈트 디자인 발상에 아이디어를 제공해주고 있다. The recent financial crisis and recession have changed the values of human being, revealed in the retro trend longing for the old as a social phenomenon. Restoration has been also proposed in fashion as a new trend and it is being expressed in creative design. The purpose of this study is to examine the characteristics of suit designs of the past in order to examine the expression characteristics of retro style suit designs that appeared as a trend in recent. The result of the research is as follows. First, as for the compromise of shape, men`s jacket shape and feminine shape were compromised to change it into a new image shape. Second, as for the creation of hi-touch texture, the hi-touch texture created through advanced weaving and technique was combined with retro style to express the sensibility of the past into a new creativity. Third, as for the creativity of line, unique and original design was created through the new surface partition through the inside composition line of suit and the movement of connecting line such as the shoulders and sleeves. Fourth, as for the combination of ornamental elements, visual sense of rhythm was given to the suit to create an interesting design. Through the integration of modern material, modern tailoring technique and detail into the major representative style of the past, the women`s suit of the 21st century is creating new line and new proportion unlike that of the past and recreating the shape of suit design, thereby providing ideas in the concept of suit design of future.

Purpose: We evaluated the structure, innervation of the extraocular pulley smooth muscle in the rabbit. Methods: A rabbit was transcardially perfused with physiological saline followed by 4% formalin. The rabbit orbit was dissected en bloc, paraffin processed, and serially sectioned for Hematoxylin-Eosin and Masson`s trichrome stain. The distribution of the extraocular pulley smooth muscle was confirmed by immunohistochemistry with human smooth muscle(SM) α-actin. Results: We found that the pulley structure consisted of focal condensations of eleastin and collagenous connective tissue in relationship to the rectus muscle in the equatorial Tenon`s fascia. The pulley smooth muscle in relation to medial rectus muscle was immunostained with TH and nNOS. Conclusions: The data support the presence of the extraocular pulley smooth muscle in the rabbit orbit having both parasympathetic and sympathetic innervation.
경공막 유리체 절제술을 시행받은 무수정체안에서 이차적 전방인공수정체 삽입술의 임상고찰

Purpose: We studied the result and clinical significance of anterior chamber lens implantation in completely vitrectomized eyes without capsular support. Methods: We evaluated 15 aphakic eyes of 15 patients. According to the cause of pars plana vitrectomy, there were 6 eyes for lens dislocation, 5 for intraocular foreign body, and 4 for rhegmatogenous retinal detachment. Results: The final available postoperative best-corrected visual acuity was 0.5 or better in 9 eyes (60%). No statistically significant difference was noted in preoperative and postoperative IOP. The average of postoperative astigmatism was 1.14±0.79D. The most common early complication was temporary increase in IOP, but we could control IOP within normal limit with transient treatment. The postoperative late complications were glaucoma, IOL decentration and cystoid macular edema, but serious postoperative complications such as bullous keratopathy and retinal detachment were not found. Conclusions: The anterior chamber IOL implantation appears to be well tolerated and represents a simple alternative to transscleral fixation of a posterior chamber IOL in completely vitrectomized aphakic eyes.

Purpose: The result of embolization via an external superior ophthalmic vein approach of carotid cavernous sinus fistula was first described around 1970s. We report a case of coil embolization of carotid cavernous sinus fistula using a superior ophthalmic vein approach. Methods: A 70-year-old female had a 3-month history of headache, periocular pain, and diplopia. Diagnostic orbital contrast-enhanced CT, brain MRI and contrast-angiography and cerebral angiography revealed a carotid cavernous sinus fistula. Fistula occlusion via transfemoral endovascular embolization failed, so we then tried coil embolization using an external superior ophthalmic vein approach of carotid cavernous sinus fistula. Results: The coil embolization via an external superior ophthalmic vein approach was difficult because of venous tortuosity and poor exposure of part of orbital roof area. But, X-ray-guided direct puncture of the superior ophthalmic vein was successful. We thus had good results with coil embolization of carotid cavernous fistula. Conclusions: The coil embolization via superior ophthalmic vein approach is an effective and challengeable treatment when surgical cauterization or conventional endovascular embolization fails. We suggest that there is need to training ophthalmologists to be experienced in external orbital surgery.
