        Effects of Low-Dose Tamsulosin on Sexual Function in Patients With Lower Urinary Tract Symptoms Suggestive of Benign Prostatic Hyperplasia

        Purpose: The aim of the present study was to evaluate the effects of low-dose tamsulosin on sexual function in patients with lower urinary tract symptoms (LUTS) suggestive of benign prostatic hyperplasia. Materials and Methods: A total of 138 male LUTS patients aged more than 50 years with an International Prostate Symptom Score (IPSS) ≥8 were enrolled in this open-label, multicenter, prospective, noncomparative observational study. Clinical assessments included IPSS, quality of life (QoL) index, International Index of Erectile Function (IIEF), Danish Prostate Symptom Score (DAN-PSS), and an early morning erection questionnaire. The data were recorded at baseline and at 1 and 3 months after treatment with tamsulosin 0.2 mg/d. Adverse events were analyzed in all patients. Results: During the study period of 3 months, the IPSS and QoL index significantly improved from baseline by –11.40±9.40 and –1.11±1.36, respectively (p<0.001). However, there were no clinically relevant changes in total IIEF score (mean difference, 1.63±15.50; p=0.406) or the 5 subdomains (p>0.05). Furthermore, DAN-PSS weighted scores (A×B) showed no clinically relevant changes (mean difference on Q1, Q2, and Q3: –0.45±2.94, 0.27±2.50, and –1.27±2.27, p>0.05). In addition, there were no clinically significant changes in responses on the early morning erection questionnaire. Conclusions: Tamsulosin at the dose of 0.2 mg significantly improved the IPSS and the QoL index compared with baseline. However, tamsulosin did not exhibit any significant impact on sexual function or any negative impact on ejaculatory function.

        Husserl 생활세계 경험의 지평구조의 교육적 함의

        The main aim of this study is to analyze the horizon-structure, one of the structures of the life-world experience of Edmund Husserl, which is centered on practical theory emphasizing matter over form, and clarify its educational implications. The material objects are perceived by the interaction of noetic and noematic horizons, that is, one object is given by perspective variation, which forms the noematic horizons consisting of the fields of perception as one unitary element. Objects as such stand under fundamentally different highest genera, but all the objective senses and all noemata taken as complete, however various they may be otherwise, belong in principle to a single highest genus. Therefore we can define that Noema is an objectivity belonging to consciousness but possessing a nature of its own. The surrounding world is perceived to our consciousness as the meanings in the horizon-structure rather than as simple facts. When we direct our glancing ray toward an individual thing, the horizon-structure has the present real things which are already familiar, those of which are presently unknown, of which it is possible to have experience and subsequent knowledge. In order to fulfill up to these expectation, at the first stage of the contemplative intuition which percedes all explication, we directed toward the object "taken as a whole." This simple apprehension and contemplation is the lowest level of common, objectifying activity. Secondly, the higher level of the exercise of this interest is the true explicative contemplation of the object. If the contemplation then turns into explication, the interest follows the direction of the expectation which has been awakened. Explication is penetration of the internal horizon of the object by the direction of perceptual interest. At the third level of perceptual operations, in contrast to its internal determinations of explicates, relative determinations arise which display what the object is in its relation to other objects. Education is to help the students know the world and sympathize with it by greatly expanding the horizon of their own knowledge. The lite-world is experiened in the form of horizon, which is expanded infinitely in the form of internal and external horizon. The educational meaning of the horizon-structure is the task of expanding their own horizon incessantly in the problem of episteme or the way of life. The principle of the expansion of horizon has an important effect on the purpose and contents of education. The expansion of horizon is based on the expansion of the subjective experience, so it is possible that the contents of education is the means to an end if the purpose of education is excessively emphasized and it is elaborately logical and scientific.

        Estimated Cardiorespiratory Fitness is Inversely Associated with Metabolic Syndrome Risk in Korean Older Men: focusing on the 2007-2011 Korea National Health and Nutrition Examination Surveys

        The association between cardiorespiratory fitness (CRF) and metabolic syndrome (Mets) is unclear in older Korean adults. The current study aimed to investigate the diagnostic value of CRF for risk stratification of Mets in older Korean men. The data used in the study were obtained from 1220 men aged 65 years and older who participated in the Korea National Health and Nutrition Examination Surveys IV and V. CRF was estimated using a previously validated algorithm. Mets was defined using the criteria of the National Cholesterol Education Program with the adoption of a Korean-specific waist circumference cutoff point for men. Age, weight, income, marriage, education, smoking, and alco- hol consumption were included as covariates. The current findings of the study suggest that the lowest 25%CRF was sig- nificantly associated with increased risks for abdominal obesity (p < 0.001), hyperglyceridemia (p = 0.050), and hypertension (p = 0.017) in Korean older men. Furthermore, the highest 25% eCRF was significantly associated with a lower risk of MetS (odds ratio, OR = 0.555, 95% confidence interval = 0.363-0.847, p = 0.006) compared to the lowest 25% eCRF (OR = 1). The current findings of the study suggest that physical activity targeting CRF promotion should be emphasized as a therapeutic measure against the risk for MetS in older men.

        The Feasibility Study of MRI-based Radiotherapy Treatment Planning Using Look Up Table

        뇌병변 등의 방사선 치료에 있어 CT (Computed Tomography) 영상만을 이용한 종양 체적(Tumor volume) 윤곽의 정확한 설정은 CT 영상의 부족한 연부조직 대조도 특성에 의하여 한계를 가진다. 따라서 자기공명영상(Magnetic Resonance Images, MRI)이 보다 정확한 목표 체적의 윤곽을 그려내기 위해 광범위 하게 사용되고 있다. 치료계획을 위해 획득한 자기공명영상에 진단단계에서 얻어진 CT영상의 전자밀도를 융합하는 방법과 자기공명 영상으로부터 만들어진 가상의 CT를 이용하는 방법 등이 자기공명 영상장치를 기반으로 한 방사선 치료 계획의 선량계산을 위하여 소개되어 왔다. 본 연구는 MRI기반의 선량계산의 가능성을 확인 해보기 위해 15명 환자의 진단 MR 영상을 통하여 Look Up Table (LUT)을 만들어 MRI 기반 선량계산과 기존의 CT 기반 선량계산을 비교 검증 하였다. 여기서 lMRI는 획득한 MR 영상에 LUT를 이용한 전자밀도 보정을 한 것이며 wMRI는 획득한 MR영상을 물 밀도로 동일화 시킨 것이다. 6 MV anterial 방향의 조사가 CT, lMRI, wMRI에 적용되어 치료계획으로 비교되었으며 또한 환자의 병변위치에 따라 2문 조사에서 5문 조사의 치료계획이 비교되었다. CT기반 치료계획을 기준으로 하여 등선량 분포와 DVH의 차이는 wMRI 보다 lMRI에서 더 적었으며 최대선량 차이가 91 cGy vs. 57 cGy, 평균선량이 74 cGy vs. 42 cGy, 최소선량 차이가 94 cGy vs. 53 cGy로 측정되어 각각의 선량 평가 면에서 그 차이가 wMRI보다 lMRI에서 더 적었다. 이러한 결과는 wMRI의 경우 공동내 선량계산에서 CT기반 선량계산과 차이가 나기 때문이다. 따라서 본 연구의 결과는 lMRI 기반 선량계산의 가능성이 있음을 보여준다. In the intracranial regions, an accurate delineation of the target volume has been difficult with only the CT data due to poor soft tissue contrast of CT images. Therefore, the magnetic resonance images (MRI) for the delineation of the target volumes were widely used. To calculate dose distributions with MRI-based RTP, the electron density (ED) mapping concept from the diagnostic CT images and the pseudo CT concept from the MRI were introduced. In this study, the look up table (LUT) from the fifteen patients' diagnostic brain MRI images was created to verify the feasibility of MRI-based RTP. The dose distributions from the MRI-based calculations were compared to the original CT-based calculation. One MRI set has ED information from LUT (lMRI). Another set was generated with voxel values assigned with a homogeneous density of water (wMRI). A simple plan with a single anterior 6MV one portal was applied to the CT, lMRI, and wMRI. Depending on the patient's target geometry for the 3D conformal plan, 6MV photon beams and from two to five gantry portals were used. The differences of the dose distribution and DVH between the lMRI based and CT-based plan were smaller than the wMRI-based plan. The dose difference of wMRI vs. lMRI was measured as 91 cGy vs. 57 cGy at maximum dose, 74 cGt vs. 42 cGy at mean dose, and 94 cGy vs. 53 at minimum dose. The differences of maximum dose, minimum dose, and mean dose of the wMRI-based plan were lower than the lMRI-based plan, because the air cavity was not calculated in the wMRI-based plan. These results prove the feasibility of the lMRI-based planning for brain tumor radiation therapy.

        Parameter identification for nonlinear viscoelastic rod using minimal data

        Parameter identification is studied in viscoelastic rods by solving an inverse problem numerically. The material properties of the rod, which appear in the constitutive relations, are recovered by optimizing an objective function constructed from reference strain data. The resulting inverse algorithm consists of an optimization algorithm coupled with a corresponding direct algorithm that computes the strain fields given a set of material properties. Numerical results are presented for two model inverse problems;(i)the effect of noise in the reference strain fields (ii) the effect of minimal reference data in space and/or time data.

        해리수직편위에 대한 상직근재후전술

        Purpose: When recurrent dissociated vertical deviation (DVD) is cosmetically objectionable after superior rectus recession is used to surgically treat DVD, inferior rectus resection or superior rectus re-recession should be considered. The effect of re-recession of the superior rectus was, therefore, evaluated as a treatment of recurrent DVD. Methods: We retrospectively reviewed the medical records of 9 eyes from 7 patients who had undergone superior rectus re-recession as a second surgery due to recurrence after superior rectus recession for DVD. Results: The mean follow-up period after re-recession of the superior rectus was 32.1 (22~66) months. The preoperative mean deviation was 16.0 prism diopters (PD), and the postoperative mean deviation was 4.9PD. According to the classification of recurrent DVD by Scott, the surgical results of five eyes were excellent (0~4PD), those of 2 eyes were good (10~14PD), and those of 2 eyes were fair. The mean corrected DVD was 4.1PD per 1 mm recession. Conclusions: Postoperatively, 7 of 9 eyes (77.7%) were aligned within 9PD. In cases of recurrent DVD, superior rectus re-recession yielded satisfactory results with minimum surgery on the superior rectus. The results of this study will be helpful in deciding surgical amount of superior rectus re-recession to be performed. J Korean Ophthalmol Soc 2010;51(2):266-269

        구결막에 발생한 신경초종 1예

        Purpose: The schwannoma is a tumor originating from Schwann cell proliferation. Schwannoma is a rare disease, making up only 1% of all the tumors that develop in the orbit. Schwannomas usually arise from the choroids or the ciliary body, and occurrence in the conjunctiva is especially rare. Few cases have been reported worldwide, and no cases have been reported in Korea thus far. We report our case along with a literature review. Case summary: A 31-year-old male patient visited our department with a history of discomfort of in his right eye for the past 5 years caused by a bulbar conjunctival mass. On ophthalmologic examination, a 5×3×3 mm, elevated, yellowish mass with a smooth surface was observed on the bulbar conjunctiva adjacent to the superonasal limbus near the 2 o’clock area of the right eye. We performed excision and biopsy for diagnosis and treatment of the bulbar conjunctival mass and confirmed the pathologic report for the diagnosis of schwannoma. 서론: 신경초종은 슈반세포(Schwann cell)의 증식에 의해 발생되는 종양으로 안와에서 발생하는 종양 전체의 1%를 차지하는 드문 질환 이다. 안구 내에 발생하는 경우, 주로 맥락막이나 모양체에 발생한다. 결막에 발생하는 경우는 극히 드물어 세계적으로도 보고된 바가 적으며 국내에서는 아직 보고된 바가 없다. 저자들은 구결막에 발생한 신경초종 1예를 확진하고 치험하였기에 문헌고찰과 함께 보고하는 바이다. 증례요약: 5년 동안의 우측 구결막 종괴로 인한 이물감을 주소로 내원한 31세 남자 환자로, 안과검사상 우안의 2시 방향 상비측 윤부주 위 구결막에 표면이 매끈하고 경계가 분명하며 융기된 5×3×3 mm 크기의 황색 종괴가 관찰되었다. 진단 및 치료 목적의 절제생검을 시행하여 신경초종으로 확진하였다.

        Partial-isolation LDMOS의 항복전압과 온저항 분석

        본 논문에서는 Partial isolation lateral double diffused metal oxide semiconductor(Pi-LDMOS)의 항복전압에 대해 시뮬레이션을 통해 분석하였다. 항복전압 변화는 Partial buried oxide(P-BOX)의 다양한 파라미터(길이, 두께, 위치)에 따라 조사되었고, 그 메커니즘에 대해 명기하였다. 또한 항복전압과 trade-off 관계에 있는 온저항의 변화를 P-BOX 파라미터 변화에 따라 분석하였고 Figure of merit(FOM)을 계산하여 비교하였다. 제안된 구조에서 Lbox=5μm, tbox=2μm, Lbc=2μm일 경우 138V의 가장 높은 항복전압을 나타내었고, Lbox=5μm, tbox=1.6μm, Lbc=2μm일 경우 가장 높은 FOM을 나타내었다. 이는 conventional LDMOS대비 항복전압은 123%, FOM은 3.89배 향상된 수치이다. 따라서 Pi-LDMOS는 높은 항복전압과 FOM을 가져 Power IC의 안정적인 동작범위 향상에 기여할 수 있다. In this paper, the breakdown voltage of Pi-LDMOS (Partial isolation lateral double diffused metal oxidesemiconductor) was analyzed by simulation. Breakdown voltage variation is investigated under various settings ofParial buied oxide(P-BOX) parameters(length, thickness, location) and their mechanism is specified. In addition,the change in on-resistance in the breakdown voltage and trade-off relationship was analyzed according to the changein the P-BOX parameter, and the Figure-of-merit(FOM) was calculated and compared. In proposed structure,Lbox=5 μm, tbox=2 μm, and Lbc=2 μm showed the highest breakdown voltage of 138V, and Lbox=5 μm, tbox=1.6μm, and Lbc=2 μm showed the highest FOM. Compared to conventional LDMOS, the breakdown voltage is 123%and FOM is 3.89 times improved. Therefore, Pi-LDMOS has a high breakdown voltage and FOM, which cancontribute to the improvement of the stable operating range of the Power IC.
