슬관절 전치환술후 일상생활의 문제점 : 농촌지역의 거주환자를 중심으로

Forty-two patients (fifty-three knees) with severe arthritis of the knee were treated by total knee replacement. We used 42 P.F.C. knees and 11 A.M.K. knees. The forty-two patients consisted of thirty three females and 9 males ranging in age from 29 to 80 years (mean, 59 years). The mean follow-up period was 23 months (range, 4 months to 40 months). One of the primary goals of total knee replacement (TKR) is the restoration of painless, functional range of motion (ROM). In the authors experience, however arthroplasty never restores the full flexion or function of the normal knee. We analyzed the results and problems in daily activities of the patients living in rural area after TKR, We obtained following results; 1. Pain relief and level walking, stair up and down were satisfactory in 90 percents of the patients. 2 The mean postoperative flexion was 107 degrees and ranged from 55 degrees to 135 degrees. The mean postoperative flexion contracture were 5 degrees and ranged from 0 to 16 degrees. 3. The average Knee Rating Score of Hospital for Special Surgery was improved from 48 preoperatively to 82 postoperatively. 4. Patients living in rural area had many difficulties in daily life. 5. Equipment requirements at home and change of life style are necessary in bilateral cases and patients less than 100 degrees of ROM in single TKR.

Twenty four patients(twenty seven knees) with degenerative arthritis of the knee were treated by the carbon fiber arthroplasty and the after-surgery effects were reviewed. The twenty four patients consisted of fourteen females and ten males ranging in age from 45 to 76 years(mean, 58 years). The mean follow-up period was 13 months(range, 9 months to 17 months). Carbon fibers have many attractions. In its pure form, it is chemically inert and biocompatible with the environment of the knee, and induces an abundant collagenous fibrous tissue organized around and along the carbon fibers. Many of the patients demonstrated an excellent recovery process : e.g., they quickly became able to move without pain. I employed arthroscopy for 2 to 12 months after surgery in 6 patients and found that very few rods or pads were still visible in 2 months after implantation. I have seen no evidence of distintegration in any of the pads or rods, or migration of carbon fiber debris into the joint space or the surrounding tissues. The carbon fiber arthroplasty appears to be appropriate in the surgical management of grade Ⅱ articular cartilage defects (i.e., erosion of the articular cartilage down to the subchondral bone) in the painful knee.
Risk Premium, Liquidity Premium, and Expectations Hypothesis in the Treasury Bill Market

This paper examines whether the risk premium and the liquidity premium play an important role in explaining excess holding period return and whether two components can explain the empirical failure of expectations hypothesis. The paper finds from the study of U.S. Treasury Bill rates that the risk premium and the liquidity premium are important in explaining excess holding period return. However, the expectations hypothesis is not salvaged under the maintained hypothesis concerning the liquidity premium and risk premium although two premiums improve the forecastability of yield spread. The paper attributes the results to the possibility that the difference between the relative bid-ask spread of T-bill rates is not accurate measure for the time-varying liquidity.
