Flavobacterium Indologenes에 의한 각막염 1예

목적 : Flavobacterium indologenes는 기회감염의 형태로 매우 드물게 각막염을 일으키는 것으로 알려져 있다. 저자들은 당뇨병의 과거력을 가진 1안에서 Flavobacterium indologenes에 의한 각막염을 경험하여 이를 보고하고자 한다. 대상과 방법 : 각막염이 있었던 1안에 대해 병력청취, 세극등현미경검사, 도말 및 배양검사, 항생제감수성검사 등을 시행하였다. 결과 : 내원시 시행한 각막찰과 도말배양검사에서 Flavobacterium indologenes가 검출되었다. 항생제감수성검사상 감수성이 있다고 나타난 Piperacillin을 점안하여 치료하였고 치료후 시력의 호전과 증상의 개선을 보였다. 결론 : 전신질환이나 안면역성이상이 동반되는 등의 기회감염이 증가하는 상황에서 발생한 각막염의 경우 원인균으로 Flavobacterium indologenes을 고려해 볼 만 하다. Purpose: Flavobacterium indologenes is known to cause keratitis very rarely. Authors have experienced 1 case of keratitis from Flavobacterium indologenes with history of diabetes mellitus, thereby reporting it. Methods: History taking, slit lamp examination, staining and culture, sensitivity test about antibiotics were performed on 1 case of keratitis. Results: Flavobacterium indologenes was detected in staining and culture that was performed on the first visit. Piperacillin was used based on the sensitivity test about antibiotics. Improvement of corneal lesion and symptom was observed with the use of piperacillin. Conclusions: Flavobacterium indologenes can be considered as a casual pathogen in keratitis with condition susceptible to opportunistic infection such as systemic illness or abnormal ocular immunity.

문신제거용 Nd:YAG 레이저에 의해 발생한 황반원공 1예

Purpose: To report a case of a macular hole resulting from accidental exposure to tattoo removal by the Q-Switched Nd:YAG laser, which was treated successfully by vitrectomy and silicone oil infusion. Case summary: A 33-year-old man presented with decreased visual acuity after accidental exposure to a Q-Switched Nd:YAG laser. According to fundus examination, vitreous hemorrhage and macular edema were observed. After 21 days, a macular hole had developed which was treated by standard pars plana vitrectomy and gas tamponade. Unfortunately, closure was not obtained on the first attempt. Therefore, a second attempt using silicone oil infusion was performed. Four months after the initial visit, BCVA had increased to 20/50, and anatomical occlusion was achieved. Conclusions: The authors of the present study experienced an unusual case of macular hole developed from the Q-Switched Nd:YAG laser used to remove a tattoo. A satisfactory visual acuity was achieved after silicone oil infusion despite failure in the first surgery. J Korean Ophthalmol Soc 2011;52(3):373-377

Research Purpose: This paper aims to slove various problems that may arise in the field through a mathematical model enabling optimization of steel product transportation logistics Research Methods: Among the various costs incurred in shipping steel products using port equipment, it has an objective function of minimizing costs by reducing a number of other pier transfers through optimization of shipping work. In order to utilize the mathematical model, the actual problem situation is identified, and related engineering knowledge is studied and linked to a formula. Using the mathematical model established in this way, the optimal solution is derived and empirically verified. Results in Research: The invisible effects as well as the temporary logistical cost savings achieved by the improvements demonstrated in the study are considered valuable. The invisible powerful effect of using the mathematical model is that it is able to break the stereotypes about work standards that has been hardened for a long time and change the behavior of the field. Research Conclusion: Based on the results of this study, it is expected that it will be helpful in promoting innovative tasks for improving the logistics competitiveness of steel products by applying a mathematical model to big data. In addition to the arithmetic effects of the empirical evidence mentioned in this research paper, it is suggested that this model will contribute to the development of the shipping business of steel companies by leading to the discovery of ideas using mathematical models in improvement activities in other fields in the future. 본 연구는 다양한 철강제품을 선적하는 해송 물류의 효율화를 위해, 현실에서 발생될 수 있는 문제에광범위하게 적용될 수 있도록 최대한 많은 변수들을 모형에 포함 시켰다. 수리모델을 수립하고 최적의 해를도출하여 현장에서 실제 검증하였으며, 검증 내용을 가지고 작업자들을 설득하고, 현장에서 검증 및 위험대책을 마련하여 최종적으로 작업기준을 재개정하고 해송 출하 물류를 혁신적으로 개선하였다. 구체적으로, 수리모델을 활용한 해송 출하 물류 개선활동인 코일 운송장비 적재기준 개선, 후판제품 받침목 포설기준개선, 제품선적 시 선박 내 작업 기준 개선 등을 통해 물류비를 절감할 수 있었고, 이는 단발성의 효과가아니라 매 작업이 반복될 때마다 발생하는 지속적인 효과로서 그 의의가 매우 크다고 할 수 있다. 본 연구는 실증된 내용들에 의해 얻어진 물류비용 절감도 중요하지만, 눈에 보이지 않는 효과도 값진것으로 생각한다. 수리모델 활용의 보이지 않는 강력한 효과는 오랜 시간 굳어져 온 작업기준에 대한 고정관념을 깨고 현장의 행동을 바꿀 수 있었다는 것이다. 수리모델을 통한 합리적인 해결방안과 그 도출 원리를이용한 설득으로 작업자뿐만 아니라, 항만 근로자들의 해송 출하 업무에 대한 개념을 혁신적으로 개선할수 있었다. 본 연구결과를 바탕으로 수리모델을 접목하여 철강제품 물류경쟁력 향상 및 고객 만족을 위한 획기적인과제를 추진하는 데 도움이 되기를 기대하며, 빅데이터(big data) 분석 결과를 수리적으로 모델링하고 최적의해결방안을 모색한다면, 더욱 강력하고 효과적인 방법을 찾을 수 있을 뿐 아니라, 실시간으로 변하는 입력값을 활용하여 정확한 패턴 예측 및 관리를 수행할 수 있을 것으로 기대된다.
