        Reactive oxygen species (ROS) induces distinct gene expression in human fibroblast like synoviocytes (HFLS)

        Adhesive capsulitis of the shoulder is a common cause of pain that occurs during shoulder movement, thereby restricting shoulder rotation in clinical practice. Although most patients respond to pain relief treatment (NSAID or corticosteroids) by improving their range of motion, it remains poorly understood without any definitive treatment algorithm. In addition to immune cells, synoviocytes, chondrocytes and osteoblasts in the joint are known to produce pro-inflammatory mediators such as reactive oxygen species (ROS), inflammatory cytokines and lipid mediators, presumably contributing to the pathogenesis of osteoarthritis (OA) and adhesive capsulitis. Although inflammation and also fibrosis are proposed to be the basic pathological changes of a frozen shoulder, there is a lack of information regarding the downstream targets of the pro-inflammatory ROS signaling pathway in the synoviocytes and also how these ROS targets are modulated at the transcription level by a corticosteroid - dexamethasone. In this study, we used human fibroblast like synoviocytes (HFLS) to characterize the signaling targets of ROS by employing a human DNA microarray tool and studied the role of dexamethasone in this process. Our data suggest that several genes such as FOS, FOSB and NFkBIZ, which are known to be involved in pro- or anti- inflammation response, are modulated at the transcription level by ROS and dexamethasone.

        젊은 성인 무지 외반증 환자에서 원위부 갈매기형 절골술의 결과

        Purpose: To analyze the risk factors and the treatment results of the distal chevron osteotomy for the hallux valgus in young adult. Materials and Methods: Eleven cases of hallux valgus in 9 patients who underwent the distal chevron osteotomy between February 2000 and January 2004 were included for the study. For the clinical evaluation, we used hallux-metatarsophalangeal interphalangeal scale of American Orthopaedic Foot and Ankle Society (AOFAS). For radiological evaluation, we measured hallux valgus angle (HVA) and intermetatarsal angle (IMA). Results: Five patients had family history of hallux valgus. Symptoms were aggravated by military drill and sports activity. The average AOFAS scale was 62.5 preoperatively, and 88.4 at final follow-up. The HVA and IMA were $34.9^{\circ}$, and $15.8^{\circ}$ preoperatively, $16.3^{\circ}$ and $8.2^{\circ}$ at postoperative 6 weeks, and $19.9^{\circ}$ and $9.8^{\circ}$ at final follow-up. Conclusion: The symptoms of hallux valgus in young adults were aggravated by military drill and sports activity. Distal chevron osteotomy was useful for the treatment of the moderate to severe hallux valgus in young patients.

        퇴행성 척추 측만증과 척추관 협착증에서 Matrix Metalloproteinase-3 발현의 차이

        목적: 추간판의 기질의 파괴와 퇴행성 변화에 깊이 관여하는 것으로 알려진 세포외 효소인 matrix metalloproteinase-3(MMP-3; stromelysin)을 이용하여 퇴행성 측만증과 측만증이 동반되지 않은 단순 척추관 협착증 환자의 추간판에서의 MMP-3의 발현의 정도를 western blotting과 면역조직화학 염색을 이용 분석하여 추간판 기질의 파괴가 퇴행성 요추 측만증의 병인에 있어서 관련됨을 증명하고자 한다. 대상 및 방법: 추간판 탈출증, 척추관 협착증 및 퇴행성 요추 측만증 환자에서 수술 중 제거한 추간판의 조직을 이용하였다. 모든 조직의 채취는 후방 도달법에 의한 추간판 제거술로 시행되었으며, 채취된 조직은 즉시 액화 질소에 의하여 냉동 보관하였다. 채취된 조직 중 추간판 탈출증 2예를 대조군으로, 척추관 협착증 8예 및 퇴행성 요추 측만증 7예를 대상으로 하였다. Antihuman MMP-3를 이용하여 추간판의 면역화학적 염색과 western blotting을 시행하였다. 결과: 면역조직화학 염색 결과 MMP-3의 발현에 있어서 퇴행성 요추 측만증의 추간판에서 측만증이 동반되지 않은 척추관 협착증의 추간판보다 상당히 많은 양성 염색을 나타내었으며, western blotting 의 정량적 분석에 있어서도 퇴행성 요추 측만증(평균 optical density: 20.68±8.4)에서 척추관 협칙증 (평균 optical density: 6.24±3.6)보다 유의하게 많은 발현을 보였다. 결론: 퇴행성 요추 측만증에 있어서 추간판의 퇴행성 변화와 관계가 깊은 MMP-3의 발현은 단순 척추관 협착증에 비하여 유의하게 많은 발현을 보였다. 따라서 퇴행생 요추 측만증의 병인에 있어서 MMP-3가 추간판의 퇴행성 변화에 중요한 역할을 하는것으로 생각된다. Purpose: This experimental study hypothesize that "rapid degeneration of the intervertebral disc over the ability of auto-stabilization by osteophytes bridging to adjacent segments might be the pathomechanism of degenerative lumbar scoliosis (DLS)." An effort to prove this hypothesis was attempted by confirming the differences in the expression of matrix metalloproteinase-3 (MMP-3) in DLS and pure spinal stenosis. Materials and Methods: The intervertebral disc materials obtained during a discectomy through a posterior approach, were included in this experimental study. Two protruded herniated nucleus pulposus were included for the control group. Eight spinal stenosis and seven DLS were included as experimental groups 1 and 2, respectively. The expression of MMP-3 was evaluated by immunohistochemical staining and western blotting using anti human MMP-3 antibody. Results: In the immunohistochemical stains, sparse staining was noted in the control group. More staining, however, mainly extracelluar, was noted in the discs of DLS compared with those of the spinal stenosis group. In western blotting, greater MMP-3 expression was noted in the discs of DLS (mean optical density: 20.68) than in those of the spinal stenosis group (mean optical density: 6.24) which was statistically significant (p<0.05). Conclusion: Rapid degeneration of the intervertebral disc might be an important factor for the pathogenesis of DLS. MMP-3 could be one of the key enzymes for the rapid degeneration of the intervertebral discs especially in DLS.

        반건양건에 의한 발음성 슬관절

        여러 원인에 의해 드물게 슬관절에 마찰음을 감지할 수 있는데 대부분 외측 반월상 연골판의 전위나 원판형 연골판을 가지고 있는 예에서 나타나며, 그 외 대퇴 이두건, 반건양건, 장경대, 슬와건 등에 의해서도 나타난다. 외상 경험이 있는 환자에서 슬관절의 내측부에 동통과 함께 반건양건에 의해 생기는 발음성 슬관절 환자를 경험하여 수술적 치료 후 좋은 결과를 얻어 문헌과 함께 보고하과 한다. Snapping syndrome has rarely been reported in the knee. A snapping knee mainly occurs in the case of a translation of the lateral meniscus or a lateral discoid meniscus. Other causes include biceps femoris tendon, semitendinosus iliotibial band, and popliteus tendon. We recently experienced a case of a snapping knee caused by the semitendinosus tendon passing over the medial tibial condyle as a result of trauma.
