        외이도에 발생한 이구선암종 1예

        Ceruminous adenocarcinoma is a malignant subtype of ceruminoma. Ceruminous gland tumors of the external auditory canal (EAC)histological presentations with diferent malignant potential. A 51-year-old woman presented with one month history of a right side otalgia. Physical examination showed an iregular surface elevation on the anterior portion of the right EAC and a normal tympanic membrane. Histologic examination revealed adenoid cystic carcinoma. After radiation therapy, lateral temporal bone adenocarcinoma. At the last folow-up at 10 months, there was no evidence of recurrence. Ceruminous adenocarcinoma of the EAC is a rare malignant neoplasm and as definitive diagnosis is sometimes not provided until surgery, a biopsy must be performed in every EAC lesion. Combined radical surgery and radiation is the mainstay of therapy.

        흰쥐에서의 카르복시메칠셀룰로오스나트륨과 겔란검 혼합겔의 유착방지 효과

        The aim of the present study was to investigate the efficacy of a mixed gel formulation composed of sodium carboxymethyl cellulose and gellan gum (Na-CMC gel) for the prevention of adhesions after laminectomv. The anti-adhesive effect of the Na-CMC gel was tested in a controlled randomized study using an animal model of lumbar laminectomv. The animals (60 female Sprague-Dawlev rats) were randomly allocated into two treatment groups to receive the Na-CMC gel on the injured area or no gel (control). The incidence of adhesions and their grade were blindly evaluated at 4. 8 and 12 weeks after surgery. The amount of scar tissue and tenacity were grossly reduced by the Na-CMC gel at postoperative 4. 8. and 12 weeks. The mean adhesion scores were 0.75. 125. and 1.38 at 4. 8. and 12 weeks in the gel-treated group. respectively. No significant inflammatory reaction was observed and the healing of wound was not affected by the Na-CMC gel. The Na-CMC gel reduced the amount of scar formation and tenacity in rat laminectomv model without affecting the healing of operation wound and other complications. Therefore. the Na-CMC gel may be the potential to prevent postsurgical adhesions in clinical state.

        An advanced maxillary sinus cancer requires an extensive ablation that results an extensive facial deformity, including a skin defect. Reconstruction has to be considered in a radical maxillectomy, especially with skin defect may be accomplished in one stage with a microsurgical free transfer of a latissimus dorsi flap. A man of right maxillary sinus cancer, squamous cell carcinoma, 47 years old of age, had soft tissue invasion of the cheek region. He underwent a radical maxillectomy with extensive skin excision. The maxillectomy and skin defects were reconstructed with the double skin island latissimus dorsi myocutaneous free flap. The cosmetic result and the functional outcome of the nose were thought to be considerably satisfied.

      • 하수슬러지를 이용한 복합소재 개발연구

        본 연구는 하수 종말 처리에 의해 발생되는 하수 슬러지를 잔 골재(경량골재)로 사용하여, 폴리머 콘크리트를 제조하여 그 때의 역학적 성질에 대하여 검토하였다. 하수 슬러지는 5일간 110±5℃에서 수분함량 0.1%미만으로 건조한 후 파쇄하여 No.4∼No. 200 번체의 잔골재 표준 시방서 기준에 부합되도록 사용하였다. Pellet는 보통 회전pan에서 만드나, 본 실험에서는 수동으로 제조하였다. 제조한 하수 슬러지 경량 골재는 평균 겉보기 비중은 1.58(g/㎤), 단위용적중량은 1.98(t/㎥)으로 KS F 2534의 경량골재의 절건비중과 단위용적에 따른 분류에 의하면 중위급(medium weight) 경량골재에 속하는 것으로 측정되었다. 또한 잔 골재 중 하수 슬러지 경량 골재를 0%, 5%, 10%, 15%, 20%, 25%, 30%로 치환하여 그때의 압축, 휨, 인장강도를 측정하였다. 그 결과 하수 슬러지의 증가에 따라 강도 감소를 보였으나, 일반 경량콘크리트 제조 또는 단열용 콘크리트 블록 제조에 사용이 가능하다고 사료된다. 그러므로 앞으로 건조수축, 크리이프 특성 및 내구성에 관한 연구가 더욱 심도 있게 이루어져야 폐자원인 하수 슬러지를 이용한 건자재의 개발이 가능하리라 판단된다. In this study, the polymer concrete was produced by using the lightweight aggregate that had been manufactured using the sewage sludge and its physical properties were experimentally examined. Experimental results show that when the portion of sewage sludge lightweight aggregate in the fine aggregate increases, slump values decreases. Also, the compressive, the flexural and the tensile strengths decrease when the portion of sewage sludge lightweight aggregate increases. Although its strengths decreases, the sewage sludge lightweight fine aggregates is thought to be used for construction materials.

      • 콘크리트 구조물 보수용 폴리머 시멘트 모르타르의 제조 및 물성 연구

        라텍스로 처리한 초속경시멘트(RS-LMC)에 사용되는 라텍스의 안정성의 평가 및 안정성을 증대시키기 위한 연구를 수행하였다. 비이온계 및 음이온계 계명활성제를 사용한 라텍스 시제품에 대하여 전기전도도를 측정하는 방법을 이용하여 안전성을 평가하였다. 라텍스를 PEG를 첨가한 후 라텍스 용액의 안정성에 미치는 영행을 평가하였다. 라텍스의 침전제로 CaCl_2를 사용하였다. 음이온계 계면활성제를 포함한 라텍스는 CaCl_2를 첨가하면, 즉시 침전현상이 발생되나, PEG를 처리한 경우에는 침전현상이 매우 느리게 일어나서 안정화 효과가 있음을 확인하였다. 반면, 비이온계 계면활성제를 포함한 라텍스는 침전확인이 거의 발생되지 않아, 이러한 라텍스가 RS-LMC에 적합함을 확인하였다. 비이온계 계면활성제를 포함한 라텍스라 할지라도 1년이상 장기간 보존하면 안정성이 저하됨을 전기전도도를 측정하는 방법을 통하여 확인하였다. 장기간 보존한 라텍스의 경우 라텍스 용액내에서 미생물이 다량 성장하였음을 확인하였으며, 이러한 라텍스 용액의 경우 안정성이 저하됨을 알았다. In this study, the polymer-modified mortars has been made for investigating its physical properties, in which cement, polymer dispersions and redispersible polymer powder were used as a binder, and silica sand as the fine aggregate. The effects of polymer-cement ratio on strength properties, waterproofness, resistance of carbonation and resistance of chloride ion penetration of polymer-modified mortars were examined. Experimental results show that the flexural, compressive, tensile and adhesion in tension strengths of the polymer-modified mortars increased with increasing polymer-cement ratio. The water absorption, carbonation depth and chloride ion penetration depth of polymer-modified mortars tended to decrease with increasing polymer-binder ratio. Addition of the polymer dispersion and redispersible polymer powder were found to be effective for improving strength properties and durability of polymer-modified mortars.

        인체 비점막과 비용에서 Na+:HCO3-cotransporter(NBC) mRNA와 K+:Cl- cotransporter(KCC) mRNA의 발현과 분포

        Background and Objectives:Electrolyte transport by nasal epithelia has ben sugested to be important for controlling the transport of ion and water in various fluid-transporting epithelia is strictly dependent on the localization of specific membrane proteins in the polarized epithelial cells. Na+: HCO3-cotransporter (NBC) transports Na+ and HCO3- into the intracellular from extracellular space and induces the evacuation of H+ , regulating pH. K+: Cl- cotransporter (KCC) controls the cel the presense of mRNA for NBC and KC in human inferior turbinate and nasal polyp. Materials and Method:The expres-sion of NBC and KC mRNA isoforms in inferior turbinate mucosa and nasal polyp was evaluated, using RT-PCR and in situ hybridization. Results:RT-PCR revealed that the inferior turbinate and nasal polyp mucosa expressed kNBC, KC1 and 4 mRNA. In in situ hybridization, their distribution was noted in the epithelial layer and submucosal glands of both mucosa. Conclusion:controling the fluid and ion transport in nasal epithelium and submucosal glands.

        Hockey Stick-shaped Merocel?? 이용한 아데노이드 출혈의 치료

        Background and Objectives:Efective control of bleeding from the adenoidectomy site is more dificult than bleeding from key stick-shaped Merocel. Subjects and Method:Twenty-three cases of masive bleeding in children who were not treated to electrocauterization and epinephrine packing during adenoidectomy and 6 cases of imediate bleding on adenoid on the day after operation in pediatric patients were included. In 23 cases of massive bleeding during operation, hockey stick-shaped Mero-celit was inserted to the nasopharynx through nasal cavity after local infiltration anesthesia for nasal mucosa. We evaluate the clinical usefulness of the hockey stick-shaped Merocelpacking. Results:There was no postoperative bleeding in all cases of compressive hemostasis using hockey stick-shaped Merocel. There were temporary complications such as nasal obstruction, . There were no severe compli-cations such as sleep apnea or respiratory disturbance. Conclusion:Effective hemostasis for continuous adenoidectomy site bleeding in children was achieved using hockey stick-shaped Merocelwithout severe complications.

        결핵성 경부 림프절염의 항결핵제 치료기간에 관한 연구:6개월과 12개월 요법의 무작위 임상대조연구

        Background and Objectives:Tuberculous cervical lymphadenitis is one of the most common extrapulmonary manifestations of tuberculosis. Foreign guidelines recommend a six-month short course chemotherapy for tuberculous peripheral lymphadenitis as an initial standard regimen. However, there have been some debates about the duration of chemotherapy in South Korea. The aim of this study is to determine the reasonable duration of chemotherapy for tuberculous cervical lymphadenitis in a randomized controlled study. Subjects and Method:We analyzed clinical outcome of 80 patients with tuberculous cervical lymphadenitis treated at Korea University Medical Center from April 1998 to November 2001. All patients were randomized in two groups by treatment regimens. All patients were given antituberculous chemotherapy for 6 or 12 months, starting with isoniazid (H), rifampicin (R), ethambutol (E) and pyrazinamide (P) for 2 months, then continuing with isoniazid, rifampicin and ethambutol for the remaining 4 months or 10 months, respectively. Results:Forty-seven patients were given a 12-month regimen (2 HERZ/10 HRE), and 43 patients (91.5%) were cured. Thirty-three patients were given a 6-month regimen (2 HERZ/ 4 HRE), and 20 patients (60.6%) were cured. The cure rate of the 12-month regimen appeared significantly higher than that of the 6-month. In both groups, no recurrence was found during the follow up period up to 12 months. Conclusion:The 12-month regimen could be recommended as a reasonable chemotherapy regimen for tuberculous cervical lymphadenitis in South Korea. Single lymph node excisional biopsy is an important diagnostic method. but, it has no additional benefits of treatment.
