        한국 노인이 겪는 우울감 및 자살사고에 대한 질적 연구 분석

        Objectives: To analyze the reports of qualitative research about depressed mood and suicidal ideation of Korean Seniors. Methods: Eleven published qualitative research studies were selected for analysis. We extracted common topics and factors of each stage, Selected factors were paralleled and synthesized to make a flow chart of the phenomena. Results: Depressed mood arousals of Korean seniors are categorized by causation factors, which include economic, psychosomatic, and social factors, and positive-negative coping strategies, which include cognitive-behavioral factors. Suicidal experiences are categorized as ideation, suicidal attempt, and after-suicide stage. In the latter, participants reported three types of lifestyles by their enthusiasm for 'being alive'. Conclusions: Through the qualitative synthesis of research, we could analyze and categorize major factors and coping strategies of participants who exhibited a depressed mood or suicidal ideation.

        코로나-19 대유행 상황에서의 심신중재요법에 대한 연구 동향 분석

        본 연구의 목적은 코로나 19 대유행 상황에서 심신중재요법의 연구동향을 알아보고 향후 코로나 19 상황에서 명상 및 심신중재요법 연구의 방향 및 실제 환경에서의 활용 방안을 제시하는 데에 있다. 검색된 임상연구 및 임상연구 프로토콜은 코로나 19 확진 환자, 코로나 19 환자를 진료하는 의료진, 의료기관 봉쇄로 인하여 진료 받지 못하는 타 질환 환자, 불안감을 호소하는 일반 시민을 참여자로 하여 계획되고 있었다. 우울, 불안 등의 부정적인 정서를 개선하기 위한 목적이 가장 다빈도로 검색되었으며, 대응전략의 변화, 전염을 촉발할 수 있는 문제 행동의 개선 등도 일부 연구에서 심신중재요법의 목적으로 설정됨을 확인할 수 있었다. 또한, 검색된 연구에서 심신중재요법은 다양한 방식으로 계획되었으며, 연구자들은 어플리케이션, 인터넷, 녹화 영상 등으로 진행된 개입의 비교를 통하여 비대면 환경에서 가장 효과적인 개입방법을 모색하고자 하였다. 현재는 결과를 보고하는 임상연구보다 프로토콜 단계의 연구들이 많은 시점으로 본 연구에서는 해당 임상연구들의 계획과 목적을 파악하는 데에 그쳤으나, 본 동향 분석은 향후 심신중재요법을 코로나 19 상황에서 적용하거나 연구를 준비하는 과정에서의 참고가 될 수 있을 것으로 사료된다. 또한 향후 더욱 실제적인 영역에서의 임상연구 및 활성 대조군을 포함한 연구가 진행되어 각 연구 참여자군에 대하여 어떤 방식으로, 어떤 심신중재요법을 시행하는 것이 최적의 효과를 발생시킬 수 있는지에 대한 연구가 진행되어야 할 것이다. The aim of this study is to analyze the research trend in Mind-Body Intervention on COVID-19 Pandemic and suggest guidelines for future research and how to apply meditation or Mind-body Intervention under COVID-19 Pandemic in clinical scene. Most of the studies and clinical protocols being searched in relation to Mind-Body Intervention on COVID-19 were designed for confirmed COVID-19 patient, healthcare professionals treating COVID-19 patient, other patients who cannot get usual care for pre-existing medical condition in Pandemic, and healthy individual expressing anxiety. Negative emotions such as depression, anxiety were most frequently searched terms for purpose of Mind-Body Intervention, change in coping strategy and reforming problem behaviors which can cause contagion were also searched as purpose of Mind-Body Intervention. Researchers of these studies modified Mind-Body Intervention in various forms, from application, internet to recorded video to find the most ideal form of Mind-Body Intervention in non-face-to-face environment. In present, there are more clinical research protocols than clinical studies reporting the results, leading this study to end up just understanding the design and purpose of these papers inevitably. However, this research trend analysis would be helpful in preparing future studies and prescribing Mind-Body Intervention on COVID-19 in clinical scene. Also, further studies, including active control, are needed to determine the most ideal and cost-effective Mind-Body Intervention methods for participants suffering from COVID-19.

        한의학 치료 증진을 위한 사티어변형체계적치료 모델의 적용 - 국내 질적 연구 분석

        Objectives: The purpose of this study was to analyze domestic qualitative research papers of the Satir Transformational Systemic Therapy and suggest the possibility of applying it to Korean medicine counseling. Methods: Domestic research papers were gathered by keywords ‘Satire Counseling’ or ‘Satir Transformational Systemic Therapy’ and ‘Qualitative Research’ in RISS (http://www.riss.kr/), KISS (http:// kiss.kstudy.com/), OASIS (http://oasis.kiom.re.kr/). The results were seven papers. Seven papers were selected based on grounded theory and case studies. Researchers analyzed the phenomena during the course of counseling to discuss the mechanism of the Satir Transformational Systemic Therapy and consider its applicability to Korean medicine psychotherapy. Results: The Satir Transformational Systemic Therapy has three points: ‘Self’, ‘Communication’, and ‘Family’. Positive changes such as ‘Self-recovery’, ‘Consistent communication’, and ‘Recovery of relationships’ are derived from these three points. And techniques such as iceberg exploration, family maps, three-members in a born family, and the family sculpture are also effective means. Conclusions: The Satir Transformational Systemic Therapy is valuable in the clinical fields of Korean medicine counseling, especially relative to problems stemming from relationships.

        상담 장면에서의 명리의 활용에 대한 국내 연구 동향 분석

        Objectives: The aim of this study is to investigate current research trends of Four Pillars of Destiny and verify its values and potential in the counselling scene, as the Four Pillars of Destiny’s territory has been expanding to counselling, medical and psychiatric realm nowadays. Methods: The studies were searched from psychotherapy to general consultation, directly or indirectly related to counseling and Four Pillars of Destiny. Twenty-one published research studies were selected for analysis. The studies were categorized into 7 groups, meta-analysis, comparison with other personality tests, user’s trend analysis, utilization in job counseling, disease prediction study, utilization in treatment counseling, and use in Korean medicine. Results: The selected studies attempted to expand Four Pillars of Destiny’s usage through combination with other fields such as artificial intelligence, Korean medicine, and personality test. Furthermore by analyzing Four Pillars of Destiny itself to extract its key elements in counseling, such as therapeutic counseling factors and occupational counseling factors. Conclusions: At present, there are no standard use of Four Pillars of Destiny in counseling scene, for no large-scale research has been conducted or completed on this subject. This current status quo leads this paper to end up just understanding the counseling factors and possibilities of Four Pillars of Destiny rather than its psychological theory and clinical effect. However, this research trend analysis will be helpful in preparing future studies investigating Four Pillars of Destiny’s counseling effect, application in the counseling scene and its psychological theory. Also, further studies, including confirmation of the theory through the operational definition, prospective research, control study, statistical technique are required in order to evaluate Four Pillars of Destiny's psychological theory and its effects to verify its use in clinical scenes.

        사암침법의 정신과적 활용에 대한 질적 사례연구 -한 의료기관에 대한 현장연구-

        Objectives: To investigate validity and effectiveness of Saam five element acupuncture treatment in psychological problems. Methods: We selected a Korean medicine doctor known to apply Saam five element acupuncture treatment to the psychiatry field. An investigator visited the doctor’s clinic to observe the cases and interview patients. Duration of each interview was 30 minutes to one hour. A semi-structured questionnaire was used for in-depth interviews. All interviews were recorded and converted into texts. Converted texts were then summarized and analyzed qualitatively. Results and Discussion: Treatment process of Saam five element acupuncture designed for psychiatric use is divided into four stages: qi transformation (氣化), acupuncture treatment, deep breathing, and acupressure. In the first stage, patients are asked to express their feelings and emotions in a physical sense; to imagine their feelings have colors, weight, and physical form, and to describe those characteristics. Next, the doctor evaluates each patient’s qi according to the six qi (六氣), and then determines protocol of Saam five element acupuncture treatment. This treatment is focused on suffering of the patient and specific symptoms or emotions. The treatment duration is brief and changes in symptoms are frequently checked during treatment sessions. Without discussing details of stressful events, the doctor enables patients to express physical aspects of their feelings. He also provides them with psychoeducational resources. As a result, patients experienced immediate changes in body sensation, emotions and thinking, decreased response to triggers, and increased self-control after treatment. Conclusions: The Saam five element acupuncture treatment has the potential to be used as a treatment for psychiatric disorders. Treatment can be effective in managing psychiatric disorders when applied by primary Korean medicine clinics.

        교통사고 상해증후군 환자의 급성 스트레스 장애와 외상 후 스트레스 장애의 유병률과 한의변증유형 분석

        Objectives: To investigate the prevalence of acute stress disorder (ASD) and post-traumatic stress disorder (PTSD) in patients with traffic injuries. In addition, PTSD patients was classified using ‘pattern identification for jing ji and zheng chong'. Methods: Questionnaires such as the primary care PTSD screen for DSM-5(PC-PTSD-5), Korean version of PTSD checklist-5 (PCL-5-K), and the instrument of pattern identification for jing ji and zheng chong were conducted on 195 patients within 3 days to 1 year after traffic accidents. Patients were recruited from six medical institutions. Collected data were used to determine the prevalence of acute stress disorder and post-traumatic stress disorder. Results: On PC-PTSD-5, the prevalence was 39.1% for ASD and 50% for PTSD. On PCL-5-K, the prevalence was 20.4% for ASD and 29.3% for PTSD. Satisfying both PC-PTSD-5 and PCL-5-K, the prevalence was 18.2% for ASD and 25.8% for PTSD. As a result of pattern identification for jing ji and zheng chong, ‘weakness of heart and gall bladder type’ accounted for the highest proportions in both ASD and PTSD groups. Conclusions: In this study, the prevalence was 39.1% for ASD and 50% for PTSD by PC-PTSD-5. Satisfying both PC-PTSD-5 and PCL-5-K, the prevalence was 18.2% for ASD and 25.8% for PTSD. Further large-scale prospective studies are needed to analyze the prevalence of ASD and PTSD, the rate of progression from ASD to PTSD, and the type of pattern identification.
