갑상선암의 경부 림프절 전이 진단을 위한 미세침세척액 티로글로불린 측정법

갑상선암 환자의 경부 림프절에서 미세침흡인생검술(fine needle aspiration biopsy, FNAB)을 시행하며 얻은 미세침세척액의 티로글로불린(thyroglobulin, Tg)측정법의 진단적 성적과 유용성을 알아보고자 FNAB 세포검사의 결과와 비교하였다. 갑상선암의 경부림프절 전이가 의심되어 초음파 시행한 총 37명의 환자 48개의 FNAB 검체를 대상으로 하였고, 수술 후 조직검사결과 또는 장기간 영상 추적검사를 기준으로 하여 세포검사와 미세침세척액 티로글로불린(FNAB-Tg)측정법을 비교하여, 각 검사의 특이도, 민감도 및 정확도를 평가하였다. 추가적으로 Tg-항체가 양성일때 FNAB-Tg 검사에는 어떠한 영향을 미치는지 Tg-항체 검사를 같이 시행한 34명을 대상으로 분석하여 보았다. FNAB 검체의 세포검사에서 전이 음성인 36개 검체 중 35개 검체에서 음성을 보였고, 전이 양성인 12개 검체 중 9개 검체에서 양성을 보였다. FNAB-Tg측정법의 경우 전이 음성인 36개의 검체 중 32개의 검체에서 음성을 보였고, 전이 양성인 12개의 검체 중 12개 검체 모두 양성을 보였다. 따라서 세포검사의 민감도와 특이도는 각각 75.0% (9/12)와 97.2% (35/36)였고, 위음성이 25% (3/12)였다. 반면에 FNAB-Tg 측정법의 민감도와 특이도는 각각 100% (12/12)와 88.9% (32/36)였고, 위양성이 11.1% (4/36)였다. 정확도는 두 검사 모두 91.7% (44/48)였다. Tg-항체에 대한 검사 결과를 얻을 수 있는 34명을 따로 분석한 결과, Tg-항체의 존재 유무가 FNAB-Tg 측정법의 진단 성능에 영향을 주지 않음을 확인할 수 있었다. FNAB 검체를 이용한 세포검사는 특이도가 높고 반면, FNAB-Tg 측정법은 민감도가 높은 검사임을 확인할 수 있었다. 따라서 림프절 전이가 의심되는 갑상선환자에서 FNAB를 시행할 때 세포검사와 FNAB-Tg 측정법은 동시에 시행되어야 할 상호보완적인 검사들이다. Purpose: Several studies report that detection of thyroglobulin (Tg) in fine-needle aspiration (FNA) biopsy washout fluid from lymph nodes identifies recurrences or metastases of differentiated papillary thyroid cancer (DPTC) in the neck with higher sensitivity and specificity than fine-needle aspiration cytology (FNAC). We evaluate the diagnostic efficacy and usefulness of Tg measurement in FNA washout fluid (FNA-Tg) and compare with FNAC. Materials and Methods: Forty-eight FNA samples of 37 patients who undergone ultrasonography to detect cervical lymph node metastasis of DPTC, were included for this study. Lymph node metastasis was confirmed by histopathologic examination or long-term imaging follow-up. Sensitivity, specificity and accuracy of FNA-Tg and FNAC were calculated. In 34 patients, we evaluated diagnostic accuracy of FNA-Tg according to the presence or absence of Tg antibody. Results: Sensitivity, specificity and accuracy of FNAC were 75.0%, 97.2% and 91.7%, respectively, and those of FNA-Tg were 100%, 88.9% and 91.7%, respectively. The presence of Tg antibody was not relevant to the diagnostic accuracy of FNA-Tg. Conclusion: FNA-Tg is a as accurate as FNAC with higher sensitivity. FNA-Tg and FNAC are complement techniques for diagnosing lymph node metastasis of DTPC.
하니컴 샌드위치 복합재를 적용한 자동무인경전철 차체 구조물의 구조 안전성 및 충돌 특성 평가 연구

This paper describes the results of structural integrity and crashworthiness of Automate Guideway Transit(AGT) vehicle made of sandwich composite. The applied sandwich composite of vehicle structure was composed of aluminum honeycomb core and WR580/NF4000 glass fabric/epoxy laminate composite facesheet. Material testing was conducted to determine the input parameters for the composite facesheet model, and the effective equivalent damage model for the orthotropic honeycomb core material. The finite element analysis using ANSYS v11.0 was done to evaluate structural integrity of AGT vehicle according to JIS E 7105 and ASCE 21-98. Crashworthiness analysis was carried out using explicit [mite element code LS-DYNA3D. The results showed that the structural integrity and crashworthiness were proven under the specified loading and crash conditions. Also, the modified Chang-Chang failure criterion was recommend to evaluate the failure modes of composite structures after crashworthiness event.
자동무인경전철 복합재 차체 구조물의 구조 시험 및 해석적 평가

This paper describes the results of structural test and finite element analysis for rubber wheel-type Automatic Guideway Transit(AGT) made of aluminum honeycomb sandwich composites with WR580/NF4000 glass-fabric epoxy laminate facesheets. The static tests of vehicle structure were conducted according to JIS E7105. These static tests have been done under vertical load, compressive load and 3-point support load. The structural integrity of AGT vehicle structure was evaluated by displacement, stress obtained from LVDT and strain gauges, and natural frequency. And finite element analysis using Ansys v11.0 was done to compare with the results of static test. The result showed that the results of structural integrity for static test were in an good agreement with these of finite element analysis.
모노레일 대차프레임에 대한 구조 안전성 및 피로강도 평가에 관한 연구

In this paper, the structural integrity and fatigue strength for the bogie frame of Monorail being developed in domestic was evaluated. Presently, the standard of evaluation for the bogie frame of monorail was not regulated. Therefore, the evaluation of the structural integrity and fatigue strength for the bogie frame was performed on the basis of the UIC 615-4 standard. The structural integrity of the designed bogie frame was evaluated by displacement and Von-Mises stress under each load conditions. And the fatigue strength was evaluated by combined main in-service load conditions specified at UIC 615-4 standard and it was compared with result of fatigue analysis using winLIFE v3.1 with the function of batch processing. The results shows that the structural integrity and fatigue strength of the designed bogie frame was satisfied, and the fatigue analysis using batch processing was more effective than conventional fatigue analysis using combined load conditions.
철도차량용 직물 유리섬유/에폭시 적층 복합재의 피로수명 향상 방안 연구

본 연구는 철도차량에 적용되는 직물 유리섬유/에폭시 적층 복합재의 피로특성 및 피로수명에 대해 평가하였다. 이때, 피로시험은 0.1의 응력비(R)와 5 ㎐의 주파수에서 인장-인장 하중 조건이 고려되었으며, 시험편은 카본/에폭시 플라이가 보강된 시험편과 보강되지 않은 시험편 두 가지 타입을 고려하였다. 또한, 직물 유리섬유/에폭시 적층 복합재의 피로수명을 평가하기 위해 철도차량 차체와 언더프레임에 주로 사용되는 알루미늄 6005와 비교하였다. 본 연구를 통하여 3장의 카본/에폭시 플라이가 보강된 직물 유리섬유/에폭시 적층 복합재 시험편이 보강되지 않은 기본 시험편보다 피로강도 및 피로수명이 크게 향상됨을 확인하였다. In this study, the fatigue characteristics and life of a woven glass fabric/epoxy laminate composite applied to railway vehicles was evaluated. The fatigue test was conducted using a tension-tension load with a stress ratio R of 0.1 and frequency of 5 ㎐. Two types of woven glass fabric/epoxy laminate composite was used in the fatigue test: with and without carbon/epoxy ply reinforcement. In addition, the fatigue life of the woven glass fabric/epoxy laminate composite was compared with that of aluminum 6005, which is used in the car body and underframe structures of railway vehicles. The test results showed that the failure strength and life of the woven glass fabric/epoxy laminate composite reinforced with three carbon/epoxy plies had a remarkable improvement compared with that of the bare specimen without reinforcement.
바이모달 트램 구동축 서브프레임에 적용된 용접 사각관의 굽힘 특성 평가

This paper describes the results of experiments on the bending behavior of welded square tube structures for drive shaft sub frame of BIMODAL Tram. The used specimens to test were two different type made of ATOS60 and ST52-3. The square tube made of ST52-3 is the non-welded structure, while the square tube made of ATOS60 is the welded structure. The welded square tube made of ATOS60 will be applied for drive shaft sub frame of BIMODAL Tram. This welded square tube could reduce the manufacturing cost in comparison of non-welded square tube. The results showed that the bending performance of welded square tube made of ATOS60 was proven, and the bending behavior was in an good agreement with that of nonlinear finite element analysis.
