        Cellulose를 생산하는 Acetobacter xylinum GS11의 분리·동정

        산업적으로 유용한 미생물 유래 셀룰로오스를 생산, 이용하기 위해 전국 각지 양조식초의 덧으로부터 세룰로오스의 생산성이 높고, 조질의 균일설을 나타내는 균주를 분리하였다. 분리균 GS11은 gram 음성이고, 간균(0.6×2.2~3.2㎛)의 형태를 하고 있으며, 편모를 가지고 있어 운동성을 보였다. 또한 세포내 지방산 조성은 다량의 불포화 지방산 C_18:1과 포화지방산 C_16:0, C_14:0이 대부분을 차지하였고 DNA 염기조성(G+C) 함량은 58.4% 이였으며, ubiqunone은 Q_10을 갖는 것으로 나타났다. 이러한 형태학적, 생리·생화학적 특성의 결과에 따라 본 균주는 Acetobacter xylinum GS11으로 동정되었다. A. xylinum GS11의 배양기간 동안 셀룰로오스 생산성을 검토하고자 250mL 삼각플라스크에 균주를 접종하여 30℃에서 12일간 정치 배양하였다. 그 결과 기질인 glucose의 소비는 접종 후 급속하게 감소하여 셀룰로오스 생산에 이용되었으며, 셀룰로오스의 생산은 배양 9일 경에 2.8 g/l로 최대의 생산량을 나타냈다. To produce and utilize microbial cellulose being industrially useful, bacterium posessing a high ability to produce cellulose and the uniform structure of cellulose pellicle was isolated from the traditionally fermented vinegar in korea. The isolated strain, GS11 was gram-negative, rod(0.6×2.2~3.3㎛)and motile. It also had 58.4% of DNA base composition, posessed straight-chain C_18:1, and C_14:0 fatty acid, and contained ubiquinone Q_10. According to its morphological, physiological and biochemical propertiews, the isolated strain was identified as Acetobacter xylinum. A. xylinum GS11 was cultured for the study of cellulose productivity during the static culture, 12 days at 30℃. As this result, the amount of glucose was quickly decreased, and then used as substrate for cellulose production. In this culture, the maximal production of cellulose peaked with 2.8 g/l after 8 days.

        Enhanced Production of Phenolic Compounds from Pumpkin Leaves by Subcritical Water Hydrolysis

        Enhanced production of individual phenolic compounds by subcritical water hydrolysis (SWH) of pumpkin leaves was investigated at various temperatures ranging from 100 to 220oC at 20 min and at various reaction times ranging from 10 to 50 min at 160oC. Caffeic acid, p-coumaric acid, ferulic acid, and gentisic acid were the major phenolic compounds in the hydrolysate of pumpkin leaves. All phenolic compounds except gentisic acid showed the highest yield at 160oC, but gentisic acid showed the highest yield at 180oC. The cumulative amount of individual phenolic compounds gradually increased by 48.1, 52.2, and 78.4 mg/g dry matter at 100oC, 120oC, and 140oC, respectively, and then greatly increased by 1,477.1 mg/g dry matter at 160oC. The yields of caffeic acid and ferulic acid showed peaks at 20 min, while those of cinnamic acid, p-coumaric acid, p-hydroxybenzoic acid, and procatechuic acid showed peaks at 30 min. Antioxidant activities such as 2,2-diphenyl-1-picrylhydrazyl and ferric reducing antioxidant power values gradually increased with hydrolysis temperature and ranged from 6.77 to 12.42 mg ascorbic acid equivalents/g dry matter and from 4.25 to 8.92 mmol Fe2+/100 g dry matter, respectively. Color L* and b* values gradually decreased as hydrolysis temperature increased from 100oC to 140oC. At high temperatures (160oC to 220oC), L* and b* values decreased suddenly. The a* value peaked at 160oC and then decreased as temperature increased from 160oC to 220oC. These results suggest that SWH of pumpkin leaves was strongly influenced by hydrolysis temperature and may enhanced the production of phenolic compounds and antioxidant activities.

        유방암 수술 환자에 대한 가정간호서비스의 경제성 평가

        Purpose: This study conducted an economic evaluation of hospital-based home care services for the patients who had undergone breast cancer surgery. Methods: A total of 12,483 patients over 18 years of age who had received breast cancer surgery in 26 tertiary hospitals in 2018 were analyzed with the claim data from the Health Insurance Review & Assessment Service using cost-minimization analysis and societal perspectives. Results: There were 156 patients who utilized hospital-based home care services within 30 days after breast cancer surgery, and they received 2.17 (SD=1.17) hospital-based home care service on average. The average total cost was 5,250,028 KRW (SD=1,905,428) for the group receiving continuous hospital-based home care and 6,113,402 KRW (SD=2,033,739) for the group not receiving continuous hospital-based home care (p<.001). The results of the economic evaluation of continuous hospital-based home care services in patients who had undergone breast cancer surgery indicated a total benefit of 953,691,000 KRW, a total cost of 819,004,000 KRW, and a benefit-cost ratio of 1.16 in 2018. Conclusion: Continuous hospital-based home care was considered economically feasible as the total costs for the group receiving continuous hospital-based home care were lower than those of the group not receiving continuous hospital-based home care. Therefore, policy modification and financial incentives are recommended to increase the utilization of hospital-based home care services for patients who had undergone breast cancer surgery.

        건강보험 자료를 이용한 전국 의료기관 가정간호 실시 및 이용 현황 분석: 2008년-2017년

        Purpose: The purpose of this study is to analyze the status quo of hospital-based home care utilization in Korea from 2008 to 2017. Methods: Data from a total of 1,396 medical institutions, 350,390 patients, and 3,563 home care nurses were analyzed using claim data from the Health Insurance Review & Assessment Service. Results: The number of hospital-based home care agencies decreased from 177 in 2008 to 115 in 2014. This number started to increase in 2015 and reached 179 in 2017. The number of hospital-based home care patients declined from 35,056 in 2008 to a low of 26,848 in 2013. This number started to increase in 2014 and reached 67,863 in 2017. Essential hypertension was the most common disease among hospital-based home care patients from 2008 to 2015. The number of hospital-based home care visits declined from about 500,000 in 2008 to a low of 362,000 in 2013. This number started to increase in 2014 and reached 658,000 in 2017. Conclusion: It is necessary to vigilantly monitor hospital-based home care agencies, patients, and the utilization of services. This may help establish platforms for providing community and home-based nursing services for the super-aged society in Korea.

        증강현실 기반의 수술간호 실습교육 애플리케이션 개발 및 평가

        본 연구는 증강현실 기반 수술간호 실습 교육 애플리케이션을 개발하기 위한 방법론적 연구로서, 연구과정 은 ADDIE 모형에 따라 분석, 설계, 개발, 구현, 평가의 5단계로 구성되었다. 요구도 분석 결과 나온 세부 항목 중 전문가 집단과 간호대학생 집단에서 공통적으로 요구도가 높은 항목은 수술 기구 항목이었다. 요구도를 기반으 로 수술실에서 가장 빈번하게 사용하며 진료과에 구분 없이 보편적으로 사용되는 7가지 분류의 51개의 수술기구를 선정하여 AR Book으로 제작하였다. 증강현실기반 애플리케이션은 Unity 3D 엔진을 기반으로 제작하였으며, Android OS로 애플리케이션을 빌드하여 대상자가 이용할 수 있도록 하였다. 전문가 평가 결과 전체 평균은 73.4±4.3으로 전체적으로 높게 나타났으며, 학습자 만족도 조사 결과 높은 만족도를 보였다. 제작된 증강현실 기 반 수술간호 콘텐츠는 이전의 다른 학습 방법과는 달리 현실성과 휴대성, 접근성, 간편함을 갖춘 새로운 방식의 수술 기구 학습 애플리케이션을 처음으로 개발한 부분에 그 의의가 있다. The purpose of this research was to develop an augmented reality-based surgical nursing practice education application. The research proceeded in ADDIE order of analysis, design, development, implementation, and evaluation stages. In the analysis phase, we analyzed the surgical nursing learning needs of the expert group and nursing student group. As a result of the analysis, the demand for surgical instruments was generally high. We selected 51 surgical instruments of the 7 categories most frequently used in the operating room based on the needs, and produced them as AR Book. The augmented reality application was created based on the Unity 3D engine, and the application was built with Android OS so that the target person could use it. Expert assessment of application for augmented reality-based surgical nursing practice education application showed 73.4±4.3 out of 80. This study is significant in the part that first developed a new method of learning tools for surgical instruments. We determined that an augmented reality-based application has reality, portability, accessibility and simplicity.

        간호대학생의 생명의료윤리 원칙 인식에 대한 분석

        Objectives This study was attempted to identify and systematically understand how nursing students perceive the principles of biomedical ethics. Methods Through an open questionnaire, first-year nursing students who took a biomedical ethics lecture for one semester were asked to choose the most important principle among the four principles of biomedical ethics and write the rationale for it. A total of 99 subjects were analyzed using the content analysis method. Results Among the four biomedical ethics principles, the importance distribution was shown in the order of principle of respect for autonomy(45.5%), principle of nonmaleficence(29.3%), principle of beneficence(20.2%), and principle of justice(5.0%), and 14 themes and 42 statements supporting the basis were revealed. There were a total of 15 statements on the principle of respect for autonomy, and 4 themes were included: inherent rights, right to choose, right to know, and right to be respected(45.5%). There were a total of 15 statements on the principle of nonmaleficence, including 4 themes: avoiding harm, minimizing suffering, basic duties, and licensing as a nurse(29.3%). There were a total of 9 statements on the principle of beneficence, and 4 themes were included: the role of medical personnel, use of professional knowledge, active help, and human well-being(20.2%). There were a total of 3 statements on the principle of justice, including 2 themes: establishment of standards and fairness(5.0%). Conclusions It was confirmed that the students showed an active attitude to solidify their moral values and to cultivate a sense of ethics. In the undergraduate curriculum, not teacher-centered lectures, but learner-centered bioethics education that reflects students' understanding and feedback should be provided.
