성장호르몬 결핍증과 특발성 저신장증 환아에서 성장호르몬의 치료 효과

Purpose:This study was designed to evaluate the effect of growth hormones on children with growth hormone deficiency(GHD) or idiopathic short stature(ISS). Methods:Between January 1988 to July 2003, 45 patients(M26, F19) with GHD and 24 patients (M13, F11) with ISS were enrolled in this study. Height standard deviation score(Ht SDS) for chronological age(CA) and Ht SDS for bone age(BA) were obtained for each patient upon diagnosis and after growth hormone(0.1-0.15 IU/kg) was injected subcutaneously daily. Results:Ht SDS for CA was -2.06±0.23 before treatment, -1.60±0.21 at one year, -1.52±0.23 at two years, -1.61±0.28 at three years, -1.60±0.31 at four years, and -1.54±0.32 at five years, showing a statistically significant increase for five years(P<0.05). Meanwhile, the values were -2.03±0.55 before treatment, -1.44±0.66 at one year, -0.14±1.06 at two years, -0.68±1.27 at three years, -1.16±0.96 at four years, and -1.37±0.94 at five years, in ISS, showing a statistically significant increase for the first three years(P<0.05) only. Ht SDS for BA in GHD was -0.53±0.19 before treatment, -0.39±0.18 at one year, -0.32±0.20 at two years, -0.43±0.22 at three years, -0.39±0.19 at four years, and -0.32±0.22 at five years, not showing a decrease, and the decrease in ISS was statistically not significant(P≧0.05). Conclusion:Both groups exhibited significant increase in Ht SDS through growth hormone treatment, proving its efficacy. As for the evaluation of growth-related factors, the 1st year increase of Ht SDS was the most important factor in evaluation of growth effect in both groups. However, further study is required to investigate the effect of GH therapy on ISS. 목 적 : 1985년 이후 유전자 재조합 성장호르몬(rh-GH) 제제는 성장호르몬 결핍증(GHD) 이외에 비결핍성 저신장증(non- GHD)에서도 사용되어져 왔다. 이에 저자들은 GHD와 특발성 저신장증(ISS)으로 진단된 환아들에게 GH을 투여하여, 전후의 성장 상태 평가와 성장에 영향을 주는 인자를 알아보고자 본 연구를 시행하였다.방 법 : 1988년 1월부터 2003년 7월까지 연세의료원에서 GHD 45명(M26, F19), ISS 24명(M13, F11)을 대상으로 진단 시와 GH 치료(0.1-0.15 IU/kg/day) 후 역연령에 대한 신장 SDS 와 골연령에 대한 신장 SDS를 측정하고, 인슐린양 성장인자(IGF-1), 인슐린양 성장인자 결합단백-3(IGFBP-3) 농도를 측정하고 분석하였다. 예측 성인 신장치는 Bayley and Pinneau method를 이용하였다.

This study is to analyze the case of Yongsan station-area development project appearing rapid process speed, and find that the way reinvesting income of development project based on profitability analysis is to be effective investment plan. Prior to profitability analysis, It selects the international office facility of investment buying object based on analysis of Yongsan station-area development project. By proving this, It draws that income from Yongsan-station area development project is reinvested by the way buying office building built in Yongsan international office district. As a result of analyzing the profitability, Reinvesting income from Yongsan-station area development project in office building achieves higher profit rate than holding a cash, bond investment, fixed deposit, debt redemption. This result shows that investing the office building is not only enabled to create a stable profit but also improves the business profitability highly if borrowing the debt by low cost using the good credit grade of korail. Therefore, This study shows that reinvesting office building of railroad-station area development project is able to be a new profit model and solve KORAIL's sales deficit.

최근 NFC(Near Field Communication) 기반의 통신 기능을 채용한 스마트기기 및 OS 등이 확산되면서, 기존의 RFID를 이용한 다양한 응용분야가 NFC로 대체되고 있다. Smart NFC 기술은 기존 서비스 및 기기와 쉽게 융·복합이 가능하며, 스마트폰을 활용한 결제·의료·인증 등 새로운 Network Communication 패러다임의 창출이 예상되고 있다. Smart NFC 보안 인증기기(Smart NFC Security Authenticator: SNSA)의 H/W와 S/W를 개발하고, 무선통신 시험결과 허용기준값 이내를 만족하고, 토폴로지 영향의 실험 및 분석 결과, Daisy Chain 토폴로지가 Star 토폴로지에 비해 신호성능이 우수함을 보였다. As smart devices and OS with communication capabilities based latest NFC (Near Field Communication) have been spreaded, many applications with using existing RFID are being replaced to NFC. Smart NFC technology and existing services and devices can be easily combined convergence and advantage of smart phones, such as authentication and billing, medical care, the creation of a new paradigm of Network Communication are to be expected. By developing H/W, S/W of the Smart NFC Security Authenticator(SNSA), satisfying with wireless communication test results within accepted reference value, analyzing and testing the impact of topology, the signal performance of Daisy Chain Topology was much better than Star Topology's.

최근 녹색정보기술(G-IT), 녹색유비쿼터스기술(G-UT) 산업분야의 전문화에 따른 보안 방범 유지를 위한 국가 주요기관 시설물 또는 주요 산업 시설물 등의 보안 중요성이 강조되면서, 정부관공서 기업 및 군부대내에 최첨단 기술을 접목한 보안시스템(Security System) 구축이 절실히 요구되는 실정이다. 본 논문은 경비구역내의 경보발생장치로부터 신호를 다양한 형태의 통신기술로 수신하여, TCP/IP 망을 통하여 로컬 관제 센터와 원격관제 서버로 전송하는 녹색 유비쿼터스 지능형 무인경비 다중화장비에 관한 연구이다. 본 연구를 통하여, 무인경비 시스템의 통합 솔루션(Total Solution)을 확보하여 상호 시너지 효과 (Synergy Effect)가 가능하며, 국내외 시장 확대에 크게 기여할 것이다. 녹색 유비쿼터스 환경 구축 사업(Gu-Home, Gu-City, Gu-Health 등)중에서 방범/경비 분야에 적용할 수 있으며, 녹색 유비쿼터스 비전(Gu-Vision)을 제시할 수 있는 국가경쟁력을 갖춘 기업확장에 기여할 것으로 사료된다. Recently emphasizing the importance of security such as national major institutions' facilities or major industrial facilities for the maintenance of security/crime prevention due to specialization of the Green Information Technology(G-IT) and Green Ubiquitous Technology(G-UT) industry fields, The Security System Building linked high technology within the government-related organization, enterprise and army the military is urgently required. This paper is about the green USN intelligent an unmanned guard MUX system that receive the signals, from alarm device within surveillance area, with various ways of communication techniques and then transmit to local control center and remote control server trough TCP/IP network. This study enables the mutual senergy effect by realizing a total solution of an unmanned guard system and also significantly contributes to the global/domestic market expansion. That can be applied to the crime prevention/security fields in the Green Ubiquitous Environment Implemented Business(survalance-Home, Gu-City, Gu-Health, etc.), and will contribute to expand companies with international competitiveness that can provide the Green Ubiquitous Vision(Gu-Vision).

We experienced a case of typical Kearns-Sayre syndrome in a 16-year-old boy who suffered from bilateral ptosis for 7 years and dizziness for 3 years. He had total external ophthalmoplegia, atypical pigmentary retinopathy, complete AV block (the location of which was proven infra-His by His bundle recording), short stature, kyphoscoliosis, sensorineural hearing impairment, progressive cerebral atrophy, and a slight increase of protein in CSF. Electronmicroscopy of endomyocardial biopsy specimen from the RV revealed aggregations of swollen mitochondria often showing lass of cristae and containing myeloid bodies and lipid droplets or lipofuscin granules in the cytoplasm and mitochondria. To prevent sudden death and improve exercise capacity, a permanent pacemaker (Quadra, Telectronics) was implanted and programmed into a VAT mode.
