        제1형 당뇨병, 소뇌 실조, 진행성 난청을 수반하는 MELAS 증후군 환자 1예

        Mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) syndromeis a maternally inherited mitochondrial disorder. MELAS syndrome manifests different clini-cal symptoms due to mitochondrial heteroplasmy and individual susceptibility and tends to beaggravated by metabolic events. It presents various clinical manifestations including hearingimpairment, diabetes mellitus, and stroke like episodes. However, to date, only a few cases ofMELAS syndrome accompanied with cerebellar ataxia have been reported in Korea. Wehereby present an unusual case of a patient diagnosed with MELAS syndrome involving dia-betes mellitus, cerebellar ataxia and sensorineural hearing loss, which were aggravated bymetabolic stresses such as pregnancy and hypoglycemic shock.

        Genetic Analysis of Locally Advanced Well-Differentiated Thyroid Cancer: Clinical Prognostic Implications

        Background and Objectives Despite the usually favorable prognosis of well-differentiatedthyroid cancer (WDTC) following appropriate treatment, advanced T-staged WDTCs are as-sociated with poor prognosis. This study focused on identifying genes associated with theprognosis of locally advanced WDTC by analyzing a The Cancer Genome Atlas cohort. Subjects and Method We analyzed the data of 501 patients with WDTC and classifiedthem into two subgroups: pathological T4 stage (Cluster 1) or T1-3 stage (Cluster 2). We com-pared the mRNA expressions of thyroid cancer-related genes, and the somatic mutation fre-quencies of various cancer genes between the two subgroups. Results Cluster 1 included 23 patients (papillary=21/follicular-variant papillary thyroid can-cer [FVPTC]=2) and Cluster 2 478 patients (papillary=371/ FV PTC=10 0/ot her s=7). Cluster 1showed worse overall and disease-free survival than Cluster 2 ( p<0.05 and p=0.12 , respe c-tively). Patients with pT4 stage had about a 1.8-fold increased risk of overall recurrence ordeath. MET, SERPINA1, TIMP1, PROS1, FN1, CDKN2A, and CDKN2B were significantly ele-vated while TG, DNAH9, TFF3, CR A BP1, TPO, JAK2, KIT, KDR, and NFE2L2 were signifi-cantly lower in Cluster 1 (all, p<0.05 and adjusted p<0.05). A TERT, EIF1A X, and ATMshowed significantly frequent somatic mutations in Cluster 1 when compared to Cluster 2. Wealso identified seven pathways related to 16 genetic markers. Conclusion Locally advanced WDTC presented 16 genetic alterations. We identified so-matic mutations associated with local invasion transformation. Relevant pathways related tothe 16 genetic markers could be therapeutic targets. Genetic analysis of locally advancedWDTC may be used to predict clinical applications in patient survival.

        면역약침을 병행한 만성 전립선염/만성 골반통 증후군 환자 치험 1례

        Objectives The purpose of this study is to report the clinical effects of immuno pharmacopuncture and herbal medicine Simmidojeok-san on chronic non-bacterial prostatitis. Methods The patient was treated with Simmidojeok-san ; 0.2 cc of V pharmacopuncture injection was given into Jeonyang and 0.6 cc of V pharmacopuncture injection was administered into Hoeeum (CV1). National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) and Numerous Rating Scale (NRS) were used to assess treatment effect. Results NIH-CPSI and NRS decreased after treatment. Conclusions The herbal medicine Simmidojeok-san and V pharmacopuncture therapy are effective in treating chronic non-bacterial prostatitis.

        감기 치료 빈용 처방 구성 약물의 효능 연구

        Objectives: To investigate the anti-inflammatory, analgesic, anti-pyretic and anti-histamine activities of 20 herbal medicines to test their efficacy in common cold treatment. Methods: For all experiments, the herbal medicines were extracted with 80% ethanol and freeze-dried. To determine the anti-oxidative properties, we tested DPPH-free radical-scavenging activity and xanthine oxidase inhibitory activity. To determine anti-inflammatory and analgesic potential, we investigated acetic acid-induced vascular permeability and writhing test in ICR mice. For anti-pyretic activities, an LPS-induced pyrexia study was conducted in rabbits. To evaluate the anti-histamine activity, we examined compound 48/80-induced systemic anaphylaxis in ICR mice and the release of β-hexosaminidase on rat basophilic leukemia (RBL-2H3) cells. Results: Ephedrae herba, Forsythiae fructus, Cinnamomi ramulus, and Cimicifugae rhizome showed potent free-radical scavenging activities. Gentianae macrophyllae radix inhibited acetic acid-induced vascular permeability. Schizonepetae spica and Cimicifugae rhizome inhibited acetic acid. Cinnamomi ramulus and Angelicae decursivae radix inhibited LPS-induced pyrexia. Angeliace dahuricae radix and Asari radix inhibited compound 48/80. Scutellariae radix, Cinnamomi ramulus, Ephedrae herba, and Zingiberis rhizoma crudus potently inhibited the release of β-hexosaminidase. Conclusions: We examined the anti-inflammatory, analgesic, anti-pyretic and anti-histamine activities of 20 herbal medicines;We examined the anti-inflammatory, analgesic, anti-pyretic and anti-histamine activities of 20 herbal medicines Codonopsis pilosulae radix, Zingiberis rhizoma crudus, and Cinnamomi ramulus showed novel efficacy. These results suggest that some of herbal medicines may be very effective in treating common cold.
