Violence in our society, combined with improving transport system, resulted in increased numbers of patients with cardiac wounds reaching the hospital alive. Most patients with penetrating cardiac injury, rather than blunt injury, present with a syndrome of either hemorrhagic shock or cardiac tamponade. And they should be operated upon as soon as possible. Often the atrioventricular valves and other important cardiac structures are also damaged by the penetrating instruments or missile. Both intracardiac communications and atrioventricular fistulas may result in significant left-to-right shunts accompanied by congestive heart failure, necessitating surgical correction. Usually, retained cardiac foreign bodies, which are almost always bullets or fragments of missiles, may lie within a cardiac chamber or in the myocardium. Emboli of bullets or other missiles from distant sites to the right side of the heart are numerous enough to require attention. Recently we experienced a case with intracardiac foreign body due to penetrating cardiac injury. A 19 year-old man was admitted to our hospital due to penetrating anterior chest wound by iron segment. The roentgenogram of the chest revealed a radio-opaque metallic shadow in left lower chest around the cardiac apex, mild blunting of left costophrenic space, but no cardiomegaly. During operation the foreign body was noted to be present in the cardiac chamber by the portable C-arm fluoroscopy. But during the manipulation it moved into left inferior pulmonary vein from left ventricle by way of left atrium. So we could manage to remove it from left inferior pulmonary vein by direct approach to the vein. It was iron segment, sized 0.lcm x0.6cmx0.5cm, with sharp margins. The patient had an uneventful postoperative recovery except for chylopericardium and was discharged.

Superior hypogastric plexus block (SHGPB) with neurolytics, such as absolute alcohol or phenol, have been known to be highly effective in patients with pelvic cancer affected by somatic and segmental pain or by perineal pain. However, these neurolytics can cause direct tissue damage to occur by spread of the neurolytic solution to the surrounding area. In the case of a 59-year-old female, who has suffered from severe lower abdominal pain (VAS 9-10) due to the metastasis of cervical caner, SHGPB was successfully performed SHGPB using a percutaneous radiofrequency thermocoagulation method (less than VAS 3). It is suggested that SHGPB by radiofrequency thermocoagulation appears to be a safe and effective procedure for the management of lower abdominal pain related to malignancy, without the complication of neurolytics.

Topical epinephrine is useful to reduce bleeding and to give a clear operative field during general anesthesia. However, epinephrine at clinical doses has adverse cardiovascular effects, such as transient hypertension and arrhythmia. We report a case of cardiac arrest following the local infiltration of epinephrine during desflurane anesthesia. The patient recovered without sequelae after intensive care. However, it is recommended that caution is exercised when the local infiltration of epinephrine is used during general anesthesia. (Anesth Pain Med 2011; 6: 41∼44)

민간 조종사 전환과정 중 업무안배 능력 향상을 위한 조종역량 요인 분석연구

조종사의 역할은 항공사의 안전에 절대적으로 중요하다. 초기 비행훈련 단계에서 군과 민간 출신별 실제 비행에 필요한 조종사 역량 습득에 차이가 있는지를 확인하기 위해, 본 연구에서는 배경별 교육훈련의 진행상황과 효과를 분석하여 차별화된 역량기반 교육을 연구하고자 한다. 특히, 업무안배 능력은 다른 조종역량에 대한 이해가 바탕이 되어야 하여, 타 조종역량이 업무안배에 미치는 효과를 분석하여 교육 방안을 제시하고자 한다. 이를 위해 군과 민간 출신에 따라 ICAO가 정의한 8개 역량 중 정기 숙련도 점검 결과에 차이가 있는지 여부를 판단하며, 군과 민간 출신의 평균 차이 및 두 집단의 업무안배에 미치는 조종역량 요인을 분석하였다. 분석결과 업무안배에 미치는 요인으로서 의사소통 능력과 상황인식 능력이 중요한 요인으로 분석되었으며, 군출신의 경우는 상황인식 능력이 뛰어날수록 업무안배 능력이 높은 것으로 나타나는 반면, 민간출신의 경우 의사소통 능력이 뛰어날수록 업무안배 능력이 높아지는 것으로 나타남에 따라 업무안배 교육에 출신에 따른 차별화된 교육이 필요한 것으로 나타났다.

한·일 프롤레타리아 시에 나타난 주제 의식 고찰

프롤레타리아문학은 문학을 수단으로 하여 무산계급 해방을 목적으로 하는 운동 중심의 문학형태이다. 이를 위해 조직된 KAPF(한국)와 NAP(일본) 작가들은 하나의 창작이론을 지침으로 하여 작품활동에 임했으며 근저엔 맑시즘을 사상적 배경으로 삼고 있다. 논쟁을 통해 전개된 여러 비평 가운데 그 중 폭넓게 진행된 것은 대중화론이다. 프로문단의 성패가 대중적지지 여부에 달려 있기에 이들에게 계급적 이데올로기를 주입시키기 위하여 창안된 이론이다. 양국 모두 10 개항으로 결의했는데 한국의 경우는 일본 것을 고스란히 답습한 양상이다. 본고는 10 개 항목에서 가장 많이 작품으로 형상화된 4 개 항목의 작품들을 주제적 차원에서 고찰하였다. 노동현장과 반봉건적 농촌문제, 제국주의 전쟁 반대 그리고 국제적인 연대감 등이 그것이다. 노동현장과 관련해서는 작업장의 참상 고발을 다루기도 했으나 이에 그치지 않고 노동파업을 제제로 삼아 예술과 정치의 병행이라는 아지·프로로 발현된다. 프로시의 과반수 이상을 차지하는 아지·프로시는 무산계급의 주체세력 이 노동자임을 반증한다. 반봉건적 농촌현실 개혁은 지주와의 소작쟁의로 나타났는데 핵심 동인은 소작료 인상이었다. 세계 5대 강국으로 제국주의 침략 전쟁을 반대하는 반전시는 일본 작가들에만 사용되었고 한국은 피지배민족으로서 사회여건이 허락지 않았다. 한일 양국 외에 세계 프롤레타리아와 국제적인 연대감은 프로문단이 지닌 국제적 추수주의에 기인한다. Korean Proletarian Literature was affected by Japanese one. And Korean writers followed the literary theory of Japanese ones in the same way. So we have considered that the theme so is the same between two countries, but this thesis trys to be different from other. Proletarian circles used not only the literature as a political means for breaking classes but also struggled against the bourgeoisie through the world. The literary companies like NAPE(Japan) or KAPF(Korea) are formed and they did literary activities as a movement not a pure literature. Most Japanese outhors joined these companies belonged to the communist Party and Korean ones were also under the communism based on Marxian-Leninism. The common themes of two contries are reality of labor's life, agrarian dispute and inspiration of national collective, especially labor's strike is focused. This reflects the main communists consisted of labors and labor's groups were already organized. Japanese poem specially tells us about pacifism. Japan, which is one of the 5 strongest countries, has tried to attack several countries, that is, the purpose of war based on imperialism is only for benefits of capitalists. They were suppressed terribly by government and broke down because they were a literary organism related to proletarian literature continued for about 10 years, from the late of 1920 to the early of 1930, were ruined. In these two contries, the literature related to political purpose didn't exist any more from that time.

민간 조종사 전환과정 중 조종역량 결정요인 분석

Since airline safety depends significantly on the role of pilot, the importance of airlines' training qualified pilots and investing in the training program grows. This study aims to analyze the determinants of the commercial airline pilot competencies during the transition course. To this purpose, one-year training evaluation results of 215 new airline pilots in the JET transition course of Company K in 2019 are studied with correlation coefficient analysis, factor analysis, ANOVA, and regression analysis. Undergraduate major and Uljin-trained pilots show higher final-stage scores in PROC, AUTO, and CRM. Due to Uljin-trained pilots' higher first-stage scores, their score increase during the course is limited. Uljin-trained pilots' MANUAL scores in the final stage are lower than those of undergraduate major and overseas-trained pilots. Influence of trained location is found greater than English competency, undergraduate GPA, and ground school scores on commercial airline pilot competency evaluation scores during the program. The results will be useful in establishing scenario-based training focusing on different background to apply competency-based training and assessment.

다중클래스 계층에서 다중상속을 이용한 재사용에 관한 연구

The technology of "Reuse" is now under study in order to remove the gap between the demand and supply of software. This software technology can maximize software efficiency. Especially, if more than two classes are combined through multiple inheritances of multiple classes using Object-Oriented Programming techniques, one can minimize the conflict between the members in classes and can find the reusable classes to construct an efficient library through standardized oranization and classification. This thesis suggested a multiple inheritance technology which can be used to make computer programs and edit them efficiently through the Reuse of predefined classes. It also verified the appropriateness of the method through X-Window, a G.u.1(Graphical User Interface).
