Surgical Treatment of Pathological Fractures Occurring at the Proximal Femur

Purpose: To analyze the results of surgical treatment for pathological fractures at the proximal femur. Materials and Methods: Nineteen patients with a pathologicalfracture were included. The mean age was 65.7 years old. The patients comprised8 males and 11 females. Primary tumors, types of pathological fractures, surgical procedures, and postoperative complications were recorded. MusculoskeletalTumor Society (MSTS) functional score was used for functional evaluation. A Kaplan-Meier survival analysis was used to determine survival rate. Results:The primary malignancies were 6 cases of breast cancer, 3 cases of lung cancer, 3 cases of renal cell carcinoma, 2 cases of cholangiocarcinoma, 2 cases of hepatocellular carcinoma, 1 case of esophageal cancer, 1 case of colon cancer, and 1 case of ovarian cancer. Pathological fractures included 8 cases of pertrochantericfractures and 11 cases of subtrochanteric fractures. Intramedullary nailingwas performed in 10 cases, and joint replacement surgery was performed in 9 cases. Postoperative complications included local recurrence in 1 case, infection in 1 case, and nail breakage in 1 case. The mean postoperative MSTS score was 21. The mean survival period was 10.6 months. Patient survival rates were 42.1% after 6 months, 26.3% after 12 months, and 10.5% after 24 months. Conclusion: Surgical treatment of pathological fractures at the proximal femur provided early ambulation, and excellent pain relief. The surgery was well tolerated emotionally. Surgery is necessary for improving the quality of life in such patients; however, more cases of pathological fractures in these regions should be subjected to detailed analysis.

송강의 생애를 크게 두가지로 나눌 수 있다. 그 하나는 정치적 활동의 단연한 욕망에서 오는 관직의 대부분과, 또다른 하나로는 그의 작가적 천성의 탐구의식의 내부를 형성하고 있는 자유시적 방법의 자아몰입이 곧 그것이라 하겠다. 이 두가지 근본적 상대의 이율황쟁은 곧 그의 본연한 특성이 되고, 당시 전형적 양태의 본질이 되는 것이다. 후세의 학자들이 역주 성강을 말함에 있어서 일률적이고 보련적 방식에 의한 당시 사회적 또는 정치적 배경만을 들추어 그의 작품에 나타나고 있는 제반동기와 대상들을 절대적이고 불변의 그것으로, 변군사상이란 관념앞에 정의를 내리고 있음에 다소 의심을 품게 됨이 내가 여기서 말하고저 함의 대의가 될 것이다. 즉 일언하여 나는 송강을 인간 그 자체에 있어서 의아하게 생각하는 점이 한 두가지가 아니다. 무릇 전세기를 통하여 보건데 대부분의 시인들은 정감면에 있어서 특이한 조직과 거기에 따르는 긴박한 의욕을 갖고 있었음을 우리는 안다. 하물며 송강도 그 예외일 수는 없는 것이 아니겠는가 하는 극히 상식적인 나의 착안에서 출발됨을 다만 헛된 일이라 생각하기 보다 다른 한편 필요한 요건의 하나가 되지 않을까 하는 일이다. 당시 정치적으로 그를 불운한 시대의 인물이라 평하여 그에게 유리한 비평이 가해지고 있음을 보는데 이는 곧 그의재질에 탄복하여 그를 두둔한 말들이 아닌가 한다. 나는 여기에서 그렇게 말할 수 있고 도 그렇게 믿고 싶어 하였던 그들에게 동정을 보낸다. 왜냐하면 송강의 뛰어난 상징적 기개에 속고 말았다는 사실이다. 송장자신으로 볼때 그는 체면을 지극히 중요시 하였음을 짐작하는데, 곧 무슨 말인가 하면 자기의 사회적 정치적 위치로 봐서 어떤 여성을 사랑함에 있어서 직접적 표현 같은 것을 무척꺼렸다는 것이다. 고어에 '님' 과 '임의 개별적 존재에서 '님' 은 곧 스승이나 임금을 칭할 때고 '임'은 이성간의 호칭으로 사용했다는 그것에 나는 황홀한 시야를 집중적으로 보내고 있는 것이다. '님' 이라는 하나의 통념으로 자기 스스로를 숨길 뿐더러 그 대상을 숨기고 있엇다는 사실이다. 송강 역시 인간임엔 틀림없는 사실일진덴 그에게서 연정같은 뿌리를 짜르고 살수는 없었을 것이다. 즉 임군을 그린다는 고상하고 절대권력의 형태를 통함으로서 그의 개인적 이성의 사모와 뜻을 구애됨이 없이 자유로히 발할 수 있었다는 이상적인 방법에 그는 몸을 숨기고 있었던 것이다. '사미인곡' 과 '속미인곡' 은 그 좋은 예가 되는 것이다.

In turning, the chip may be produced in the form of either a continuous chip or discontinuous chip. Continuous chips are dangerous to the operator and difficult to handle at high speed machining. The signal of AE(Acoustic Emission) is related to cutting conditions, tool materials, test conditions and tool geometry in turning. The relationship between AE signal and chip formation was experimentally investigated. The experimental result show that two types of chip formations are possible to classify from the statistical analysis of the amplitude of AE signal.

Toxicity of chlorpyrifos‐methyl to Sitophilus zeamais collected in Korea and biochemical differences

In this study, lethal concentration (LC50) values of chlorpyrifos‐methyl (CPM) were determined for two Korean strains (CBNU and KNU) of Sitophilus zeamais. The two strains had similar susceptibilities (1.70 and 1.86 μg a.i./cm2, respectively) to CPM. Carboxylesterase (CE) activity was twice as high in the CBNU strain as in the KNU strain. Lower acetylcholinesterase (AChE) activity was also noted in the latter; however, the activity of glutathione S‐transferase (GST) was twice as high as in the CBNU strain. Gel electrophoresis of CE of crude extracts from adults of the two strains of S. zeamais showed clearly different band patterns, with molecular weights of 60 kDa and 71 kDa in the CBNU and KNU strains, respectively. MALDI‐TOF MS/MS was used to profile small proteins (less than 10 kDa), with results indicating that 206 proteins are expressed differently in the two strains. The peak of interest of 2247.7 m/z was applied to TOF‐TOF MS and its de novo peptide sequence was identified as a tyrosine phosphatase fragment. Phospholipids from the two strains were analyzed and 34 phospholipids were found to be significantly different between strains. Results suggest that the two strains collected from Korea showed different biochemical results, presumably differences in insecticide selection by different living locations.
