피부 T세포 림프종에서의 p53 및 mdm-2단백 발현에 관한 연구

P53 gene mutation appears to play an important role in the development of systemic lymphoma, and may be associated with tumor progression. However, its role in cutaneous lymphoma is currently unknown and only very limited series have focused on the role of p53 in cutaneous T-cell lymphoma. As dysfunction of p53 has also been suggested to occur after interaction with mdm-2 cellular oncogene, we have performed immunohistochemical analysis to investigate the patterns of p53 and mdm-2 expression in 12 cases of cutaneous T-cell lymphoma. Six cases were positive for p53, of which five were found to be high grade lymphomas, and eight cases were positive for mdm-2, of which six were found to be high grade lymphomas. All cases showing positive staining of p53 were of stage Ⅲ or Ⅳ. In six cases of primary cutaneous T-cell lymphomas, only one case was positive for p53. There was no consistent relationship between the expression of p53 and mdm-2, and the abnormal nuclear expression of p53 and mdm-2 did not seem to correlate with the presence of Epstein-Barr virus infection.
선상 태선과 편평 태선의 면역조직화학적 소견에 대한 비교 연구

Lichen striatus(LS) is an uncommon linear eruption characterized by discrete, flat-topped lichenoid papules resembling lichen planus(LP). The histologic picture of LS is nonspecific and shows so many remarkable variations that it can be easily misinterpreted. In the present study, we have evaluated the immunohistochemical aspects of 10 cases of LS and 4 cases of LP using paraffin-embedded section to evaluate if they are useful when differentiating LS from LP is difficult in clinically and histopathologically. There was no significant difference of CD4 and CD8 expression between LS and LP. In all cases focal reactivity for HLA-DR was confined to keratinocytes adjacent to the HLA-DR+ dermal lymphoid infiltrates except one case of LS in which HLA-DR was expressed on the surface of keratinocytes in diffuse pattern. There was a marked increase in S-100^- epidermal Langerhans cell in LP rather than LS.

The progressive increase in the resistance of N. gonorrhoeae to antibiotics, particulaly the alarming discovery of beta-lactamase producing strains, has created an urgent need to reevaluate the treatment of gonococcal infection and to serch for effective alternative treatment regimens. Thiamphenicol, a chloramphenicol analong in which the nitro group has been substituted with a methylsulfonyl group, has been suggested as an alternate drug to penicillins. The initial study population, 225 male uncomplicated gonorrhea patients, were treated with two regimens in the period October, 1982 to February, 1983. They were divided into two groups and assigned to treatment with 2.5 gm thiamphenicol PO (regimen A) or 2.0 gm spectinomycin IM(regimen B) according to a random number sheet. Out of 225 patients 196 patients returned 3-5 days after the treatment for a test of cure. Sixty three (41.4%) of 152 patients who received thiamphenicol failed to recover, 25 (56.8%) of 44 PPNG urethritis and 38 (35.2%) of 108 non-PPNG urethritis. Forty eight of these failure cases except one and those 44 in the regimen B who received spectinomycin recovered. The unacceptably high failure rate found in our trial in the treatment of both PPNG and non-PPNG urethritis with thiamphenicol 2.5 gm PO regimen is quite disappointing. The high MIC to thiamphenicol found in isolates from Seoul, Korea is undoubtedly the cause of the high failure tate.

Eccrine angiomatous hamartoma (EAH) is a rare benign cutaneous lesion. EAH is usually a single, aymptomatic lesion that appears as a nodule or plaque on the extremities, mainly the legs. Hyperhidrosis and/or pain may be present. Histologically, it is characterized by the proliferation of well-differentiated eccrine glands, in close association with well-differentiated, this-walled venules and capillaries in the dermis to the subcutis. We report a case of EAH showing atypical clinical appearance of fat hlrnia on the both heels in a 50-year-old female patien.
비임균성의 요도염에서 Azithromycin과 Doxycycline의 치료효과 비교

Currently, there is no single-dose therapy that is effective in the treatment of nongonococcal urethritis. Azithromycin is a new azalide antibiotic and has activity in vitro against Chlamydia trachomatis, and its novel pharmacokinetics suggest that single dose may be effective in the treatment of non-gonococcal urethritis. The aim of this study is to find out the efficacy and safty of Azithromycin in the treatment of males with uncomplicated non-gonoccocal urethritis. 32 patients were given a single 100mg oral dose of Azithromycin and 30 patients were treated with doxycyclin 100mg bid for 7 days. Follow-ups after one and two weeks to examine for cure and adverse events were made. The result showed that 27 out of 32 patients treated with Azithromycin were cleared of urethritis and 24 out of 30 patients treated with doxycycline were cleared of urethritis. Single dose azithromycin shows a similar effectiveness as a 7-day regimen of doxycycline and the patient's compliance has also improved.
P087 : Adverse effects of oral cyclosporine in the treatment of various skin diseases

Background: Cyclosporine (CS) has been used for various skin diseases including psoriasis and atopic dermatitis. However, dermatologists are reluctant to use CS in clinics because of the adverse effects. Objectives: To investigate the adverse effects of CS for dermatological uses. Methods: A retrospective study of 1,335 patients with CS treatment was performed. Results: 15.6% of patients showed adverse effects. Gastrointestinal (GI) symptoms were the most common adverse effect. Other adverse effects included hypercholesterolemia, hypertrichosis, headache, hypertension, hypertriglyceridemia in order of frequency. Hypercholesterolemia was more often in men, but GI symptoms and hypertrichosis in women. Hypercholesterolemia, hypertriglyceridemia and GI symptoms were more common in adults, but hypertrichosis in young. GI symptoms and headache occurred more often within three months of CS treatment, but hypertension, hypercholesterolemia and hypertriglyceridemia after three months. Hypertrichosis was more often in group with low initial dose. GI symptoms occurred more often with lower CS cumulative dose group and hypertension and hypertriglyceridemia in higher CS cumulative dose group. Conclusion: Comparing to previous studies, the frequency of adverse effects of CS was proven to be low. Therefore, in dermatologic field, we suggest that CS is relatively safe with periodic follow-up.
