      • Cisapride가 금식기 담낭의 수축운동에 어떤 영향을 미치는가?

        Cisapride의 식사후 담낭의 수축운동에 대한 영향을 규명하고자 20-30대의 건강인 12명(남자 6명, 여자 6명)을 대상으로 placebo, cisapride 10㎎ 혹은 20㎎을 경구복용한 후 담낭체적의 변화를 초음파로 측정하여 다음과 같은 결과를 얻었다. Placebo를 복용한 군에서는 검사기간중 완만한 체적의 변동이 있었으나, cisapride를 복용한 후에는 담낭체적이 지속적으로 현저하게 증가되었다. 이러한 담낭체적의 증가는 placebo를 복용한 군과 비교해 볼 때, cisapride 10㎎을 복용한 군에서는 60분후부터, 20㎎을 복용한 군에서는 50분후부터 통계학적 유의성이 있었으며(p<0.05), 이러한 경향은 검사종료시까지 지속되었다. 그리고 최대체적은 cisapride 10㎎을 복용한 군에서는 120분후에 최초체적의 145%에, cisapride 20㎎을 복용한 군에서는 90분후에 최초체적의 162%에 도달하였다(Fig. 2). 한편 체적곡선 아래의 면적은 cisapride 10㎎을 복용한 군은 31325 ±2518, 20㎎을 복용한 군은 33355 ±4726로, placebo를 복용한 군(23510 ±2864)보다 유의하게 컸다(p<0.05) 이상의 결과에서 cisapride의 경구섭취는 금식기 담낭의 수축운동을 억제하는 이완효과가 있음을 알 수있었다. 그리고 이러한 담낭수축억제효과는 장기간 cisapride를 복용하는 환자에서 담석형성이 촉진되는 효과가 있을 수 있고, 반대로 담석존재시 선통 발작을 억제하거나 예방하는 효과가 있을 수도 있다는 것을 시사한다. Cisapride(C23H29CIFN304) is one of the well-known prokinetic agents. It operates upon whole gastrointestinal tract, from lower esophageal sphincter to anal sphincter, and holds the most promise for patients with colonic motility disorders. It is known to function as an indirect cholinergic stimulant through increased release of acetylcholine in the intramural plexuses. Unlike metoclopramide, it does not have the antidopaminergic effects on the central nervous system. But its effect on the biliary system is not clearly elucidated. This study was performed in 12 healthy volunteers, and focused on the change of gallbladder volume after cisapride ingestion during fasting period. With the interval of 2-4days, placebo, 10㎎ or 20㎎ of cisapride were given. Cisapride inhibited the contraction of gallbladder compared with placebo. The results suggest that cisapride can induces the possible cholelithiasis, but, on the other hand, it can be used as the possible spasmolytic agent in the gallstone disease.

        치아재식술 시 근관충전재로 사용된 수산화칼슘 제재의 치근단 조직 반응 및 치근 흡수의 평가

        Calcium hydroxide pastes is widely used in conventional endodontics. Along with the expanded clinical use of calcium hydroxide, literatures suggest mixing calcium hydroxide with other substance. Among added substances the vehicle plays the most importance role in the overall process because it is directly related with the velocity of ionic dissociation Of Ca^(2+) and OH^- ion. In this study, we evalutated and compared periapical tissue response and root resorption after canal was filled with mixture of saline and calcium hydroxide(Junsei Chemical Co., Japan) as a aqueous vehicle, Metapaste(Meta Co., Korea) as a viscous vehicle paste, Vitapex(Neo Dental, Japan) as a oily vehicle paste and IRM(Caulk Dentsply, USA) in replantation of rat molar. A total of 31 maxillary first molars of Sprague-Dawley female rats, 30 days old were used. The upper 1st molar was extracted and the mesiobuccal canal was filled with mixture of saline and calcium hydroxide, Metapaste, Vitapex, IRM and then replanted. Rats were sacrificed 3 weeks after replantation, the maxillae were removed, section of 4 micron were cut and stained with hematoxylin-eosin. Apical tissue response were observed under light microscope. The results were as follows : 1.Saline mixing group and Metapaste group were significant different in fibrous capsule width compared to Vitapex group and IRM group(P$lt;0.05). 2.Saline mixing group, Metapaste group, Vitapex group and IRM group did not prevent root resorption and there were no statistical difference. 3.In saline mixing group and Metapaste group, loss of pastes were observed in all samples. From the results of our study, we observed loss of pastes in saline mixing group and Metapaste group because of water soluble property and assumed it was related to inflammation in apical area and sealing ability of material. So, we should study and develop calcium hydroxide vehicle which is easily removed and more stable and because of only 3 weeks observation we need more evaluation in long period.

      • 복부 둔상에 의한 간손상 환자의 비수술적 치료

        복부 둔상으로 발생한 간 손상시 과거에는 일률적으로 개복술을 시행하였으나, 최근에는 그 치료 방침이 바뀌고 있다. 혈역동학적으로 안정된 환자군에서는 비수술적 치료를 시행하기도 한다. 저자들은 혈역동학적으로 안정되어 있으며, 복강내 타장기의 손상이 없는 간 손상 환자 7예에서 비수술적 치료를 시행하여 다음과 같은 결과를 얻었다. 전예에서 aspartate aminotransferase 및 alanine aminotranaferase의 상승이 있었으며 2주 후에 정상화 되었다. 2주 간격으로 추적한 컴퓨터 단층 촬영 소견의 변화는 손상 정도에 따라 차이가 있었다. Grade III의 경우는 열상부위가 완전히 소멸되는데 6주가 걸렸으며, Grade I,II의 경우는 4~6주 경과후에 소실되었다. 혈종%의 소실에 걸리는 시간도 대개 4~6주가 걸렸으나 혈종의 크기에 따라 차이가 있었다. 1예를 제외한 6예에서 성공하였으나, 1예에서는 다량의 담즙 누출에 의한 복막염으로 개복술이 필요하였다. Several significant advances in the treatment of hepatic injuries have evolved over the past decade. Non-operative treatment of blunt hepatic injury is one of the recent changes in the treatment of blunt hepatic injury. Nonoperative management of 7 cases of liver injury due to blunt abdominal trauma in 1991-1992 is retrospectively reviewed. All patient except 1 case were hemodynamically stable and alert. Final diagnosis and follow-up were done with computer tomography. The grade of hepatic injury of the patient was I to IV. At admission all patient had high serum aminotransferase which was normalized after 2wks. The edge of laceration disappeared after 4-6 weeks on the follow-up CT films. All but 1 case had successful clincal course. But we failed in 1 case(Grade IV) due to bile leakage. As conclusion, we suggest that nonoperative treatment is a safe method for the blunt hepatic injury patients who are hemodynamically stable and alert as previous reports.

        색상 측정 기기를 이용한 복합레진 적층 수복과 단일 수복의 색상 비교 분석

        Objectives: This study analyzed the difference in color caused by different thickness in enamel layer of composite resins when applied with single and layering placement technique, and evaluated if the results agreed with the shade guide from the manufacturers to verify reliability of the color matching process of the manufacturers. Materials and Methods: For single composite resin samples, 6 mm diameter and 4 mm thickness cylindrical samples were fabricated using Ceram-X mono (DENTSPLY DeTrey) and CIE L*a*b* values were measured with spectrophotometer. Same process was done for layering compositie resin samples, making 3 dentinal shade samples, 4 mm thickness, for each shade using Ceram-X duo (DENTSPLY DeTrey) and enamel shade resins were layered in 2 mm thickness and CIE L*a*b* values were measured. These samples were ground to 0.2 mm thickness each time, and CIE L*a*b* values were measured to 1 mm thickness of enamel shade resin. Results: Color difference (△E*) between single and layering composite resin was 1.37 minimum and 10.53 maximum when layering thicknesses were between 1 mm and 2 mm and 6 out of 10 same shade groups suggested by manufacturer showed remarkable color difference at anythickness (△E* > 3.3). Conclusion: When using Ceram-X mono and duo for composite resin restoration, following the manufacturer's instructions for chossing the shade is not appropriate, and more accurate information for Ceram-X duo is needed on the variation and expression of the shades depending on the thickness of the enamel.       • 레일리 페이딩 채널에서 길쌈부호를 채용한 16QAM/OFDM 시스템 성능에 관한 연구

        In this paper, we simulated the performance of 16QAM/OFDM system with 1/2 convolutional coding technique in Rayleigh fading channel with COST-207 delay model. This system includes the Time/Frequency Interleaver, which is adapted EUREKA-147, and the 2-dimentional Pilot Symbol Assited Modulation(PSAM) method with or without low pass filter(LPF). In the simulation, we consider the coding effect according to channel decoding method and the performance of channel estimation with or without LPF. As a result of simulation, We obtained the result that the system using 4bit soft decision Viterbi decoding is required about 11dB -- 14.5dB to satify the BER 10^-3 in Rayleigh fading channel according to channel condition. And also the system performance of PSAM with LPF has thc better peformance of about 2dB than that of system without LPF. And the coding effect of this system is about 2 - 6dB according to channel decoding method.

      • Box-Jenkins 모형을 이용한 X ̄관리도와 CUSUM관리도의 민감도 분석

        It is revealed that, as the use of automated manufacturing is increasing and the process inspection technology is improvements in industry for recently years, the data from mass production system is exhibited some degree of autocorrelation. The operation characteristics of traditional control charts developed under the assumption of independence may be adversely affected by the presence of serial correlation. Therefore, we try to explain a autocorrelated construction by being in contact with a time-series models, so called the Box-Jenkins forecast models, which has been proposed as a very good forecasting tool and allows to distinguish the variations resulted from a autocorrelation structure from one due to unusual but assignable causes. In this paper, autoregressive/moving average parameters in the use of Box-Jenkins forecast models is estimated, and a sensitivity analysis is given by comparing X control chart with CUSUM control chart for the variation of error or forecast residuals.

        As the rapid growth of industrial society, many enterprises are inclined to 100 ppm management, even more to ppm management from the traditional 3σ management method. Therefore, it needs to extend process capability index for guarantee of high quality assurance level to ppm unit. Thus, in this paper, using the probability of standard normal distribution from 3σ tp 6σ, which was developed by an author, the optimal numbers of inspection facility is determined for guarantee of process capability index and quality assurance level. Also, high quality assurance level is determined by considering the precision of inspection when one and two sided specification is given for the probability which is out of the specification limit.

      • 醱酵飼料 제조에 관한 硏究 : 3. 醱酵飼料 제조시 水分添加 水準에 따른 아미노산 組成에 관한 硏究 3. A Study on Amino acid Composition as added Moisture Levels

        To estimate the essential and nonessential amino acid compositions as added moisture levels in manufacturing fermentation feedstuffs, the three kinds of experimental conditions, T1, T2, T3 were made with rice bran(90kg), leaf mold(10kg) according to added moisture 40kg, 60kg, and 80kg, respectively. In T1 and T2 cases, the contents of threonine, phenylalanine, histidine, aspartic acid, glutamic acid, cystine, arginine increased significantly during fermentation, however, in T3 case, the thereonine content increased significantly after 9 hours fermentation but decreased at 12 hours fermentation(P<0.05). In T1 and T3 cases, the contents of valine, leucine and arginine increased significantly after 9 hours fermentation and decreased at 12 hours fermentation(P<0.05). Methionine increased for three hours fermentation and then did not increase in content in the T1 and the T3. In T2 case, methionine level increased gradually for whole fermentation time. In T3 case, phenylalanine, histidine, aspartic acid, glutamic acid and cystine increased significantly after 9 hours fermentation, but decreased at 12 hours fermentation(P<0.05). In T1, T2 and T3, isoleucine, lysine, serine, proline, glycine, alanine and tyrosine increased significantly during fermentation time(P<0.05).

      • 대망에 발생한 방선균증 1례

        Actinomycosis is an uncommon chronic suppurative disease characterized by multiple abscess, draining sinuses, abundant granulation, and dense fibrous tissue, caused by the actinomyces organisms. It occurs in three major sites: cervicofacial, abdominal, and thoracic. The abdomen is the second most common involved site in previous studies, and there are three ways this organism can affect the gastrointestinal tract: through a blood-borne infection, by swallowing, or by proliferation at a more distal intestinal area. Abdominal actinomycosis is an unusual disease but remains an important differential diagnosis in patients with abdominal mass. Obtaining the diagnosis is difficult and rarely made preoperatively, but provide the key to its management. Medical therapy is effetive in eradicating the disease; however, because of the large amount of reactive fibrosis formed by the infection, it requires long-term therapy. Surgical treatment may be essential in many cases, primarily when malignancy cannot be excluded or if extensive necrotic tissue exists. There are a few description for actinomycosis involving omentum. We report a case of patient with omental actinomycosis, suspected to have a malignancy requiring an exploratory laparotomy with resection.
