Objectives:Hypothalamic-Pituitary-Adrenocortical(HPA) system dysregulation plays an important role in the pathophysiology of major depressive disorder. Medically unexplained somatic symptoms comprised the predominant complaints of korean patients with major depressive disorder. It might be related to dysregulation of HPA system and somatic symptoms. The aim of this study was to investigate the association between Hamilton Depression Rating Scale severity and the Plasma Adrenocorticotropic Hormone(ACTH) & Serum Cortisol level in Korean patients with major depressive disorder. Methods:Our study design was prospective. A total 111 hospitalized patients with major depressive disorder (DSM-IV) at Anam Hospital, Korea University Medical Center between Jan 2009 and May 2011 were selected. Hamilton Depression Rating Scale was measured by psychiatrist. Plasma ACTH, Serum Cortisol levels were analyzed at the time of admission. Results:There are significant correlations(p<0.05) between Hamilton Depression Rating Scale of somatic component(HAM-D 11-15 item) and Plasma ACTH and Serum Cortisol level. However, other HAM-D sub items were not significantly correlated with Plasma ACTH and Serum Cortisol level except somatic component. Conclusion:Our Study results suggest that hyperactivity of HPA axis might be correlation with somatic symptoms in korean patients with major depressive disorder.
만성요통환자에 있어서 근에너지기법과 치료마사지의 복합프로그램 적용 효과

In order to compare and analyze major techniques employed by the college judo competitors of Korea who participated in the 43rd National Male and Female Judo Championships by Weight Class, the players in the light, middle and heavy weight classes were classified into 3 categories: (1) those who are taller than opponents(by +4 cm or more); (2) as tall as opponents(-3~+3 cm); and (3) smaller than opponents(by -4 cm or less). Scoring points and used techniques were examined from videotapes and matching records, which resulted in the following conclusions: 1. In case of light weight players who are taller than opponents, foot(34.73%) was used most frequently and effectively, followed significantly by hand(28.54%), pressing(15.35%), waist(15.31%) and pull-down(4.25%) sacrifice throw techniques(1.66%) in order(p<0.00l). Those who are as tall as opponents used foot techniques(34.88%) more effectively than hand although hand techniques(38.38%) were employed more frequently. For those who are smaller than opponents, hand techniques(35.03%) were used most frequently and effectively, followed significantly by foot(32.57%), waist(8.39%), pressing(16.12%) and pull-down sacrifice throw techniques(7.8%) sequence(p<.001). 2. In case of middle weight players who are taller than or as tall as opponents, foot(30.72%) was used most frequently and effectively, followed significantly by hand(35.42%), pressing(21.32%), waist(8.15%) pull-down sacrifice throw(0.42%) and cross lock(l.06%) in order(p<0.001). However, for those who are smaller than opponents, hand techniques were used more frequently and effectively than pressing, waist and cross lock

The purpose of this study was to analyze the convenience, dearness and effectiveness of the rating system of the GB. This Green Building uses a rating system which includes a network-diagram; Sorensen-network as well as a Sequence-diagram. This study is consisted of a visualized work sheets that were divided into network-diagram, annual energy consumption, air pollution as well as LCC. It has technique of separated rating for the three departments that of the globe environment conservation, health and convenience, construction ability, as well as highest mark such as the object building's performance was which is not considered with non environmental damage level. This program aimed at establishing a weight given to input to default value and needs constant study for this program. Also this study established a inventory database for estimating air pollution quantity.

The purpose of this impact statement was improve to a method of rating and design for the environmental performance of the green building by that an analyze to the weighting point and the structure of the five rating tools: GBTool, LEED, BREEAM, Korean government's method, CAS BEE. For improve to the accuracy, convenience, effectiveness of the rating and design method from the result of analyzed five tools, this impact statement was suggestion followings; 1) Divided assessment and public announcement that 'Global environmental conservation department' and 'Health & convenience department' at assess and design to the environmental performance of the green building. 2) Using by modified Network-diagram method in the EIA method at assess and design to it. 3) Arrange each 25% of weighting points of energy consumption criteria and air pollution criteria according to above divided condition.

To determine the recovery time from noise-induced temporary threshold shift(TTS), a prospective field study was conducted at three worksites where workers are known to be exposed high level of noise. Subjects were selected according to answers on a questionnaire which inquired about otological history and previous noise exposure, including avocational, military and occupational exposures. After excluding employee with past otologic problems, recent exposure to high level noise, and under medications, total 92 employees participated in the study. Among 92 participants, complete consecutive audiometric examinations were carried out at 0∼2 hours, 5∼7 hours, 14∼16 hours after worktime noise exposure on 26 participants wearing hearing protectors and 22 participants wearing no protective devices. The difference between the hearing level 0∼2 hours after noise exposure and 5∼7 hours is statistically significant by paired t-test(p<0.01). The median recovery times calculated from the data of 22 participants wearing no protective hearing devices are 15.6 hours at 4000Hz, and 7.7 hours, 10.3 hours, 8.4 hours at 1000Hz, 2000Hz and 8000Hz respectively. These data suggest that when measuring the pure tone audiometry for noise exposed workers, at least 16 hours noise-free interval is required.

<P>We report a case of synchronous occurrence of KIT-positive acute myeloid leukemia (AML) and gastrointestinal stromal tumor (GIST). A 63-year-old woman was hospitalized for dizziness, and abdominal computed tomography revealed an exophytic gastric mass and hepatic metastasis. The patient was diagnosed with GIST and was administered imatinib (400 mg/day) for the metastatic unresectable tumor. After 2 weeks of imatinib treatment, the patient developed pancytopenia, which persisted even after the drug was discontinued, thereby necessitating bone marrow biopsy. Biopsy examination indicated AML, and karyotyping revealed a complex karyotype. We did not observe point mutations at residues D816 and N822 of KIT. Therefore, the patient received standard induction chemotherapy, but on the 18<SUP>th</SUP> day after completion of chemotherapy, she died of septic shock and multi-organ failure. Since KIT plays an important role in both GIST and AML, we consider that both these malignancies may have been associated with each other.</P>

Diagnosis and Treatment of Lupus Nephritis: Survey Results on Four Important Issues

Objectives. To investigate the perception of and treatment pattern with regard to the four important issues in the management of lupus nephritis (LN), and to identify which parts of the LN treatment are difficult for physicians to carry out in clinical practice. Methods. Four steps were carried out: pre-survey, LN symposium, post-survey, and meeting after the symposium.The two surveys were conducted with the same contents regarding renal biopsy, induction and maintenance treatment for class III and IV LN, and treatment for class V LN. The results of the first survey and the changes in opinion reflected in the second survey were comparatively analyzed. Results. In the first survey, most of the respondent physicians replied that they would immediately conduct biopsy in the case of significant proteinuria. For the induction treatment of class III and IV LN, most of the respondent physicians selected high-dose cyclophosphamide. Mycophenolate mofetil and steroid combination therapy were selected for the maintenance treatment, and tacrolimus for the treatment of class V LN. There was a controversy in the drug selection, however, especially on the maintenance treatment of class III and IV LN and on the treatment of non-responsive class V LN. Conclusion. Some discrepancies were found in the treatment of LN in the real world. Although no recommendation was made for Korean LN patients in this study, the study results will help physicians select the most reasonable treatment for Korean LN patients based on experts` experiences and objective evidence.
