        A 10-Year Retrospective Analysis of Clinical Profi Les, Laboratory Characteristics and Management of Pyogenic Liver Abscesses in a Chinese Hospital

        Background/Aims: Pyogenic liver abscess (PLA) is a serious, life threatening condition with a high mortality rate that represents a diagnostic and therapeutic challenge. The aim of this study was to collect demographic data and clinical, laboratory and microbiological characteristics of PLA patients treated between 2000 and 2010. We also aimed to collect information regarding our management experience of these cases. Methods: As a retrospective review, 47 patients with PLA in a tertiary referral center were examined to determine their demographic characteristics, clinical features, and laboratory, imaging, and microbiologic findings as well as the treatment outcome. Results: Cryptogenic PLA was the most frequently identified type of PLA, while benign biliary tract disease was the most frequently identifi able cause of PLA (18/47 patients; 38.3%). Leukocytosis and elevated alanine transaminase were common laboratory findings and were observed in 35 (74.5%) and 22 (46.8%) patients, respectively. Increased fibrinogen was also detected in 11 of 15 investigated cases (73.3%). Notably, infection-induced thrombocytopenia occurred in 8 patients (17%). Diabetes mellitus was associated with the occurrence of infection induced shock when compared to the non-diabetic group (p<0.05). Patients with two or more comorbid diseases had longer hospitalizations when compared to patients with one comorbid disease or those without comorbidities (p<0.001). The number of days needed to establish diagnosis was correlated with the length of hospitalization (p<0.001). The overall hospital mortality rate was 2.1% (1/47). Conclusions: Characteristics of PLA patients from the past 10 years are presented. The number of days needed to establish a PLA diagnosis was correlated with the length of the hospital stay. The hospital stay of PLA patients can be further improved by early diagnosis and effective treatments during the early stages of PLA progression. (Gut Liver 2011;5:221-227)

        Identification and expression profiles of putative chemosensory protein genes in Cnaphalocrocis medinalis (Lepidoptera: Pyralidae)

        Insect chemosensory proteins (CSPs) are small, water-soluble proteins which can bind and transport hydrophobic odorants through sensillum lymph to activate odorant receptors, thus play important roles in the olfactory recognition. In the present study, by searching the transcriptome data sets, a total of 22 candidate CSP genes were identified from Cnaphalocrocis medinalis, a serious lepidopteran rice pest in Asia. CSP proteins encoded by these genes showed typical characteristics: an N-terminal signal peptide, four conserved cysteine residues and the pattern of cysteine spacing. The expression profiles of these putative CSP genes were investigated using real-time quantitative PCR. The results showed that five genes (CmedCSP4, CmedCSP8, CmedCSP11, CmedCSP18 and CmedCSP21) were expressed primarily in antennae, suggesting their involvement in olfactory processes; whilst other genes are mainly expressed in non-olfactory tissues, such as abdomen and legs, indicating a broader physiological function for these CSPs. The findings will lead to a better understanding of the molecular mechanisms of chemoreception in C. medinalis.

        Proteomic Analysis of Salt Stress Responses in Rice Shoot

        To gain a better understanding of the mechanism of rice (Oryza sativa L.) in response to salt stress, we performed a proteomics analysis of rice in response to 250 mM NaCl treatment using shoots of 3-day-old nascent seedlings. The changes of protein patterns were monitored with two-dimensional gel electrophoresis. Of 57 protein spots showing changes in abundance in response to salt stress, 52 were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. The identified proteins were classified into eight functional categories. Several novel salt stress-responsive proteins, including protein synthesis inhibitor I, photosystem II stability/assembly factor HCF136, trigger factor-like protein and cycloartenol-C24-methyltransferase are upregulated upon salt stress. In order to figure out the different and similar molecular mechanism among salt and other stresses,regulation of some salt responsive proteins under other abiotic stress (cold and dehydration) and abscisic acid application was also analyzed. The possible molecular mechanism of rice seedlings in response to salinity and other stresses were discussed.

        Chinese herbal injections for coronavirus disease 2019 (COVID-19): A narrative review

        Background The outbreak of Coronavirus disease 2019 (COVID-19) has caused more than 180 million infections and 3.9 million deaths. To date, emerging clinical evidence has shown the synergetic benefits of Chinese herbal injections in treating this contagious respiratory disease. This review aims to summarize and analyze the efficacy and safety of Chinese herbal injections in the therapy of COVID-19. Methods The literature from 3 electronic databases, PubMed, CNKI, and Web of Science, were searched using the search terms “COVID-19”, “SARS-CoV-2”, “traditional Chinese medicine”, “herb”, “herbal”, and “injection”. Then the identified articles were comprehensively evaluated. Results Limited data demonstrated that Chinese herbal injections could significantly improve the clinical outcomes of COVID-19 patients, especially in combination with conventional treatment strategies. The benefits of which were mainly associated with the relief of symptoms, prevention of secondary infection, regulation of inflammation and immune function. There was also evidence showing the inhibitory effects on SARS-CoV-2 replication in vitro. Nevertheless, available real-world data suggested the increased risk of adverse event. Furthermore, the defects of existing researches and the insights for discovering novel antiviral drugs were prospectively discussed. Conclusion Evidence-based advances revealed that Chinese herbal injections such as XueBiJing injection and ShenMai injection, exerted potent effects against COVID-19. Further laboratory researches and clinical evaluation are needed to gather scientific evidence on the efficacy and safety.

      • Weighted Gene Co-expression Network Analysis in Identification of Endometrial Cancer Prognosis Markers

        Objective: Endometrial cancer (EC) is the most common gynecologic malignancy. Identification of potential biomarkers of EC would be helpful for the detection and monitoring of malignancy, improving clinical outcomes. Methods: The Weighted Gene Co-expression Network Analysis method was used to identify prognostic markers for EC in this study. Moreover, underlying molecular mechanisms were characterized by KEGG pathway enrichment and transcriptional regulation analyses. Results: Seven gene co-expression modules were obtained, but only the turquoise module was positively related with EC stage. Among the genes in the turquoise module, COL5A2 (collagen, type V, alpha 2) could be regulated by PBX (pre-B-cell leukemia homeobox 1)1/2 and HOXB1(homeobox B1) transcription factors to be involved in the focal adhesion pathway; CENP-E (centromere protein E, 312kDa) by E2F4 (E2F transcription factor 4, p107/p130-binding); MYCN (v-myc myelocytomatosis viral related oncogene, neuroblastoma derived [avian]) by PAX5 (paired box 5); and BCL-2 (B-cell CLL/lymphoma 2) and IGFBP-6 (insulin-like growth factor binding protein 6) by GLI1. They were predicted to be associated with EC progression via Hedgehog signaling and other cancer related-pathways. Conclusions: These data on transcriptional regulation may provide a better understanding of molecular mechanisms and clues to potential therapeutic targets in the treatment of EC.

        Low-Temperature Synthesis of Highly Efficient, Deep-Red Zn-Cu-In-Se/ZnSe Fluorescence Quantum Dots

        We report a facile synthesis method on CuInSe2 (CISe)-based quantum dots (QDs) by using tri-n-octylphosphine selenium (TOPSe) as selenide precursor, with assistance of oleylamine (OAm) and n-dodecanethiol (DDT). We demonstrate that the OAm and DDT jointly contribute to the formation of the low-temperature-decomposable metal-sulfide clusters, and promote the QD nucleation at relatively low temperature range of 180–200 ℃. Furthermore, to improve fluorescence property, Zn-doping and ZnSe coating are simultaneously carried out. The obtained deep-red ZnCISe/ZnSe QDs possess higher quantum yield of 65% at wavelength of 670 nm, which is in the best performance range ever reported. Then, we investigate the improvement mechanism, where the sufficient Zn replacement of In sites is the crucial factor. This modified core–shell structure provides two benefits, on the one hand, the enhancement on intrinsic defect-related recombination, and the other hand, the improved core–shell interface that reduces the nonradiative recombination.

      • Cecropin Suppresses Human Hepatocellular Carcinoma BEL-7402 Cell Growth and Survival in vivo without Side-Toxicity

        Conventional chemotherapy against hepatocellular carcinoma typically causes various side effects. Our previous study showed that cecropin of Musca domestica can induce apoptosis in human hepatocellular carcinoma BEL-7402 cells in vitro. However, whether cecropin inhibits BEL-7402 cell in vivo and the question of possible side effects remained undentified. The present study confirmed tumor-inhibitory effects of cecropin in vivo, and furthermore strongly suggested that cecropin cytotoxicity in BEL-7402 cells in vivo may be mainly derived from its pro-apoptotic action. Specifically, we found that cecropin exerted no obvious side effects in tumor-bearing mice as it had no significant hematoxicity as well as visceral toxicity. Therefore, cecropin may be a potential candidate for further investigation as an antitumor agent against hepatocellular carcinoma.

        Influence of Psychological Factors in Primary Dysmenorrhea Patients on De qi : a Secondary Analysis of a Randomized Controlled Trial

        Background: De qi , the needling sensation, is important in acupuncture treatment. Almost all studies believe that deep needling and manipulation could achieve a significant de qi sensation. However, relatively few studies have examined the effect of psychological factors on de qi, and those that did often reached different conclusions. Objectives: To explore the influence of psychologic factors on de qi in patients with primary dysmenorrhea (PD). Methods: Sixty-eight PD patients with cold and dampness stagnation were randomly allocated to de qi (deep insertion using thick needles, with manipulation, n=17) and non-de qi groups (shallow insertion using thin needles, without manipulation, n=51). Both groups received bilateral needling at Sanyinjiao (SP6) for 30 min. De qi was assessed using the Acupuncture De qi Clinical Assessment Scale (ADCAS). The patients’ acupuncturerelated anxiety and their expectations of the relationship between needle sensation and curative effect were evaluated using a five-point and four-point scale, respectively. Results: Within the de qi group, all patients experienced the de qi sensation, although anxiety levels were unrelated to de qi. Patients’ expectations correlated negatively with de qi timing, and positively with electric sensation. Within the non-de qi group, 59.5% of patients experienced de qi. Between those who experienced it and those who did not, no significant differences were found in anxiety levels, although patients’ expectations differed significantly. Among patients who experienced de qi sensations in the non-de qi group, anxiety and throbbing were positively correlated. Additionally, patients’ expectations correlated positively with de qi intensity, as well as coldness, and numbness. Conclusion: Psychological factors should be considered when studying de qi since PD patients’ expectations could influence the de qi sensation at SP6.

        Fabrication of PVDF-blended ultrafiltration membranes incorporated by chiral mesoporous silica for enantioseparation

        Novel poly(vinylidene fluoride) (PVDF) ultrafiltration membranes blending with chiral mesoporous SiO2materials were constructed to improve the anti-fouling and separation performance of PVDF-blendedmembranes. The formation of asymmetric mesoporous structure of chiral mesoporous SiO2 caused theirlarge hydrophilic surface and significant chiral features, which enhanced the hydrophilicity and polarityof the resulting PVDF membranes blended with chiral mesoporous SiO2. So, the PVDF-blended membranescontaining chiral mesoporous SiO2 had a high permeation performance and anti-fouling property. After blending with chiral mesoporous SiO2, the maximum flux of membranes exceeded 500 Lm2h1and was 6.5-fold higher than that of neat PVDF. Due to the stereo-specific interaction between enantiomersand chiral recognition sites introduced by chiral mesoporous SiO2 in the membranes, allPVDF-blended membranes had evident enantioseparation performance for DL-racemic tryptophan. TheC-SiO2-20/PVDF-blended membranes had the highest enantioseparation performance under the sameaddition content and the maximum enantiomeric excess (e.e.%) was more than 20 %. Given the strongstereo-specific interaction and hydrogen bonding between d-tryptophan and b-cyclodextrin (b-CD), theaddition of more b-CD would introduce more chiral recognition sites into the membranes, therebyenhancing the enantioseparation performance of the membranes for d-tryptophan. The maximum e.e. % of C-SiO2-20/b-CD/PVDF-blended membranes for the tryptophan enantiomer reached 55%.
