급성백혈병의 화학요법에서 조혈간세포 보충요법

1. 목적 급성백혈병의 화학요법에 있어서 관해유도 요법시에 보다 더 강력한 항암화학요법을 시행하고 골수억제의 회복을 위하여 말초혈액에서 채취한 조혈모세포의 투여로 그 효과를 증대시키고자 시도하였다. 2. 방법 관해유도 화학요법후에 골수억제시기로부터 회복단계로서 백혈구감소증이 호전도면서 단핵세포가 증가될 때에 말초혈액에서 조혈간세포(stem cell)로 인정되고 있는 CD34+세포의 함유량이 1% 이상되는 시기를 찾아내고 그 때부터 평균 4차례의 단핵구를 분리채집을 하여 -74。C에 냉동보관하였다가 제2차 화학요법후에 해동시켜서 정맥주입하였다. 3. 결과 대상 급성백혈병환자는 6명에 23회의 조혈간세포를 함유한 단핵구 채집을 시행하였고 유효한 단핵구의 채집은 4례에서 가능하였고 2례에서는 유효한 채집이 못되었다. 채집효율은 39.0-74.5%로서 평균 49.8%였다. 채집 후에 혈액학적 변화는 혈색소의 약간 감소 (평균 1.34g/dl)와 혈소판의 감소 (평균20.25%)를 나타내었다. 해동 단핵구를 투여할시에 일과성인 불쾌감, 기침, 흉부압박감, 호흡곤란 등이 있었다. 4. 결론 자가골수이식에서 자기말초혈액조혈간세포이식으로 관심이 이행되는 현시점에 자가조혈간세포를 채집하여 수혈하는 기술이 확립되는 기초를 이루었다고 본다. 이번 대상들은 재발된 금성백혈병들에 시행하였으나, 향후에는 초발시에 제일차 관해유도 화학요법 과정에 시도해야할 것으로 생각한다. Object: Peripheral blood stem cells(PBSC) were collected after remission induction chemotherapy and reinfused after intensified chemotherapy in order to increase the chemotherapeutic efficacy. Method: Collection of mononuclear cells (MNC) was started when CD34+ cells above 1% and WBC above 1,000/ul with mononuclear cell percentage above 30%, the collection procedures were done 2-6 time in each patient of total 6 cases. The collected MNCs were stored in - 750C and reinfused after thawing in the water bath at 370C Results: Twenty three collection procedures from the 6 cases of acute leukemia had made effective MNC collection in 4 cases and mean collection efficiency was 49.8%(from 39.0% to 74.5%). Hematological changes after the procedure were mean reduction of hemoglobin 1.34g/dl and mean 20.25% of decreased platelet count. There were noticed transient symptoms of discomfort, coughing, chest tightness and dyspnea in association with the infusion of stored cells. Conclusion: The basic process for the peripheral blood stem cell transplantation (PBSCT) was accomplished with this supportive care for the consolidation chemotherapy of acute leukemia. Effective PBSCT would be warrented with more qualified process and high does chemotherapy. (Korean J Blood Transfus 6(2) : 41~47,1995)

부신피질 자극호르몬 단독결핍 1례

부신피질 자극호르몬 단독결핍은 국내에서 7례만이 보고된 희귀 질환이며 병인 및 경과 등 아직 불분명한 측면이 많다. 이는 질환지체의 희귀성도 있지만 치료후 자연경과에 대한 장기적인 추적검사가 이루어진 경우가 드물기 때문이기도 하다. 저자들은 비특이적인 전신증상을 주소로 내원한 폐경기후의 56세 여자 환자에서 자가면역질환의 증거없이 고프로릭틴혈증을 보인 부신피질 자극호르몬 단독결핍 1례를 경험하고 1년간의 부신피질호르몬 대치요법후 추적검사를 실시하였기에 문헌 고찰과 함께 보고하는 바이다. Isolated ACTH deficiency is a rare disorder which shows secondary adrenal insufficiency with normal states of other pituitary hormones. A 56-year-old postmenopausal woman complaining general weakness and intermittent mental confusion presented negligible serum cortisol and ACTH level with hyperprolactinemia and mild hyponatremia. Rapid ACTH stimulation test did not show significant increase of serum cortisol level. Combined pituitary stimulation test showed negligible response of cortisol and ACTH, and slightly decreased response of prolactin and gonadotropin. But the responses of TSH and GH were normal. There was no evidence of organic pituitary of hypothalamic lesion in brain MRI and antithyroid autoantibody was not detected in serum. After receiving 12 months of glucocorticoid replacement therapy, she skipped the medicine for 3 weeks and we performed follow-up endocrinologic tests. Serum ACTH and cortisol level was also negligible and prolonged ACTH simulation test showed normal response of serum cortisol. CRF stimulation test showed no response of serum ACTH. Basal prolactin level was decreased to near normal and combined pituitary stimulation test showed normal response of prolactin with slightly blunted response of TSH and GH. We concluded that the case was nonautoimmune origined isolated ACTH deficiency which was accompanied by hyperprolactinemia and resultant deficiency of gonadotropin, so we report this case with review of literatures.

림프종 환자에서의 갈륨, TI-201 그리고 Tc-99m MIBI 섭취의 비교

Purpose; Ga-67 scintigraphy has been used for the evaluation of tumors, especially lymphoma. Recently, Tl-201 and Tc-99m MIBI were also used to tumor imaging. Tl-201 and Tc-99m MIBI had better physiologic characteristics than Ga-67, so we studied 32 biopsy proven lymphoma patients (male 24, female 8, mean age 46 years) with Ga-67, Tl-201 or Tc-99m MIBI and compared the scan findings. Materials and Methods: Twenty-three of 32 patients were injected 74-111 MBq (2-3 mCi) of Tl-201, before chemotherapy and imaged with dual-headed SPECT (Prism 2000, Picker, USA) at 30 minutes after injection. Delayed images were obtained after 3 hr in 8 patients. Twenty seven of 32 patients were injected 740 MBq (20 mCi) of Tc-99m MIBI and imaged at 30 minutes after injection. 111-185 MBq (3-5 mCi) of Ga-67 was injected in 12 patients and imaged at 48 and 72 hours after injection. Twenty eight patients were diagnosed as non-Hodgkin's lymphoma and others were Hodgkin's lymphoma. Results: Twenty patients were positive on Tl-201 scan and 3 patients showed negative findings. One of these 3 patients, Tc-99m MIBI and Ga-67 scan were positive. Twenty two patients were positive on Tc-99m MIBI scan and 5 patients showed negative findings. One of these 5 patients, Tl-201 was positive and 2 were positive on Ga-67 scan. Ten of 12 patients showed positive findings on Ga-67 scan. The sensitivity of these agents were 83.3%, 87.0% and 81.5% for Ga-67, Tl-201 and Tc-99m MIBI, respectively. The sensitivity was highest in Tl-201 scan, but there were no significant differences among three tests. In this study, there was no significant difference of uptake ratios between early and delayed images of Tl-201. Conclusion: Scintigraphy with Tl-201 and Tc-99m MIBI in lymphoma patients have similar sensitivity with Ga-67.

韓.中 關係를 결정하는 要因들에 대한 연구

본 논문은 한·중관계를 연구하는 데 있어서 중요한 요인들을 분석한 것이다. 한·중관계에서 가장 중요한 요인은 지정학적 요인으로 볼 수가 있다. 地政學的 요인은 양국관계를 결정하는 기본요인으로 매우 중시 받아야 할 것이며, 그것은 예나 지금이나 마찬가지이다. 다라서 본 논문에서는 이웃이라는 地政學的 관계의 영향을 받아 '脣亡齒寒'의 관계라고 불리어지던 歷史的 한·중관계의 결정 요인을 분석하고 더불어 政治·軍事的 要因, 文化的 要因,經濟的 要因으로 나누어서 현대 한·중관계를 결정하는 要因들을 평가해보고자 한다. The purpose of this paper is to analyze important factors for studying the relation between Korea and China. the most important factor in the Korean-Sino relation we consider is the geopolitical one developed on the bassis of nearest neighbor countries. This geopolitical factor between the Korea and China must be stressed because it has a wide influence over mutual relations between two countries, at this paper also analyze the meaning of the political-military, cultural and economic factors in Korean-Sino relation.

DTV를 위한 부가서비스 구현

DTV has the advantage of packet communication, because it is configured by packet streams. Through this method, all video, audio and data in the transmission media will be converted to digital, and then multiplexed by a 188 bytes TS(transport packet stream). We can then add other valuable packet data on it. Therefore, using the information of program guides(PG), we can get the whole PG informations on the TV screen fast and easily. In this paper, we implemented the PG function to be used in HDTV, according to the ATSC standard, and suggested valuable usages in a TV receiver. It was made with visual C++, and could be useful further in the testbed design of other auxiliary data services.

전자식 ballast의 시험용 의사부하에 관한 연구

형광들을 점등시키는 안정기의 역할은 형광등과 같이 부저항특성을 갖는 방전등에 과전류가 유입되어, 파괴되지 않도록 일정전압 및 전류를 유지시켜주는 것이다. 최근 사용되고 있는 전자식안정기는 25-30KHz의 주파수를 사용, 캐패시터에 의한 안정화전류를 유도하고 있다. 그러나 전자식안정기는 고주파를 사용하는 관계로 제품의 품질보증을 위한 제반 규정이 까다로워 생산자는 개개의 안정기를 실제 형광등을 사용하여 특성검사를 하고 있다. 본 논문에서는 전자식안정기 특성검사의 효율성을 제고하기 위해 실제 형광등과 동일한 특성을 갖는 의사부하의 설계에 관하여 연구하였다. 초기의 과도 조건을 고려하지 않는 정전압 특성을 갖는 의사부하를 설계하여 실험한 결과 제한된 조건하에서 사용될 수 있음을 확인하였다. The electronic ballast has a role of taking a stable current of fluorescent lamp, which has a characteristics of negative resistance. Recently, it has been designed and operated on using the regulated constant current with capacitor and high frequency of 25 to 30KHz. But a fabricated electronic ballast in an assembly line was difficult to test, because it used a high frequency and needed strict provisions. And then manufacturer is now testing each ballast with a real fluorescent lamp. In this paper, we studied that real lamp can be replaced by a dummy load with the same characteristics.

11세 여아에서 발생된 성숙 낭성 기형종의 염종1예

Mature cystic teratomas comprise 10% of all ovarian tumors and show the highest frequency of incidence in women under 20 years of age. Torsion of ovarian cyst is the most common complication in young female and lower abdominal pain, peritonitis, necrosis and shock are outcomes that require emergency treatment. Generally, gross finding of mature cystic teratoma shows that most of the entire ovary is replaced by multiple cystic masses, and rarely only small portion of the ovary is changed into cystic mass. We report a case of torsion of mature cystic teratoma in an 11 year old girl with a brief review of literature.

JMF를 이용한 화상회의 설계 및 구현

As the network continues remarkable growth, the need of the method for efficient and favorable human-to-human communication increase. To meet the need, applications like e-mail, news and others have been developed in various network. Videoconferencing has known as a kind of these applications to communicate with one another without face-to-face meeting. The existing videoconferencing system has a drawback that is dependent on specific platform. So, there's a increase in a need of system independent platform in these days. JMF, as one of Java API, was created for the purpose of the play, transmission, reception and obtainment of time based media. Also, JMF has a platform independent quality. In this paper, a simple platform independent system was developed by using JMF.
