Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

Results : In 1997. 99 strains of N. gonorrhoeae were isolated, among which 45(45.5%) were PPNG. Conclusion : The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 45.5% in 1997.

We report a case of tsutsugamushi disease in a 26 year-old pregnant woman who was treated with azithromycin. Her gestation period was 27 weeks and she admitted with fever, rash, and eschar on the right shoulder. Currently recommended medications for the treatment of scrub typhus are doxycycline or chloramphenicol. But, these drugs are class D drugs according to the U.S. Food and Drug Administration (FDA) Fetal Risk Summary, so they couldn't be used to treat pregnant women. Recently, a few case reports suggested that azithromycin, a relatively new macrolide antibiotic, was effective and safe for the treatment of scrub typhus in pregnant women. And, there is no evidence that azithromycin causes harmful effects to the developing fetus or to children. On the basis of current in vivo test that confirms the effectiveness of azithromycin, it may be the drug of choice for the treatment of scrub typhus in pregnant women and children. Background : Bleeding after endoscopic procedure of the stomach carries an increased risk of morbidity and mortality. In case of oozing hemorrhage, not in active hemorrhage, powdered sodium alginate may play a role in control of bleeding with convenience. Therefore, we evaluated the clinical effect and efficacy of sodium alginate powder in upper G-Ⅰ bleeding. Methods : Forty-five patients who had upper G-Ⅰ bleeding after endoscopic procedure were enrolled in this study. Twenty-five patients of them were managed by the application of powder preparation of Sodium alginate(ALTO). The others were observed without treatment. The next day, we performed endoscopy in all patients to ascertain the hemostasis. Results : Of 25 patients managed with Sodium alginate, only two patients(8%) had bleeding (hemostatic rate : 92%), On the other hand, of 20 patients who did not receive any treatment, 3 patient(15%) had bleeding (hemostatic rate : 85%). There was no statistical difference between the two groups(p>0.05). Using other endoscopic procedure, We successfully treated the patient who continued to bleed. Conclusion : We evaluated the effect of the spary of sodium alginate on oozing hemorrhage. Although there was no statistical significance in our study, it seems that the spray of sodium alginate may be a useful method for the prevention of bleeding after endoscopic procedure.

Since the long-term exposure of mutans streptococci to xylitol is known to select for xylitol-resistant(X^(R)) natural mutants, the occurrence and survival of such X^(R) strains were performed in batch culture methods. The aim of the study was to compare the differentiation and quantification of mRNA expression of the gtf genes of X^(R) and X^(S) mutans streptococci. Using a real-time reverse-transcription polymerase chain reaction, the expression of each gtf was determined. In X^(R) strains, the relative levels of transcription of gtfB and gtfC were decreased while that of gtfD was increased, suggesting the presence of independent promoters. It also suggested that mutation related to production of glucosyltransferase occurred under the exposure of xylitol could explain the caries-preventive mechanisms of xylitol. 자일리톨에 장기간 노출된 mutans streptococci는 자일리톨에 내성이 발현되어 자일리톨 내성균주가 생성된다고 알려져 있다. 본 연구의 목적은 mutans streptococci에서 자일리톨 내성균주와 감성균주의 gtf 유전자 발현량을 각각의 유전자별로 정량적으로 분석하고 비교하는 것이다. 실시간 역전사 중합효소연쇄반응법을 이용하여 각각의 gtf 발현을 조사한 결과 gtfD는 증가한 반면. gtfB와 gtfC는 감소하였는데 이는 각 유전자의 독립된 조절기전이 존재함을 보여주는 것이다. 또한 자일리톨에 노출된 mutans streptococci에서의 glucosyltransferase와 연관된 유전자변형이 자일리톨의 치아우식증 예방효과의 작용기전 중 하나임을 알 수 있었다.

The rsmA gene was cloned from soft-rot bacterium Pectobacterium carotovorum subsp. carotovorum LY34 (Pcc LY34), and its role in pathogenicity was investigated by marker exchange mutagenesis. From a cosmid library of Pcc LY34 genomic DNA, a positive clone carrying the rsmA gene was selected, and the gene was cloned by polymerase chain reaction (PCR) amplification. The gene is 186 bp in size and encodes a protein of 62 amino acids with a predicted molecular mass of 6,839 Da. The calculated pI of the RsmA is 8.16. The phylogenetic tree showed that the RsmA of Pcc LY34 appeared genetically identical to the CsrA of Pectobacterium atrosepticum SCRI1043 (100% identity) and similar to the CsrA of Yersinia pestis KIM10+(98.3%). The gene was disrupted by the $Km^r$ gene, and the cells became mutated (i.e., $RsmA^-$ mutant). The pathogenicity test revealed that the disease rating of the $RsmA^-$ mutant only differed slightly from that of the wild type on a slice of potato tuber and a Chinese cabbage stalk. These results suggest that RsmA is not an essential factor for the pathogenicity of Pcc LY34 and that the rsmA gene of Pcc LY34 is not completely derepressed in the $RsmA^-$ mutant for virulence-related genes, contrary to the results of Erwinia carotovora subsp. carotovora $RsmA^-$ mutant, which proved hypervirulent for celery petioles. These results showed that the microenvironmental conditions of the host and/or strain of pathogen are important for the coordination of virulence gene expression.
