        The author studied the heart rate changes of 150 adult patients (79 male and 71 females above 18-years old) under dental local anesthesia, and heart rate changes for breathholding alone of 20 patients. The heart rate is best reflection of the activity of the autonomic nerve system and the easiest physiological sign to monitor. The purpose of this study was to find out cardiac activity under dental local anesthesia, thereafter to clarify the mechanism of syncope from the aspect of trigeminal-vagal reflex. The studied group were devided into four. GⅠ: Local anesthesia with a different agent by epinephrine concentration and normal saline as a control. GⅡ: Local anesthesia after premedication of atropione. GⅢ: Local anesthesia under the general aensthesia. GⅣ: Heart rate changes for breathholding alone. The result were as follows: The heart rate was decreased in 138(92%) of 150 patients during local anesthetic injection, and increascd after injection. This means the possible participation of trigeminal-vagal reflex in producing heart rate decrease. Mean maximum decrease rate of G I was 14.2±7.54%, and not affected by anesthetic agents(p>0.05) Mean value (GⅡ) was 6.89±4.38%, which means that atropine inhibits the reflex partially(p<0.05). Mean value (GⅢ) was 1.20±0.77%, which means that general anesthesia blocked the reflex completly. Mean value (GⅣ) was 6.58±3.13%, which is not reached to changes by local anesthesia.

      • 점토굴착 사면의 거동에 관한 원심모형실험 및 수치해석

        This paper is the results of experimental and numerical works on analyzing the geotechnical engineering behavior and characteristics of excavated clay slope formed by the method of excavated replacement which is one of treatments in soft soil ground. For the centrifuge model tests, models of excavated clay slope were prepared by remolding the marine clayey soil sampled from the field. Tests were performed with changing the slope to investigate the behavior of them. On the other hand, numerical analyses were carried out to analyze the change of safety factor against instability of slope with time. Changes of pore water pressure, shear strength and displacement were also investigated. As results of centrifuge model tests with slopes of 1:1.5 and 1:3 using the confining body of simulating the effect of excavation, for the case of 1:1.5, slope failure occurred right after remove the confining body whereas relatively small displacements within the range of 3.2mm, implying to maintain the stability of slope, were observed for the case of 1:3 slope. From the results of numerical analyses using the software of PLAXIS to investigate the stability of slope after excavation, the minimum safety factor against slope failure was 1.28 for the case of 1:3 slope. The further researches in the future are required with considerations of build up of static pore water pressures during acceleration of centrifuge, depth of excavation influencing the behavior of the slope and permeability of the slope since excavation of the slope was not simulated well resulted from the limitations of apparatus at the stage of excavation during the centrifuge tests.

        Pseudoaneurysms form when a hemorrhage or hematoma from the injured arterial wall is replaced with an fibrous vessel wall, which is dilated by subsequent pulsating pressure. Recently, we experienced a patient with a pulsatile preauricular mass, measuring about 1.5× 1.5 cm, sustained from blunt trauma by the baseball. The presumptive diagnosis of pseudoaneurysm of the superficial temporal artery was made by Doppler sonography. Under local anesthesia, the aneurysm was excised en bloc following ligation. We report the rare case with a review of literatures.

        Hypophosphatasia 환아의 치료 증례

        1. Hypophosphatasia 는 골조직 및 치아조직의 비정상적인 석회화를 나타내는 대사장애로, 특징적구강내 소견 인 우치의 조기탈락 양상으로 치과에서 조기진단이 가능하다. 2. 본 증례에서 상악궁 확장을 동반한 의치 제작으로 심미적, 기능적으로 양호한 결과를 얻었으며, 영구치열기 까지 장기적 관찰 및 치료가 필요할 것으로 사료된다. hypophosphatasia is a rare metabolic disorder which manifests characteristics such as abnormal mineralization of bone and dental tissues, diminished serum and tissue alkline phosphatase, and increased urinary secretion of PEA. It ingerited as an autosomal recessive or dominant trait and occurs in all races. In general, hypophosphatasia can be classified in 4 subtypes which are the perinatal, infantile, childhood, adult type depending upon the age at presentation and severity. In young children with Hypophosphatasia the long bones show irregular defects, and the skull showes poor calcification. In older children with premature closure of the skull sutures there may ve multiple lucent area called gyral or convolutional markings, described as resimbling beaten copper, presumably resulting from increased intracranial pressure,Examination of the jaws reveals a generalized lucency of the maxilla and mandible, the cortical bone and lamina dura are thin, and the alveolar bone may be deficient. Clinical features of Hypophosphatasia imclude premature loss of deciduous teeth, especially incisors, hypoplasia of aplasia of root cementum, enamel hypoplasia, irregular calcification of dentin, large pulp chamber, and resorption of marginal alveolar bone and roots. Our report involves a patient with a chief complaint of early loss of both Mx. and Mn. deciduous ncisors. after conducting a through clinical and radiographic examination this patient was referred to pediatrics under the suspicion of hypophosphatasia, the diagnosis proved to be correct and successful results were accomplished through a denture made to improve esthetics and function.

        프레탈^(R)정(실로스타졸 50mg)에 대한 실로졸^(R)정의 생물학적 동등성

        Bioequivalence of two cilostazol tablets, the Pletaal^(R)(Korea Otsuka Pharmaceutical Co., Ltd.) and the Cilozol^(R)(Hanmi Pharmaceutical Co., Ltd.), was evaluated according to the guideline of KFDA, Sixteen normal male volunteers(age 20~29 years old) were divided into two groups and a randomized 22 cross-over study was employed. After two tablets containing 50㎎ of cilostazol were orally administered. blood was taken at predetermined time intervals and the concentration of cilostazol in plasma was determined with an HPLC method using UV detector. The pharmacokinetic parameters(C_(max), T_(max) and AUC_(t)) were calculated and ANOVA was utilized for the statistical analysis of parameters. The results showed that the differences in C_(max), T_(max) and AUC_(t) between two tablets were 4.99%, 1.74% and 7.68%, respectively. The powers(1-β) for C_(max), T_(max) and AUC_(t) were83.92%, 80.12% and 85.03%, respectively. Detectable differences(Δ) and confidence intervals were all less than 20%, and confidence interval of all the parameters were also less than 20% at the significance level(α) of 0.05. All of these parameters met the criteria of KFDA for bioequivalence, indicating that Cilozol^(R) tablet is bioequivalent to Pletaal^(R) tablet.

        A FET type micro urea sensor - very proper and promising shapes for in vivo, in situ measurements - had been examined on its compatibilities with clinical analysis. Investigations were performed on the interference of various amino acids, organic metabolites and waste products present in blood, which may cause interference with the sensor output. The results of the measurements of urea-nitrogen in blood plasma measurements was compared with those of conventional spectrophotometric method.

        피부화상으로 유도된 급성 부종성 폐손상에 대한 사순청량음의 효능

        Objectives : The present study was conducted to determine whether administration of heat extracts of Sasunchungtyangeum has an inhibitory effect of neutrophil-derived oxidative injury following dermal scald burn in rats. Methods: Acute lung injury was induced by scald burn (15% of TBSA) in rats. To identify acute lung edematous lung injury, 5 hrs after skin burn, protein concentrations and numbers of polymorphonuclear leukocytes in bronchoalveolar lavage (BAL) were measured. In addition, levels of lung KC (neutrophil chemoattractant cytokine) and activities of lung myeloperoxidas (MPO) were measured, and morphological changes were observed. Results : Lung weight and concentration of BAL protein, the index of lung injury were clearly increased at 5 hrs postburn compared to them of sham. Administration of heat extracts of Sasunchunryangeum after scald burn inhibited the production of KC in lung tissue and decreased the activity of lung MPO related to influx of neutrophils. In histological changes, edematous lung injury with infiltration of inflammatory cells induced by skin burn were decreased by administration of heat extracts of Sasunchunryangeum after scald burn. Conclusions : These results suggest that Sasunchunryangeum may be a successful medical stuff in acute lung injury by skin burn.

      • 사천만 담수호 조성을 위한 기초분석

        This analysis was executed for freshening reservoir proposed in Sachon Bay on the purpose of using inflow discharged through Sachon diversion weir from the Namgang multi-purpose dam during the flood seasons. The freshening processes were analysed based on the Minami's analytical model for a standard freshening reservoir after the final closure of a sea-dike and predicted the salinity variation of the lake during 7 years. The results of this study can be summarized as follows : 1. As the results of the desalinization process computation, reservoir water can be used for industrial and irrigation purposes in the alternative 1 reservoir, irrigation purpose in the alternative 2 reservoir. The salinity was predicted under 300ppm during the flood seasons and 800ppm during the drought seasons in the alternative 1, under 500ppm during the flood seasons and 1,000ppm during the drought seasons in the alternative 2, three years later after the final closing. The numbers of the suitable desalinization culverts were estimated as four in the alternative 1 and two in the alternative 2 scheme. 2. The freshening processes after the establishment of the desalinization culverts have a considerable effect in comparison with those without culverts. But the salinity was not improved in proportion to the increasing number of the culverts, more than four culverts in the alt. 1 and two culverts in the alt. 2. 3. The fresh-salt layer of the reservoir both alt. 2 is raised up over the sill elevation (-4m, -3m) in the drought seasons without culverts, however, the layer draws down below the sill elevation when more than two culverts are installed and are maintained under -5∼-8 meters in the alt. 1 reservoir and -3 meters in the alt. 2 reservoir. 4. Some considerations when the seashore freshening reservoir is proposed are as follows : ① The quantity of the inflow water from the surrounding basins or the upper stream areas. ② The conservable water quality including the desalinization in the freshening reservoir. ③ Economical and technical feasibility comparing with construction of dam in inland. ④ The minimization of an ecological and environmental disruption etc.
