        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

        Occupational hearing loss became the leading occupational disease by constioution 56% of all work-related diseases in Korea. However, the prevalance rates showed wide variations among the specific medical examination providers and were very low(0.8-1.4%) compared with those 8.4% in Japan. The sources of these variations was regarded in the preview study as 1) Employee audometric estings were primarily conducted by nurses and nurse-aids who had no formal training for audiometry; 2) althought audiometric testing equipment used by the spedific medical examination providers, it should be periodically calibrated, a significant number of them were not properly calibrated : 3) Initial audiometric testings were conducted at any available spaces in workplace or at the testing room without considering bachground noise level. One of solutions that can be done for sloving surrent noise related problems is to provide a noise related manual for the occupational health managers including doctors, nurses and hyginests. This study was done to provide such a manual. The main core of the manual was hearing conservation program. And the contents of the manual was the same as the main frame of hearing conservation program such as noise survey, provided of the diagram showing the core contents of each factor of the hearing conservation program. Hopefully, this manual will be the references of the standardized performances of the specific medical examination providers.

      • Results of Fertility preservation program: A single center cohort study

        목적: The aim of the study was to review the experience of fertility preservation (FP) program in a single center and to evaluate efficacy of FP referral program. 방법: From January 2013 to January 2016, the patients who have concern for their fertility that were referred to FP program were included. The age of patients at the time of discussing fertility preservation was under 40 years old. Total 103 patients who have undergone fertility preservation program in Severance hospital, Yonsei University College of Medicine were included. Statistical analyses were performed using chi-square test, Mann Whitney U test, and Kruscal-Wallis test according to the variables. 결과: Total 63 patients have undergone either embryo or oocyte cryopreservation, performing 82 controlled ovarian hyperstimulation cycles. Breast cancer patients composed the most: 15 patients (53.6%), 16 cycles of embryo; 22 patients (62.86%), 22 cycles of oocyte cryopreservation. Otherwise, patients diagnosed sarcoma, gastrointestinal, gynecological, and hematologic malignancy have been done the cryopreservation. Total 92 patients have received gonadotropin releasing hormone agonist for FP purpose. As similar as embryo/oocyte cryopreservation, patients with breast cancer composed the most: 46 patients (50%). The first 6 months follow up with serum anti-Mullerian hormone showed 0.23ng/dL (<0.08-13.41) in patients with breast cancer, 0.61ng/dL (<0.08-2.98) in patients with hematologic malignancy. The FP referral program between breast cancer clinic and infertility clinic was set since January 2015, and comparing the referral rate and FP rate between and after the FP program, significant increase was shown. However, referral and FP rate were similar in other cancers during the same time. 결론: The effective FP program between oncology department and infertility clinic seems to increase the rate of referral and participating FP program. Those who are expected to receive treatment that might affect fertility, should discuss about the FP options.

        Effects of Transcutaneous Electrical Nerve Stimulation(TENS), Self- Stretching and Functional Massage on the Muscle Fatigue by Maximum Muscular Strength

        The present study purposed to examine the effects of transcutaneous electrical nerve stimulation, self-stretching and functional massage on the recovery of muscle contraction force for muscle fatigue caused by sustained isotonic contraction. The subjects of this study were 45 healthy students. They were divided into transcutaneous electrical nerve stimulation group(n=15), self-stretching group(n=15) and functional massage group(n=15), and using Primus RS. We observed the pattern of changes in maximal voluntary isometric contraction force(MVIC) after causing muscle fatigue in quadriceps femoris muscle through sustained isotonic contraction. Maximal voluntary isometric contraction force(MVIC) were greatly increased after transcutaneous electrical nerve stimulation, self-stretching and functional massage. In the comparison of recovery rate of muscle contraction force for muscle fatigue caused by sustained isotonic contraction among the treatment groups, it did not show any significant differences. However, it showed that each treatment may be effective in recovery of muscle fatigue caused by sustained isotonic contraction.

<B>Prevalence of the <I>CTNNB1</I> mutation genotype in surgically resected fibromatosis of the breast</B></P><P><B>Aims: </B> To investigate <I>CTNNB1</I> mutation and β‐catenin expression in resected breast fibromatosis and to identify potential molecular markers of fibromatosis of the breast.</P><P><B>Methods and results: </B> We selected 12 patients with fibromatosis of the breast who underwent surgical resection and were confirmed by histological examination. Ultrasonography findings for 10 patients were reviewed and only two cases were suspicious for fibromatosis on imaging. On core needle biopsy for pre‐operative diagnoses, only three cases were histologically suspicious for fibromatosis. Mutations in exon 3 of <I>CTNNB1</I> were detected by direct DNA sequencing in nine (75.0%) cases: all were c.121G>A (p.T41A), which was much more frequent in breast fibromatoses than in other soft tissue lesions. Nuclear β‐catenin expression was observed in all cases and the level of expression was higher in cases with mutation. In eight of nine cases, the matched biopsy specimen showed the same <I>CTNNB1</I> mutation status as the pre‐operative specimen.</P><P><B>Conclusions: </B> In the majority of cases, clinical presentation and breast imaging are highly suspicious for carcinoma. Definitive pre‐operative pathological diagnosis by core needle biopsy is difficult. <I>CTNNB1</I> mutation and nuclear β‐catenin expression are frequently detected in sporadic breast fibromatoses, suggesting their potential as a useful tool to distinguish breast fibromatoses from other neoplasms.</P>

        MD-2 as the Target of Nonlipid Chalcone in the Inhibition of Endotoxin LPS-Induced TLR4 Activity.

        <P>Myeloid differentiation 2 (MD-2) recognizes endotoxin lipopolysaccharide (LPS), which is required for Toll-like receptor 4 (TLR4) activity. MD-2 represents a more attractive therapeutic target than TLR4 for intervention in severe inflammatory disorders due to microbial infection. Here, we suggest MD-2 as a molecular target of nonlipid chalcone in the inhibition of LPS-induced cellular inflammation. A chalcone derivative, 2',4-dihydroxy-6'-isopentyloxychalcone (JSH) competitively displaced LPS from MD-2, and was fitted into the ligand-binding site on the crystal structure of MD-2 under the most energetically favorable simulation. JSH nullified TLR4 activation mechanism and sequentially inhibited nuclear factor-관B (NF-관B) activation that involves the phosphorylation and degradation of inhibitory 관Bs and the nuclear import and transcriptional activity of NF-관B in LPS-activated macrophages. Moreover, JSH suppressed NF-관B-target inflammatory genes such as inducible nitric oxide synthase, cyclooxygenase-2, interleukin-1관 (IL-1관) and IL-6. Taken together, this study assigns the chalcone structure as an LPS antagonist binding to MD-2 with therapeutic potential against inflammatory conditions.</P>

        Templated Formation of Silver Nanoparticles Using Amphiphilic Poly(epichlorohydrine-g-styrene) Film

        This work has demonstrated that a novel amphiphilic poly(epichlorohydrine)-graft-polystyrene (PECH-g-PS) copolymer at 34:66 wt% was synthesized via atom transfer radical polymerization (ATRP) of styrene using PECH as a macroinitiator. The structure of the graft copolymer was characterized by nuclear magnetic resonance ($^1H$ NMR) and FTIR spectroscopy, demonstrating that the "grafting from" method using ATRP was successful. The self-assembled graft copolymer was used as a template film for the in-situ growth of silver nanoparticles from $AgCF_3SO_3$ precursor under UV irradiation. The in situ formation of silver nanoparticles with 6-8 nm in average size in the solid state template film was confirmed by transmission electron microscopy (TEM), UV-visible spectroscopy and wide angle X-ray scattering (WAXS). Differential scanning calorimetry (DSC) also displayed the selective incorporation and the in situ formation of silver nanoparticles within the hydrophilic PECH domains, probably due to stronger interaction of the silvers with the ether oxygens of PECH backbone than that with hydrophobic PS side chains.
