        Transcatheter arterial chemoembolization (TACE) has become an effective alternative treatment strategy for patients with inoperable hepatocellular carcinoma (HCC). Although TACE is relatively safe, acute respiratory distress syndrome associated with pulmonary lipiodol embolism is a rare and potentially fatal complication. We report a rare case of acute respiratory distress syndrome after TACE for inoperable HCC. A 75-year-old man, with huge HCC in right lobe, was treated by TACE for the first time. Seven hours after uneventful TACE procedure, he felt dyspneic and his oxygen saturation recorded by pulse oximetry (SpO2) fell to 80% despite of applying non-rebreathing mask. He underwent mechanical ventilation with a protective ventilatory strategy. We experienced a case of acute respiratory distress syndrome after TACE for HCC.

        Reliability of rehabilitative ultrasound imaging for measuring the gluteus maximus muscle at rest and during contraction

        Objective: The aim of this study was to investigate the inter-rater and intra-rater reliability of rehabilitative ultrasound imaging (RUSI) for measurement of muscle thickness with changes in angles of the gluteus maximus (GM) at rest and during contraction. Design: Cross-sectional study. Methods: Twenty-two healthy men volunteered for this study. GM muscle images were obtained in the resting position and during prone hip extension with knee flexion at hip abduction angles of $0^{\circ}$ and $30^{\circ}$, respectively. Two examiners randomly measured the thickness of the GM twice in three different positions. The first position was a comfortable prone position. The second position was prone hip extension with knee flexion (PHEKF) to $90^{\circ}$. The third position was achieved by hanging a 1-kg weight on the ankle of the lifted leg during PHEKF with the angle of the lifted leg the same as the second position. Intra-class correlation coefficients (ICCs), standard error measurements, and minimal detectable changes were used to estimate reliability. Results: The intra-rater reliability ICCs (95% confidence interval) of the GM were >0.870, indicating good reliability. Inter-rater reliability ICCs ranged from 0.668 to 0.913. The reliability of measurements of muscle thickness at each position was similar to the reliability of the angle change. Differences in muscle thickness and ratios for each position with $0^{\circ}$ and $30^{\circ}$ of hip abduction were not statistically significant. Conclusions: In the present study, the intra-rater reliability of muscle thickness measurements of the GM was good, and the inter-rater reliability was moderate to good. Reliable RUSI measurements of wide and large muscles, such as the GM muscle at rest and during contraction, are feasible. Further investigation is required to establish the reproducibility of the protocols presented in this study.

        Case Report : Kinematic analysis of rowing exercise using a motor-assisted rowing machine for rowers with spinal cord injury: a case report

        Objective: We developed a Motor-Assisted Rowing Machine (MARM) for Spinal Cord Injury (SCI), by modification of the Concept II rowing machine, so that the seats could be operated automatically in a backward and forward direction by a motor. Design: Case report. Methods: Motor rowing consisted of a chair with inclination control, a motor system, control button, monitor, program, leg supporter, safety belt, and seat. The patients were 2 men rowing athletes with SCI, classified as American Spinal Injury Association class B, participated in the study. Level of thoracic injury ranged from T8 to T10. The subjects rowed at a self-selected stroke rate with 50 watts. Two different rowing methods (static rowing without movementof the seat, dynamic rowing using MARM) were assigned to each participant during 10 minutes; 34 reflective markers were attached to their full bodies. Kinematic data were collected using the Vicon motion analysis system. Based on the full body model provided as a default by the equipment. In the rowing exercise, the rowing motions were divided into Drive Phase and Recovery Phase. Results: The two rowing methods differ in handle range, seat range, handle and seat ratio, handle velocity, and seat velocity during static and dynamic rowing. The rowing exercise using a rowing machine developed MARM increased tendency to the range of motion in the dynamic method compared to the static method. Conclusions: The newly developed MARM could be a useful whole body exercise for people with SCI.

        Objective: This study aimed to investigate the reliability and validity of a personal computer-based muscle viewer (PC-BMW) compared with that of a portable ultrasound (P-US) for measuring upper trapezius (UT) and transversus abdominis (TrA) muscle thickness at rest and during contraction. Design: Observational inter-rater reliability study. Methods: Fifty-five healthy participants (25 men, 30 women) participated in this study. PC-BMW and P-US were randomly measured at the UT and TrA muscles. Two examiners randomly obtained the images of all participants in 3 test sessions lasting 2 days. Intra-class correlation coefficients (ICCs), standard error of measurement, contraction ratio, and correlation were used to estimate reliability and validity. Pearson's correlation coefficients were used to analyze the relationship between muscle thickness measures taken from PC-BMW and P-US. Results: The intra-rater reliability ICCs of UT and TrA muscle thickness for the PC-BMW were >0.995, indicating excellent reliability. Inter-rater reliability ICCs for the PC-BMW ranged from 0.963 to 0.987. The P-US also exhibited high reliability. A high correlation was found between the measurements of the two muscles in PC-BMW and P-US (p<0.01). Conclusions: PC-BMW provides clear and excellent images, is pocket-sized and less expensive than a conventional ultrasound imaging system. PC-BMW can be utilized variously and has the advantage of rehabilitative ultrasound imaging. More research is needed to evaluate the utility of PC-BMW for rehabilitation.

        문법연구 : 대학 글쓰기 수업개발 방안 -전문대학 학생들의 글쓰기에 나타난 문법적 오류 유형을 중심으로-

        This study has its purpose on proposing appropriate goal and method of writing class by analyzing the problems and types of error found in students’ writings. It is desirable to set both the class context and goal on the basis of assessment on students’ writing competence. Especially in the case of a vocational college, the education period is short and the liberal arts classes are not various. Therefore, in this practical situation, the goal of the writing class that is opened as a single subject should clearly reflect the industry’s demand. Recently, while the general core competency that the NCS (National Competency Standards) is demanding sets communication ability as an important capability, there are many such demands in the industry. This research extracted vocational college students’ writing problems by comparing the competency of vocational college students and that of top university students within the same age. Conclusively, it has turned out that vocational college students significantly lacks structural logic in their writing abilities. As a result, it has led to short writings, short sentences, many errors in logic between sentences and incomplete paragraphs. To address these issues, it is suggested that logical structure practice learning that focuses on making outlines be applied in writing classes. Especially, based on cognitive psychology approach, visualizing outline byusing figures in classes is suggested. Its purpose is on enhancing the ability of logically composing through these practice, which will, in turn, lead to improvement on general logic of communication ability needed in general core competency

        대학생의 사고력을 높일 수 있는 쓰기와 토의 활동의 전공 연계 학습 모델

        최근에 대학 글쓰기 교과 기능이 기초글쓰기 또는 계열별 글쓰기에서 전공지식과 내용을 주제로 한 전공연계 글쓰기(WAC)로 전환하는 시점에 있다. 이러한 맥락에서 본고에서는 글쓰기의 선행 경험이 매우 부족한 전문대학생들을 대상으로 1차적으로 기초글쓰기를 지속적으로 습관화할 수 있는 전공글쓰기 프로그램인 <학습 활동지>작성 운영과정과 결과를 제시하였다. 또 본격적인 전공지식을 주제로 글쓰기를 하는 <프로젝트 전문세미나>교과목의 운영과정과 결과를 제시하였다. 두 개의 과정을 통해 학생들이 글쓰기에 대한 기초력을 다지고 지식의 구조를 체계화하는 논리성고 문장력도 강화하였다. 향후 4차산업 혁명이 주도하는 미래사회의 변화 속에서 창의융합형 인재에 대한 수요를 충족하기 위해서는 전공연계 글쓰기와 같은 생각하는 수업 모델이 개발되어야 할 것이다. Recently, university writing curriculum functions are at the point of transition from basic writing or writing by division to major-linked writing (WAC) based on major knowledge and content. In this context, this paper presents the operation process and results of < Learning Activity Sheet >, a major writing program that allows students to continue to habituate basic writing primarily for vocational college students who have very little prior experience in writing. In addition, the operation process and results of the < Project Seminar >, where writing on the subject of full-scale major knowledge, was presented. Through the two courses, students have strengthened their basic skills in writing and strengthened their logic and writing skills to systematize the structure of their knowledge. In order to meet the demand for creative and convergent talent amid the changes of the future society led by the 4th Industrial Revolution, a thinking class model, such as major-linked writing, should be developed.
