Primary apocrine adenocarcinoma (PAA) of the eyelid is a very rare tumor that originates from apocrine glands, such as the glands of Moll. A 52-year-old man presented with a 0.1 x 0.1 cm sized small painless vesicular papule on his left upper eyelid. The lesion was well-demarcated and pedunculated, with a smooth surface. It was initially thought to be a benign tumor, such as a syringoma or an apocrine hidrocystoma. We removed the tumor by shaving with carbon dioxide laser ablation and then performed a biopsy. On histopathology, tumor cells originating from the apocrine glands were distributed in a well-circumscribed cribriform pattern with mild cellular atypia. The patient was diagnosed with PAA. We report this rare case of apocrine carcinoma with atypical clinical and histopathologic findings.

A new ultrasonic-assisted micellar extraction and cloud-point pre-concentration method was developed for the determination of major saikosaponins, namely saikosaponins -A, -C and -D, in Radix Bupleuri by high performance liquid chromatography with evaporative light scattering detection (HPLC-ELSD). The non-ionic surfactant Genapol X-080 (oligoethylene glycol monoalkyl ether) was chosen as the extraction additive and parameters affecting the extraction efficiency were optimized. The highest yield was obtained with 10% (w/v) Genapol X-080, a liquid/solid ratio of 200:1 (mL/g) and ultrasonic-assisted extraction for 40 min. In addition, the optimum cloud-point pre-concentration was reached with 10% sodium sulfate and equilibration at $60^{\circ}C$ for 30 min. Separation was achieved on an Ascentis Express C18 column (100 ${\times}$ 4.6 mm i.d., 2.7 ${\mu}M$) using a binary mobile phase composed of 0.1% acetic acid and acetonitrile. Saikosaponins were detected by ELSD, which was operated at a $50^{\circ}C$ drift tube temperature and 3.0 bar nebulizer gas ($N_2$) pressure. The water-based solvent modified with Genapol X-080 showed better extraction efficiency compared to that of the conventional solvent methanol. Recovery of saikosaponins ranged from 93.1 to 101.9%. An environmentally-friendly extraction method was successfully applied to extract and enrich major saikosaponins in Radix Bupleuri.

Background: Photodynamic therapy (PDT) has been used for acne, with various combinations of photosensitizers and light sources. Objectives: We aimed to evaluate the effectiveness and safety of indocyanin green (ICG) and intense pulsed light (IPL) in the treatment of acne. Methods: A total of 1,213 patients with facial acne were retrospectively reviewed. Patients received three or five treatments of ICG and IPL at two-week intervals. Clinical response to treatment was assessed by comparing pre- and post-treatment clinical photographs and patient satisfaction scores. Conclusion: These results suggest that PDT with ICG and IPL can be effectively and safely used in the treatment of acne.

A novel green aqueous mobile phase modified with room temperature ionic liquids (RTILs) was employed in the absence of volatile organic solvents or ion-pairing reagents to analyze thiamine, a very polar compound, by reverse phase high performance liquid chromatography (RP-HPLC). Due to its strongly hydrophilic nature, thiamine was eluted near the column dead time ($t_0$) using a mobile phase without adding RTILs or ion-pairing reagents, even if a 100% aqueous mobile phase, which has weak elution power under reverse phase conditions, was used. Thus, 1-ethyl-3-methyl-imidazolium hexafluorophosphate ([EMIM][$PF_6$]), which has the strongest chaotropic effect, was selected as a mobile phase additive to improve retention and avoid baseline disturbances at $t_0$. Various mobile phase parameters such as cation moiety, chaotropic anion moiety, pH and concentration of RTILs were optimized to determine thiamine at the proper retention time. Method validation was performed to assess linearity, intra- and inter-day accuracy and precision, recovery and repeatability; all results were found to be satisfactory. The developed method was also compared to the current official United States Pharmacopoeia (USP) and Korean Pharmacopoeia (KP) methods using an organic mobile phase containing an ionpairing reagent by means of evaluating various chromatographic parameters such as the capacity factor, theoretical plate number, peak asymmetry and tailing factor. The results indicated that the proposed method exhibited better efficiency of thiamine analysis than the official methods, and it was successfully applied to quantify thiamine in pharmaceutical preparations.
An epidemiological study of alopecia in 4,012 Korean patients: 10-year single center follow up

Background: There have been many clinical studies on individual alopecic disorders, but long-term reports of overall alopecia patients are rare. Objectives: The aim of this study is to investigate the clinical and epidemiologic aspects of diseases occurring in alopecia in 4,012 patients over 10 years. Methods: From January 2006 to December 2015, clinical records of 4,012 alopecic patients who visited Chung-Ang University Hospital were analyzed for diagnosis, age, sex, and annual distribution. Results: During the last 10 years, clinically diagnosed alopecia patients comprised 5.32% of the total number of dermatology patients, showing composition of androgenic alopecia (68.1%), alopecia areata (18.0%), telogen effluvium (7.4%), and trichotillomania (1.5%). The overall ratio of male to female patients was 1.3:1. In particular, androgenic alopecia showed male predominance (M:F=2:1) with increasing proportion of female patients compared to 4:1 ratio described in 2006, while alopecia areata showed similar distribution (M:F=1:1.1). There was a trend of lowering in the age of onset of alopecia in both genders, and more than 36.6% of patients visited the clinic within one year of onset. Family history was present in 41.5% of androgenic alopecia patients and 12.9% of alopecia areata. Conclusion: In this study, we could provide the essential findings from clinical data of various alopecia patients over recent 10 years.
Two cases of lymphomatoid papulosis treated by oral methotrexate

Lymphomatoid papulosis (LyP) is a chronic papulonecrotic or papulonodular skin disease with histologic features suggestive of a malignant lymphoma. The management options for LyP have been described, including simple observation, oral antibiotics, surgical excision, oral or subcutaneous methotrexate, intralesional corticosteroids, and radiation therapy. Methotrexate (MTX) has been reported to be safe and effective in LyP. We report two cases of LyP treated by low-dosey oral MTX administration. The first patient, a 31-year-old woman presented with a 2-year history of multiple erythematous papules with central ulceration on face, trunk and both extremities. The second patient, a 33-year-old woman presented with a 10-week history of erythematous vesicle-like appearance nodules on the legs. Histopathological findings of two cases confirmed the diagnosis of CD30+ LyP. Oral administration of low-dose methotrexate (7.5-15.0 mg/week) was found to be dramatically effective in resolution of skin lesions in both cases.
Reactive eccrine syringofibroadenoma with cobblestone appearance secondary to exfoliative dermatitis

Reactive eccrine syringofibroadenoma (ESFA) is a rare adnexal tumor of eccrine ductal differentiation secondary to primary cutaneous diseases. A 73-year-old man presented with erythematous scaly patches and plaques on the nearly whole body for 20 years. Also, physical examination revealed multiple pinkish confluent nodules merged as cobblestone appearance on the both lower legs for 2 years. The histopathologic findings is consistent with EFSA on the pinkish nodule, and exfoliative dermatitis on the erythematous patch. Here in, we report an unprecedented case of reactive EFSA with unusal manifestation secondary to exfoliative dermatitis.

Background: Platelet rich plasma(PRP) is widely used for hair growth treatment. However, the mechanisms of PRP and combination of growth factors are not fully understood yet. In an effort to develop new therapies to enhance hair growth, we screened PRP and combination of growth factors of it Objectives: We investigate the growth factors in PRP and compare the effect on hair growth between PRP and combination of growth factors Methods: We examined the effect of PRP and combination growth factors which mixed in different ratio on human dermal papilla cells and C57BL6/J mice using western blot analysis, real-time PCR and immunocytochemistry. Also, we did organ cultures of combining different growth factors for measuring hair length. Results: Combination of EGF and PDGF-AA (1:1 ratio) induced synergistic hair growth and anagen induction activity by regulation of hair growth-related signaling pathways and molecules in hDPCs and C57BL6/J mice model. Conclusion: Combination of EGF and PDGF-AA (1:1 ratio) can be further pharmaceutically developed as a therapeutic material against hair loss and alopecia in human.
