        Conceptual Study for Tissue-Regenerative Biodegradable Magnesium Implant Integrated with Nitric Oxide-Releasing Nanofi bers

        The excessive initial corrosion rate of Mg is a critical limitation in the clinical application of biodegradable Mg implantsbecause the device loses its fi xation strength before the fractured bone heals. This study suggests a new approach to overcomethis hurdle by accelerating tissue regeneration instead of delaying the implant biodegradation. As angiogenesis is anessential process in early bone regeneration, a Mg implant coated with electrospun nanofi bers containing nitric oxide (NO),which physiologically promotes angiogenesis, is designed. The integrated device enables adjustable amounts of NO to bestored on the NO donor-conjugated nanofi ber coating, stably delivered, and released to the fractured bone tissue near theimplanted sites. An in vitro corrosion test reveals no adverse eff ect of the released NO on the corrosion behavior of the Mgimplant. Simultaneously, the optimal concentration level of NO released from the implant signifi cantly enhances tube networkformation of human umbilical vein endothelial cells without any cytotoxicity problem. This indicates that angiogenesis canbe accelerated by combining NO-releasing nanofi bers with a Mg implant. With its proven feasibility, the proposed approachcould be a novel solution for the initial stability problem of biodegradable Mg implants, leading to successful bone fi xation.

        C-reactive protein accelerates DRP1-mediated mitochondrial fission by modulating ERK1/2-YAP signaling in cardiomyocytes

        C-reactive protein (CRP) is an inflammatory marker and risk factor for atherosclerosis and cardiovascular diseases. However, the mechanism through which CRP induces myocardial damage remains unclear. This study aimed to determine how CRP damages cardiomyocytes via the change of mitochondrial dynamics and whether survivin, an anti-apoptotic protein, exerts a cardioprotective effect in this process. We treated H9c2 cardiomyocytes with CRP and found increased intracellular ROS production and shortened mitochondrial length. CRP treatment phosphorylated ERK1/2 and promoted increased expression, phosphorylation, and translocation of DRP1, a mitochondrial fission-related protein, from the cytoplasm to the mitochondria. The expression of mitophagy proteins PINK1 and PARK2 was also increased by CRP. YAP, a transcriptional regulator of PINK1 and PARK2, was also increased by CRP. Knockdown of YAP prevented CRP-induced increases in DRP1, PINK1, and PARK2. Furthermore, CRP-induced changes in the expression of DRP1 and increases in YAP, PINK1, and PARK2 were inhibited by ERK1/2 inhibition, suggesting that ERK1/2 signaling is involved in CRP-induced mitochondrial fission. We treated H9c2 cardiomyocytes with a recombinant TAT-survivin protein before CRP treatment, which reduced CRP-induced ROS accumulation and reduced mitochondrial fission. CRP-induced activation of ERK1/2 and increases in the expression and activity of YAP and its downstream mitochondrial proteins were inhibited by TAT-survivin. This study shows that mitochondrial fission occurs during CRPinduced cardiomyocyte damage and that the ERK1/2-YAP axis is involved in this process, and identifies that survivin alters these mechanisms to prevent CRP-induced mitochondrial damage.         병원의 품질경영혁신 활동과 조직성과의 관계에서 성과측정시스템의 역할에 관한 연구

        본 연구는 병원의 품질경영혁신(TQM) 활동과 조직성과의 관계에서 성과측정시스템(PMS)의 이용방식과 공정성이 매개역할을 하는지를 규명하고자 하였다. 연구목적을 달성하기 위해 투입 차원은 병원 TQM 활동의 수준으로, 전환 차원은 PMS 진단 및 상호작용적 이용방식과 PMS 공정성 수준으로, 산출 차원은 조직의 성과(구성원 태도, 재무/비재무성과)로 설정하였다. 연구가설 1은 병원 TQM 활동의 수준과 조직성과의 관계, 가설 2는 그 관계에서 PMS 이용방식의 역할, 그리고 가설 3은 병원 TQM 활동수준, PMS 이용방식, PMS 공정성, 그리고 조직성과의 순차적인 인과관계를 규명하는 것이다. 본 연구의 결과는 다음과 같다. 첫째, 병원의 TQM 활동수준은 구성원 태도와 비재무성과에 정(+)의 영향을 미쳤다. 그러나 재무성과에는 유의한 영향을 미치지 않았다. 둘째, 병원의 TQM 활동수준과 조직성과의 관계에서 PMS 진단적 이용은 매개역할을 하였으나, PMS 상호작용적 이용은 매개역할을 하지 못하였다. 셋째, 병원이 TQM 활동을 추진하면서 PMS 진단적 이용을 선택한다면 구성원의 태도 변화를 경유 하여 재무/비재무성과가 개선되었다. 그런데 PMS 상호작용적 이용을 선택한 경우에는 PMS의 공정성 수준이 높아져야지만 구성원의 태도가 변하고 재무/비재무성과의 개선으로 연계되었다. 본 연구는 병원이 TQM을 추진하는 과정에서 경영통제 차원에서 성과측정시스템을 적절히 관리하고 인적자원관리 차원에서 구성원 태도를 변화시키려는 노력은 병원조직성과의 개선에 공헌할 수 있다는 점을 시사하고 있다. This study is aimed to investigate whether use of performance measurement system (PMS) and justice of PMS play a mediating role in the relationship between TQM activities of hospital and organizational performances. We defined TQM activities of hospital as input variable, diagnostic and interactive use of PMS, justice of PMS as conversion variables, and organizational performances (attitude of internal members, financial / non-financial performances) as output variables in order to test hypotheses. Hypothesis 1 is to test the relationship between level of TQM activities of hospital and organizational performances. Hypothesis 2 is to explore a mediating role of the diagnostic and interactive use of PMS in the relationship. And Hypothesis 3 is to identify casual relationship among TQM activities of hospital, the diagnostic and interactive use of PMS, the justice of PMS, and the organizational performances. The results of the study are as follows. Firstly, the TQM activities of hospital have a positive effect on the attitude of internal members and non-financial performance. However, the effect of TQM activities of hospital on financial performance are not significant. Secondly, the diagnostic use of PMS has played a mediating role in the relationship between the TQM activities of hospital and the organizational performances, but the interactive use of PMS does not play a mediating role in the relationship. Thirdly, if hospital use the diagnostic control of PMS in the process of promoting TQM activities, financial / non-financial performances have improved through changes in the attitudes of internal members. However, in case of using the interactive control of PMS, the attitude of internal members changed as the justice of PMS increased, which is linked to improvement of financial / non-financial performances. This study suggests that efforts in the process of pursuing TQM activities trying to manage properly performance measurement systems in management controls and to change employee attitude in terms of human resource management can contribute to improve the organizational performances of hospital.

        주민의식조사를 통한 주거지역 방범용 CCTV 효과성 분석

        The aim of this study was to analyse the effects of CCTV for making a crime-free safe environment in urban residential areas through comparative studies with the view of the residents on crime and fear of crime before and after crime preventive CCTV installation. In order to fulfill this study, we analysed results from the questionnaires and field surveys from Urban residential areas. According to the results from this study, the more CCTV installations occurred in residential areas, there was a reduction in areas vulnerable to crime and fear of crime. Especially, there were affirmative results regarding CCTV installations, even in areas with little to no CCTV not taking into account crime victimization rates, and there were high expectations for the effects of CCTV in public spaces such as alleys, playgrounds and community parks. These affirmative assessments implied that crime preventive CCTV installation was important for the 'diffusion of benefit' for adjoining areas and psychological reassures the residents about the risk of crime. In conclusion, the satisfaction that CCTV provided in crime prevention and reduction in fear of crime was higher than the apprehension about the violation of privacy and human rights. In result, the public desire CCTV installations. Therefore, we think that it is necessary to approach the installation of crime preventive CCTV for residential areas with a positive light.

      • 폐결핵 환자 혈청에서 수용성 Interleukin 2 수용체 및 Adenosine Deaminase 활성도에 관한 연구

        Interleukin-2 (IL-2) induces T cell proliferation in an autocrine manner and provides a means by which antigen triggered T cells can be clonally expanded in vitro. During the following activation, the activity of IL-2 is mediated by specific high affinity IL-2 binding membrane receptors which are expressed shortly after activation. In this process, a 42 KD-fragment (soluble IL-2R) is continuously cleaved off and circulates as a soluble marker of T lymphocyte activation. Elevated level of soluble IL-2R has been identified in the serum of patients with malignant autoimmune and allergic disorders, systemic parasitic infection, undergoing graft versus host disease, acute or chronic lymphocytic leukemia and HIV-infection. ADA (adenosine deaminase) completes the process of differentiation of T cell and is essential for progression of T cell maturation. Therefore level of ADA is to correlated with magnitude of T cell immune response. The fact that expression of sIL-2R and ADA activity increases in peripheral blood T-lymphocytes of patients with active pulmonary tuberculosis suggests that T cell activation might have a major role in the pathogenesis of pulmonary tuberculosis. In order to evaluate the T cell immune response in pulmonary tuberculosis, we measured the serum concentration of sIL-2R and ADA activity in 17 patients with current pulmonary tuberculosis, 10 chronic inactive pulmonary tuberculosis, 10 as normal controls. (1) Current pulmonary tuberculosis had significantly higher levels of sIL-2R (237.24±95.47)when compared with those of inactive tuberculosis (78.6±11.06). and the control (68.17±15.4) group. (2) ADA activity in current pulmonary tuberculosis was significantly increased (34.41±20.63) when compared with those of inactive tuberculosis (24.7±14.36) and control (17.65± 5.94) group. (3) There was good correlation between sIL-2R concentration and ADA activity in serum in current pulmonary tuberculosis group. (4) sIL-2R concentration and ADA activity was decreased significantly 6 months after anti-tuberculosis drug medication. In conclusion, sIL-2R concentration and ADA activity in serum in current pulmonary tuberculosis group was increased when compared with those of inactive pulmonary tuberculosis and the control group. By the way, T cell mediated immune response was enhanced in current pulmonary tuberculosis, but in inactive chronic pulmonary tuberculosis who had treated by antituberculosis drug medication, the concentration of sIL-2R and ADA activity was nearly normal.

        조혈모세포이식 환자에서 침습성 진균 감염에 대한 Micafungin의 예방 효과 및 안전성

        Background: Micafungin, a potent inhibitor of 1,3-β-D-glucan synthase, is a novel antifungal agent of the echinocandin class. In vitro study showed that micafungin was effective against Aspergillus species as well as Candida species, but clinical data on the prophylactic efficacy against invasive fungal infections (IFIs) other than candidiasis are still lacking. Materials and Methods: We identified 60 consecutive adult hematopoietic stem cell transplantation (HSCT) recipients who received at least 3 doses of micafungin during neutropenic period. Micafungin was started as an alternative in patients who were intolerant or had adverse events (AEs) to primary prophylactic antifungal agents. We retrospectively reviewed the medical records and analyzed the efficacy and safety of micafungin for prophylaxis against IFIs. Results: The patients either had autologous (n=9) or allogeneic (n=51: 1 syngeneic, 24 sibling, 26 unrelated donor) HSCT. Itraconazole oral solution (n=58) was the most frequently used first line antifungal agent for prophylaxis and was administered for median 11 days. The most frequent cause of switch to micafungin was vomiting (n=42). The duration of neutropenia and micafungin administration was median 13 and 12 days, respectively. A successful outcome was achieved in 45 (75%) patients. Empirical antifungal therapy was initiated in 13 (22%) patients. There were 2 cases (3.3%) of breakthrough fungal infections which comprised a probable invasive pulmonary aspergillosis and a possible invasive fungal sinusitis. There was no case of invasive candidiasis. A total of 53 (88%) patients experienced at least one AE regardless of causality during micafungin administration. The most frequent AEs were hypokalemia, vomiting, diarrhea, and elevated serum aspartate aminotransferase or alanine aminotransferase. Among the aforementioned AEs, only 1 case of diarrhea could be classified as a probable relation with micafungin when causality was assessed. There was no AEs that caused discontinuation of micafungin. Conclusions: Micafungin seems to be a safe and effective agent for prophylaxis of IFIs including aspergillosis as well as candidiasis in HSCT recipients. However, further large, prospective, and randomized comparative studies are warranted for aspergillosis.
