        하악골에 발생된 Low grade 골육종:증례보고

        Osteosarcoma is a primary malignant tumor arising most often in the long bone, but rare in jaw bone. Especially osteogenic sarcoma of the mandible is a rare tumor, comprising less than 0.5% of all head and neck tumors. Osteosarcoma occurs cheifly in young persons, and presenting very survival rates. Histopathologically this tumor can be classified three types, osteoblastic, fibroblastic, chondroblastic and classified from low grade to high grade by anaplasia or mitosis of the tumor cells. Sometimes, the exact diagnosis of osteosarcoma is difficult especially in low grade osteosarcoma, even if clinical or radiographical findings suggest to osteosarcoma. So suspcious to malignant bone tumor in clinical or radiological findings, biopsy should be taked from deep portion and multiple area, and sharing the patient history and radiographs with pathologist will assist in the development of the diagnosis. We report a case of low grade osteosarcoma on the mandible initially difficulty in accurate diagnosis. The patient visited our hospital for routine dental treatment but radiographic findings displayed ill-defined radiolucency with osteoid formation on the mandible. Final diagnosis was difficulty in initial biopsy but subsequent biopsy taked from deep portion presented infiltrative growth and mitosis of the tumor cell in some area with small osteoid bone formation, so we could reach final diagnosis as low grade sarcoma. We treated this patient with bloc resection of the mandible and immediate reconstruction with iliac corticocancellous block.

        三次元的 都市管理 手段으로서의 建築物 높이基準 設定方向 硏究

        The height of buildings define the skyline of cities, and in recent years, building height control have been approached from urban design aspect to bring about harmonious townscape as well as provide identity of city. Yet in Korea, the building height control has remained based on the width of the road they face. This has resulted in many problems such as skyline control, high density developments in outlying areas, land-use problems and lack of city identity. This research examines the existing height control system, the problems inherent in it and reviews building height control in advanced industrialized countries. The paper examines various urban planning as well as site specific considerations in resolving the problems in the building height control system currently in use, and introduces a new direction in building height control based on the street block.

      • Stoeckeria algicida n. gen., n. sp. (Dinophyceae) from the Coastal Waters off Southern Korea: Morphology and Small Subunit Ribosomal DNA Gene Sequence

        <P>This paper presents a new description of the morphology of the planktonic dinoflagellate Stoeckeria algicida n. gen., n. sp. and a report of the sequence of the small subunit rDNA (SS rDNA) from cultured cells. The vegetative biflagellated cell, gametes, triflagellated planozygotes, and cyst stages of this heterotrophic species were observed in cultures. The vegetative biflagellated cells are oval, with the cell length being considerably larger than the cell width. The ranges (and mean, n=60) of cell length and width of live biflagellated cells satiated with the raphidophyte Heterosigma akashiwo were 14.4-20.8 microm (16.8) and 10.0-17.4 microm (12.9), respectively, while those of biflagellated cells starved for 3 d (n=60) were 7.3-15.9 microm (11.6) and 2.7-12.2 microm (7.3), respectively. Thin plates of the vegetative biflagellated cells were arranged in a Kofoidian series of Po, cp, X, 4', 2a, 7'', 6c, 6s, 5''', 0 (p), and 2''''. When properly aligned, the sequence of the SS rDNA of the biflagellated cells of S. algicida (GenBank Accession no. AJ841809) was 3% different from that of a dinoflagellate from Shepherd's Crook and 4% different from that of Cryptoperidiniopsoid sp. brodyi, Pfiesteria spp., or Pfiesteria-like species. In a maximum-likelihood-distance phylogenetic tree generated using the SS rDNA sequences, Pfiesteria spp., Pfiesteria-like species, and a dinoflagellate from Shepherd's Crook were closest to S. algicida, but these dinoflagellates were clearly divergent with S. algicida. Based on morphological and genealogical analyses, we suggest that this is a new species in a new genus.</P>

        Polarimetric Fiber Vibration Sensor Based on Polarization-Diversified Loop Using Short Polarization-Maintaining Photonic Crystal Fiber

        <P>In this paper, we demonstrated a polarimetric fiber vibration sensor based on a polarization-diversified loop (PDL) with short polarization-maintaining photonic crystal fiber (PM-PCF) as a sensor head and investigated the dependence of its frequency response on the sensing fiber length. The fabricated PM-PCF vibration sensor is composed of a polarization beam splitter for forming the PDL, PM-PCF as a sensor head, and two wave plates such as a quarter-wave plate and a half-wave plate. A laser diode and a photodetector were also utilized for intensity-based vibration measurement. In particular, two kinds of PM-PCF segments with fiber lengths of similar to 6 and similar to 10 cm were employed to explore the effect of the sensor head length on the sensor frequency response. The frequency response was examined over 1-3000 Hz using a piezoelectric transducer that can apply single-frequency vibration to the sensor head. Every measured frequency response showed a resonance peak at each sensor head length, and cut-off frequencies were measured as similar to 1240 and similar to 794 Hz for PM-PCF lengths of similar to 6 and similar to 10 cm, respectively. That is, a higher cut-off frequency was obtained in a shorter sensor head length. Moreover, it was observed that the sensor head length dependence of the frequency response was maximized in a frequency range from 1000 to 2400 Hz and faded away beyond 2400 Hz. From the measurement results, it is concluded that the short sensor head can provide higher normalized sensitivity, i.e., better signal to noise ratio, at frequencies higher than the resonance frequency, let alone convenience of installation and in sensitiveness to external perturbations in a PDL-based PM-PCF vibration sensor. The phase shift per unit strain and the minimum detectable strain perturbation, regarded as sensitivity and resolution, were measured as similar to 0.377 mrad/mu epsilon and similar to 0.16 n epsilon/Hz(1/2) at 2000 Hz for similar to 6-cm-long PM-PCF, respectively.</P>

        Expulsion of Fibroids to the Endometrial Cavity after Magnetic Resonance Imaging-guided High Intensity Focused Ultrasound Surgery (MRgFUS) Treatment of Intramural Uterine Fibroids

        Objectives: This report seeks to introduce some cases of the patients who received magnetic resonance imaging (MRI)-guided high intensity focused ultrasound (HIFU) surgery (MRgFUS)-based intramural uterine fibroids treatment where the post-MRgFUS intramural uterine fibroids decreased in its volume and protruded towards the endometrial cavity to be expelled by hysteroscopy. Methods: Of the 157 patients who had received MRgFUS treatment in the Obstetrics and Gynecology of the Hospital from March, 2015 to February, 2016; this study examined 6 of the cases where, after high intensity focused ultrasound treatment, intramural uterine fibroids protruded towards the endometrial cavity to be removed by hysteroscopic myomectomy. The high intensity focused ultrasound utilized in the cases were Philips Achieva 1.5 Tesla MR (Philips Healthcare, Best, The Netherlands) and Sonalleve HIFU system. Results: The volume of fibroids ranged from 26.0 cm3 to 199.5 cm<sup>3</sup>, averaging 95.6 cm<sup>3</sup>. The major axis length ranged from 4.0 cm to 8.2 cm, averaging 6.3 cm. Fibroid location in all of the patients was in intramural uterine before treatment but after the high intensity focused ultrasound treatment, the fibroids were observed to protrude towards the endometrial cavity in at least Day 5 or up to Day 73 to allow hysteroscopic myomectomy. Conclusions: In some cases, after an intramural uterine fibroid is treated with MRgFUS,, fibroid volume is decreased and the fibroid protrudes towards the endometrial cavity. In this case, hysteroscopic myomectomy can be a useful solution. (J Menopausal Med 2016;22:139-145)

        Analysis of Measurement Fractional Anisotropy Value for Diffusion Tensor Images of Brain White Matter Region by Smoking

        흡연 유무의 남성을 대상으로 뇌 백질의 손상 유무를 파악 할 수 있는 확산텐서영상을 검사하여 영상을 획득 한 후 Tract-Based Spatial Statics(TBSS)방법으로 뇌 백질 부위의 신경섬유로의 비등방도 FA(fractional anisotropy)값을 측정 분석한 결과 모든 영역에서 흡연자가 비흡연자보다 비등방성 측정값이 낮게 나타났지만 FA값은 통계적으로 유의하지 않았으며 오른쪽 맥락총 부위에 대한 FA값에서만 통계적으로 유의하였다. 본 연구의 결과 값으로 추측하자면 즉 흡연이 뇌 백질의 미세구조성 변화에 크게 영향을 미치지는 않지만 맥락총 부위에는 영향을 미친다고 할 수 있다. The fractional anisotropy value of the brain white matter region was analyzed by Tract-Based Spatial Statics(TBSS) method after acquiring the diffusion tensor image to identify the presence or absence of brain white matter damage to smoking in male. As a result, smokers showed lower fractional anisotropy than non-smokers in all areas but fractional anisotropy values were not statistically significant. the fractional anisotropy value, on the other hand, was statistically significant only for the right choroid plexus. In other words smoking, although, does not significantly affect the micro structural changes of brain white matter, it affects the choroid plexus area.

        Clinical experience with single-port access laparoscopic cystectomy and myomectomy

        Objective: This study was performed to assess our clinical experience with single-port access (SPA) laparoscopic cystectomy and myomectomy and the surgical outcomes of those procedures at our institution. Methods: The authors evaluated the surgical outcomes of SPA laparoscopic cystectomy in 293 patients and SPA laparoscopic myomectomy in 246 patients. The surgical outcomes comprised operation time, the amount of blood loss during the operation, the change in hemoglobin (before and after the operation), the change in hematocrit (before and after the operation), switching to the multi-port access method, complications, transfusions, and the duration of the postoperative hospital stay. Results: The Pearson correlation coefficient and the Spearman correlation coefficient between the operation time and the amount of blood loss were 0.312 and 0.321 for SPA laparoscopic cystectomy, respectively, and 0.706 and 0.674 for SPA laparoscopic myomectomy, respectively. The drops in hemoglobin and hematocrit were $1.33{\pm}0.78g/dL$ and $4.14%{\pm}2.45%$, respectively, in SPA laparoscopic cystectomy, while the corresponding figures were $1.34{\pm}1.13g/dL$ and $4.17%{\pm}3.24%$ in SPA laparoscopic myomectomy, respectively. Conclusion: This study reported the surgical outcomes of SPA laparoscopic cystectomy and myomectomy and compared them to previously published findings on traditional laparoscopic cystectomy and myomectomy. No significant differences were found in the surgical outcomes between SPA and traditional laparoscopic cystectomy and myomectomy.

        Clinical Consideration of Treatment to Ablate Uterine Fibroids with Magnetic Resonance Imaging-guided High Intensity Focused Ultrasound (MRgFUS): Sonalleve

        Objectives: Magnetic resonance imaging (MRI)-guided high intensity focused ultrasound surgery (MRgFUS) is a newly emerging non-invasive technique for the treatment of uterine fibroids. The purpose of this study is to review the clinical impact of MRgFUS. Methods: This study examined 157 patients. The high intensity focused ultrasound (HIFU) utilized in this study was Philips Achieva 1.5 Tesla MR (Philips Healthcare, Best, the Netherlands) and Sonalleve HIFU system. The patients were followed in post-operative Month 1, Month 3, and Month 6 to investigate any change. Then, these were further classified according to the use of uterine stimulant (oxytocin) in parallel, Funaki Type of uterine fibroid, HIFU intensity, and non-perfused volume (NPV) ratio. Results: When the uterine stimulant was utilized, the HIFU intensity was measured at significantly lower levels, compared with the group not using uterine stimulant, and treatment duration was significantly. The NPV ratio was found significantly higher in the group using uterine stimulant. Concerning the correlation between Funaki Type of uterine fibroid and average sonication power, it was found that the closer to Type I, the lower the sonication power, the shorter the treatment duration, and the higher the NPV ratio significantly. Conclusions: In this study, it was found that the lower the Funaki Types of uterine fibroids, and the higher the NPV ratio immediately after the operation, the larger the uterine fibroid volume decrease and SSS change were. Also, if uterine stimulant was used in parallel in treatment, treatment duration and HIFU intensity could become shorter and lower. (J Menopausal Med 2016;22:94-107)

        Sildenafil Inhibits Advanced Glycation End ProductsinducedsFlt-1 Release Through Upregulation of Heme Oxygenase-1

        Objectives: We examined the effect of sildenafil citrate on advanced glycation end products (AGEs)-induced soluble fms-liketyrosine kinase 1 (sFlt-1) release in JEG-3 choriocarcinoma cells. Methods: Cells were incubated with control bovine serum albumin (BSA) or AGEs-BSA, and expression of sFlt-1 mRNAand protein release was determined by quantitative reverse transcription-polymerase chain reaction (qRT-PCR)and enzyme-linked immunosorbent assay (ELISA), respectively. AGEs-BSA increased sFlt-1 mRNA expression and protein release in a dose-dependent manner. Results: Sildenafil citrate suppressed sFlt-1 mRNA expression and protein release in cells treated with AGEs-BSA in a dosedependent manner. Likewise, it inhibited the increase of reactive oxygen species (ROS) production and NF-κB activity in these cells. Cobalt protoporphyrin (CoPP) and bilirubin also inhibited sFlt-1 release and ROS production in cells treated with AGEs- BSA, whereas zinc protoporphyrin IX (ZnPP IX) antagonized the effect of sildenafil citrate. In cells transfected with the heme oxygenase-1 (HO-1) siRNA, sildenafil citrate failed to inhibit the sFlt-1 release and ROS production. Conclusion: These results strongly suggest that sildenafil citrate inhibits sFlt-1 release and ROS production incells treated with AGEs-BSA through upregulation of the HO-1 expression in JEG-3 cells.
