The main powder of PL are obtained by firing at 800~1,000˚C for l~3hrs in electric furnace, and the co-activators and activators used are Cl (NaCl)and Mn (MnCl_(2),4H_(2)O) Cu (CuCl_(2),2H_(2)O). X-ray analysis of PL-powders shows the crystal structure by the diffractive image of (110), (211) and (311) phases. The PL-Cell made by mixed these main co-activator and activator are observed the photoluminescence spectrum by varying the firing time and temperature, and modifying the ratio of the activators. The Orange band spectrum of ZnS: Mn, Cl cell are shift to long wavelength (6,000Å) more then ZnS: Cu, Cl cell (5, 500Å).

Pacemaker implantation for patients with mechanical tricuspid valve is quite challengeable because lead insertion through prosthetic tricuspid valve may cause valve dysfunction or lead impingement. Also complications due to interrupt of anticoagulation should be considered. A 65 years old woman received AAI (atrium paced, atrium sensed, inhibited) pacemaker for sick sinus syndrome and mechanical mitral valve replacement for severe mitral steno-insufficiency at the same time 16 years before. She needed to undergo mechanical tricuspid valve replacement (TVR) because of severe tricuspid regurgitation despite of medical therapy. Complete atrioventricular block developed during the TVR operation and it was not recovered even after several days of temporary pacing. We decided left ventricular pacing through coronary sinus because ventricular lead could not pass mechanical tricuspid or mitral valve and also planned to continue oral anticoagulation therapy. We could find a place where high pacing output did not pace phrenic nerve with acceptable sensing, pacing threshold. The patient recovered well without any periprocedural complications. Left ventriclcular pacing lead implantation through coronary sinus without interruption of anticoagulation can be an alternative to epicardial pacing for patients with mechanical tricuspid valve.
LC : The Factors that Affect the Improvement of Liver Stiffness in Patients with Chronic Hepatitis B

Background: The degree of hepatic fibrosis is an important prognostic factor of chronic hepatitis. Although liver biopsy is a standard tool for fibrosis, it is invasive. Transient elastography (TE), a novel noninvasive and repeatable method, can assess hepatic fibrosis quantitatively by measuring liver stiffness (LS) which is correlated with the histologic fibrosis. The aim of this study was to determine factors that affect the improvement of LS in patients with chronic hepatitis B (CHB). Methods: Between April 2007 and December 2012, 160 patients with CHB who underwent TE for more than 12 months interval, were retrospectively enrolled. Among of them, 109 patients were treated by oral antiviral agents. Patients who had alanine aminotransferase (ALT) > 80 U/L at the time of the first TE were excluded. The annual change of TE was estimated [(the first TE-the second TE)×12/interval (months)]. All the patients were grouped according to the estimated annual change of the LS ≥ 0 kPa (improved) or < 0 kPa (aggravated). Results: Ninety eight patients were improved group, and 62 patients were aggravated group. There were no differences in basal clinical and laboratory data between the two groups. However, the mean LS at the first TE (12.34±11.51 vs. 7.28±4.57 kPa, P<0.001), the proportion of patients maintaining normal ALT level (<40 U/L) throughout the study period (35.0% vs. 12.5%, P=0.002), and the proportion of receiving antiviral treatment (73.5% vs. 59.7%, P=0.07) were higher in improved than in aggravated group. In multivariate analysis, higher initial LS (OR: 1.13, 95% CI: 1.05-1.22, P=0.002) and maintained normal ALT (OR: 2.789, 95% CI: 1.38-5.64, P=0.004) were the significant factors that affect the improvement of LS. Conclusions: Higher initial LS, maintained normal ALT level may be associated with improvement of fibrosis in patients with CHB.
터널 주행속도 향상을 위한 고속열차 전두부 형상 최적화

The next generation of Korean high-speed train under development will be designed for the maximum operating speed of 350km/h and maximum speed of 400km/h. This high-speed operation may cause the noise and vibration problems around tunnel exit due to the higher micro-pressure wave than present level. In this study, the nose shape optimization was conducted for the countermeasure against these problems. Axi-symmetric solver was used for numerical simulation, and response surface was used for efficiency of optimization process. Also the multi-step optimization was conducted to find out more accurate optimal shape. Through these analysis and optimization, it was found out that the optimal nose shapes for minimization of micro-pressure wave are definitely different along the nose length variation. And the mechanism of micro-pressure wave reduction was closely investigated by the analysis of generation process of compression wave in tunnel. The results are expected to be used as design guideline for performance improvement of the next generatin of Korean high-speed train.
반응면 기법과 크리깅 기법을 이용한 설계공간의 타당성 향상

본 연구에서는 근사모델을 이용하여 설계공간의 타당성을 높일 수 있는 방법을 제시하였다. 이때 설계공간을 이동시키기 위한 기준으로 Chebyshev Inequality를 사용하였다. 이를 공탄성을 고려한 항공기 익형 설계문제에 적용함으로써 타당성이 크게 향상됨을 확인하였으며 이렇게 구한 설계공간 내에서 최적화를 수행함으로써 보다 우수한 최적값도 얻을 수 있었다. 즉 설계공간 내에서 주어진 제약조건을 만족할 확률이 증가하였으며, 설계공간을 이동시킴으로써 보다 우수한 최적점이 설계공간 내에 포함되었다고 할 수 있다. 또한 이 과정에서 반응면 모델과 크리깅 모델, 두 가지 근사모델을 사용하여 정확성과 효율성, 실험점에 대한 강건성 등을 비교하였으며, 본 연구에서 설계한 문제의 경우 비교적 선형적인 특징으로 인해 반응면이 보다 우수한 결과를 보여줌을 확인하였다. In this research, a procedure to improve the feasibility of design space is proposed by an approximation model. The Chebyshev Inequality is used as the criterion of modification of design space. This procedure is applied to the aero-elastic transonic wing design problem and the feasibility of the design space is greatly improved. Also the optimization results are improved by appling this procedure. That is, the probability to satisfy all imposed constraints is increased and the better design points are included in design space after this procedure. And the use of both a second-order response surface model and the Kriging model is investigated and compared in accuracy, efficiency, and robustness as approximation models in this procedure for different sampling methods. As a result, the second-order response surface model is more appropriate for our application than the Kriging model, because it is linear enough to be fitted well by the response surface model.

Kommerell 게실은 근위부 하행대동맥의 확장이 특징인 대동맥 궁의 드문 선천성 이상으로 이상적으로 기시하는 쇄골하동맥이 동반된다. 무증상일 수 있으나 식도나 기도 등의 압박으로 인한 증상이 동반될 수 있으며, 대동맥 박리나 게실의 파열로 나타날 수 있다. 저자들은 우측 대동맥 궁에서 동반된 Kommerell 게실이 식도를 압박하여 심한 연하곤란으로 발현한 증례를 경험하였기에 보고하고자 한다. Kommerell diverticulum is a rare congenital anomaly of the aortic arch characterized by dilation at the proximal descending aorta, which gives rise to an aberrant subclavian artery. Kommerell diverticulum is usually asymptomatic, but can also be associated with symptoms due to compression of the esophagus or trachea, and can rarely be fatal due to dissection or rupture of the diverticulum. Here, we report a rare case of dysphagia caused by compression of the esophagus by Kommerell diverticulum originating from the right-sided aortic arch. (Korean J Med 2020;95:287-291)

A nose shape is strongly related with the aerodynamic performances of train. Therefore shape definition and aerodynamic performance analysis are important for train nose shape design. In this study, a new design method was suggested for train nose shape design by configuration function. To this end, the nose shape was classified by box type and each box shape is defined. After that the 3-D shape of train was defined as several mathematical functions by combination of each box shape. Also it was shown that the wind shield of driver"s seat and complex curves of surface can be expressed using superposition of functions. This methodology can be used for grid generation of numerical analysis, and applied to aerodynamic optimization design of nose shape.
