        한국인 강박장애와 Val-158-Met Catechol-O-Methyltransferase 유전자 다형성 간의 상관성 연구

        목적 COMT(Catechol-O-Methyltransferase) 효소는 도파민성과 노어아드레날린성 체계의 조절에서 매우 중요한 작용을 하며 염색제 22q11에 위치해 있다. 일부의 강박장애 환자들은 도파민 차단제에 좋은 치료 반응을 보이며 이런 현상은 COMT가 강박장애의 병인론에 역할을 하고 있을 가능성을 말해준다. Val-158-Met COMT 유전자의 다형성은 효소의 감소를 유도하는 L(저활성) 대림형질, 활성의 증가를 유발하는 H(고활성) 대립형질로 알려져 있으며 L 대립형질 또는 LL 유전형이 강박장애와 연관이 있다는 보고가 있으나, 아직까지 강박장애와 COMT 다형성 사이의 연관성에 대한 연구결과들은 일관되지 않다. 따라서 본 연구에서는 한국인 강박장애 환자들을 대상으로 Val-158-Met COMT 유전자의 다형성과의 연관성을 알아보고자 하였다. 방법 2002년 3월부터 2004년도 2월까지 약 24개월동안 서울지역 일 대학병원과 경기지역 일 대학병원 강박장애 클리닉을 방문하여 강박장애로 진단받은 124명의 환자들을 연구대상으로 하였고 정상 대조군으로는 나이와 성별을 고려하여 자원자 170명이 선별되어 포함되었다. 강박장애군과 정상 대조군의 혈액으로부터 유전자 추출 후 COMT 유전자 형벌 (genotyping)을 시행하였다. 다형성을 H/H 와 H/L, L/L 유전형으로 나눈 뒤 강박장애와 정상대조군 사이의 유전자 빈도를 카이 검정법으로 비교하여 연관성을 관찰하었다. 또한 강박장애 환자들에게 시행한 YBOCS로부터 측정한 강박장애 심각도 및 Hamilton Depression Raing Scale (HDRS) 총점과 COMT 유전자 다형성 사이의 연관성을 비교하였다. 결과 본 연구 결과 한국인 강박장애와 COMT 다형성 간에는 유의한 상관성이 관찰되지 않았다. YBOCS 심각도와 COMT 유전자형과의 유의한 연관성이 관찰되지 않았다. 그러나, 강박장애 환자군들의 COMT 유전자형 세 군 사이의 우울증상 점수에서는 유의한 차이가 관찰되었다. 결론 한국인에서 강박장애와 COMT 유전자 다형성 사이에는 연관성이 없었다. 그러나, COMT의 H 대립형질과 강박장애 군에서 공존하는 우울증상 척도가 유의한 연관성을 보인 현상은 COMT 유전자의 다형성이 강박장애의 공존 우울증상에 영향을 미칠 수 있는 가능성을 시사한다고 생각된다. Objectives : The definite cause of obsessive-compulsive disorder (OCD) is still unknown. Evidences from familial, twin and segregation studies support the role of a genetic factor. There are also growing evidence indicating that OCD has specific neurochemical and neuroanatomical basis. Derived from the effectiveness of treatment with dopamine receptor blocker in certain part of OCD patients (eg. SSRI treatment-resistant OCD), several candidate genes related to dopamine dysregulation have been hypothesized to play an important role in the development of OCD. One of them is the Catechol-O-Methyltransferase (COMT) gene. The aim of this study was to investigate the association between COMT and OCD in Korean population. Methods : 124 OCD patients and 170 normal controls participated in this study. Genomic DNA was extracted from their blood. The genotypes and allele frequencies of the COMT polymorphism between OCD group and control group were compared. We investigated the association between severity of OCD assessed by Yale-Brown obsessive compulsive symptom scale (YBOCS) and Hamilton depression rating scale (HDRS) scores and COMT polymorphism. Results : In this case-control study, we could not find any association between COMT gene polymorphism and development of OCD. In OCD group, patients with H/H genotype had significantly higher scores for the HDRS than those with H/L or L/L genotype. Conclusion : In this study, there was no difference in genotype distributions of COMT between OCD and control groups. However, H/H genotype of COMT gene polymorphism might be related to depressive symptoms in OCD patients.

      • 본태성 고혈압이 동반된 당뇨병환자의 혈중 인슐린 농도

        Multiple lines of evidence link elevated blood pressure with diabetes mellitus. Specifically, it has been proposed that resistance to insulin-stimulated glucose uptake and hyperinsulinemia may play a central role in the cause and clinical course of hypertension. In diabetes mellitus, insulin resistance and/or hyperinsulinemia is an common finding and it is interesting whether the hyperinsulinemia may play a signigicant role in hypertension with DM in the same way as in essential hypertension without diabetes mellitus. The object of this study was to compare insulin and c-peptide levels between age, sex, and obesity matched two groups(DM with DM without hypertension). Method The study group consisted of 55 male diabetes mellitus patients, aged between 45-55 (years). Patients with obesity (body mass index · 30 ㎏/m2), renal disease (proteinuria · 300 ㎎/24hr urine), and secondary hypertension were excluded. Insulin and c-peptide were measured in overnight fasting state and after oral administration of glucose(75 gm). In the fasting, venous plasma glucose levels were similar in the hypertensive and control group(132 ±7 ㎎/dl vs 135 ±8 ㎎/dl). In the fasting, venous plasma insulin levels were higher in the hypertensive than in the control group (10.9 ±5.3 μIU/ml vs 5.5 ±3.9 μIU/ml, p = 0.0001). After loading with 75 gm glucose, venous plasma insulin level seems to be higher in the hypertensive patients than in the control patients, but the difference was not signigicant statistically (27.2 ±17.5 μIU/ml vs 19.9 ±18.9 μIU/ml, p = 0.1297). The mean insulin concentration of the essential hypertensive patients with diabetes mellitus was twice that of the normotensive patients with diabetes mellitus. In control group, there was a direct relations between insulin level and c-peptide, in both fasting state (R = 0.617, p = 0.0001) and glucose-loaded state (R = 0.531, p = 0.001). But in hypertensive group, there was no relations between insulin level and c-peptide, in both fasting state (R = 0.257, p = 0.2738) and glucose-loaded state (R = 0.307, p = 0.1885).

We report here the outcome of Korean children with HLH who underwent HSCT, which was analyzed from the data of a nation-wide HLH registry. Retrospective nation-wide data recruitment for the pediatric HLH patients diagnosed between 1996 and 2008 was carried out by the Histiocytosis Working Party of the Korean Society of Hematology. Nineteen patients who received HSCT among the total of 148 enrolled children with HLH were analyzed for the transplant-related variables and events. The probability of five-yr survival after HSCT was 73.3% with a median follow-up of 57. Two months compared to 54.3% for the patients who were treated with chemoimmunotherapy only (p<I> </I>=<I> </I>0.05). The reasons for HSCT were active disease after eight wk of initial treatment (n = 9), relapsed disease (n = 5), and FHL (n = 5). Fourteen patients are currently alive without disease after HSCT, four patients died of treatment-related events (infection in two and graft failure in two) at early post-transplant period, and one patient died of relapse at one yr post transplantation. The survival of patients who were transplanted because of active disease after eight wk of initial treatment was worse compared to those patients who had inactive state at that time (60.6% vs. 100%, respectively, p<I> </I>=<I> </I>0.06). Of the four patients who received transplants using cord blood, three died of graft failure (n = 2) and relapse (n = 1). The five-yr probability of survival after HSCT according to the donor type was 85.7% for the MRDs (n = 6), 87.5% for the MUDs (n = 8), and 40% for the MMUDs (n = 5) (p<I> </I>=<I> </I>0.03). Other variables such as age, CNS involvement at the time of diagnosis, the etiology of HLH (familial or secondary), and the conditioning regimens had no influence on the five-yr OS of the HLH patients who underwent HSCT. HSCT improved the survival of the patients who had familial, relapsed, or severe and persistent SHLH in the Korean nation-wide HLH registry. Although numbers were small, these results are similar to other reports in the literature. The disease state after initial treatment, the stem cell source of the transplant, and the donor type were the important prognostic factors that affected the OS of the HLH patients who underwent HSCT.</P>

        Evaluation of preconsolidation stress by shear wave velocity

        The behaviors of saturated soils such as compressibility and permeability are distinguished by preconsolidation stress. Preconsolidation stress becomes an important design parameter in geotechnical structures. The goal of this study is to introduce a new method for the evaluation of preconsolidation stress based on the shear wave velocity at small strain, using Busan, Incheon, and Gwangyang clays in Korea. Standard consolidation tests are conducted by using an oedometer cell equipped with bender elements. The preconsolidation stresses estimated by shear wave velocity are compared with those evaluated by the Casagrande, constrained modulus, work, and logarithmic methods. The preconsolidation stresses estimated by the shear wave velocity produce very similar values to those evaluated by the Onitsuka method (one of the logarithmic methods), which yields an almost real preconsolidation stress. This study shows that the shear wave velocity method provides a reliable method for evaluating preconsolidation stress and can be used as a complementary method.

        Review : Application of the Arrhenius Equation in Geotechnical Engineering

        The reliable measurement of geotechnical properties in cold regions should account for their fluctuations with temperature. The objective of this paper is to introduce a chemical model based on the Arrhenius equation that can predict the properties of materials as their temperature changes. The model can monitor phases and reaction rates as they change with temperature. It has been already applied in the fields of geology, construction, chemistry, materials engineering, and food science. The application of the Arrhenius equation requires a reliable estimate of the activation energy. Therefore, this study also demonstrates several methods for evaluating activation energy in different contexts through summaries and reviews of previous research related to the Arrhenius equation. This paper may be of wide use in obtaining temperature-dependent parameters in geotechnical engineering.

        This study presents the application of ultrasonic reflection method to investigate soil in underwater pile. This method uses the multiple reflection waves captured at interface. Measurement system consists of pulser, filter-amplifier, oscilloscope and computer(NI-5112). Experiment is carried out by traveling transducer horizontally. Experimental result shows the discontinuity is well detected in underwater ground. Application discussed in this paper show that ultrasonic reflection method can be an very effective tool for the investigation of the discontinuity in underwater ground.

        소산 실험을 이용한 관입 장비의 교란 효과 추정

        현장 관입 실험은 프로브 관입에 따라 주변 지반을 의도적으로 파괴시켜 지반특성을 파악할 수 있다. 본 논문의 목적은 현장 점토지반에 서로 다른 크기의 프로브 관입으로 인한 간극 수압 소산 특성을 이용하여 파괴에 따른 교란 효과를 분석 하는 것이다. 현장 실험을 수행하기 위해 콘관입 시험(CPT), 딜라토미터 시험(DMT) 그리고 현장 속도 프로브 장비(FVP: Field Velocity Probe)가 사용되었다. 샌드 매트(sand mat), PBD 그리고 사석을 이용한 지반 개량 중인 부산 북컨테이너 현장에서 실험이 진행 되었으며, 샌드 매트(sand mat)공법 이후 실내 실험을 수행하기 위해 시료가 채취 되었다, 실내에서는 물성치 실험 및 압밀 실험을 수행하여 지반 특성과 수평압밀계수 값을 도출하였다. 현장 실험은 지반 개량 후 압밀도 90% 시점에서 사석 제거 후 케이싱을 통해 CPT, DMT 그리고 FVP 순서대로 실험이 수행 되었다. 관업에 따른 영향 범위를 최소화시키기 위해 각각의 실험은 3m 간격을 유지 하였으며, 매 심도 24m에서 관업을 멈추고 시간에 따른 간극 수압 변화 양상을 측정하였다. 실내 압밀 실험을 통해 수평 압밀 계수(C<SUB>h</SUB>)를 산정하였으며, 현장 실험을 통해 각 프로브에 따른 t?? 값을 계산 하였다. t??값을 이용하여 산정된 등가 유효 반경은 FVP가 가장 작게 그리고 DMT가 가장 크게 산정되었다. The penetration of the probe produces the excess pore pressure due to the disturbance. The objective of this study is to evaluate the disturbance zone by using the dissipation of the excess pore water pressure, which was generated due to the penetration of the penetrometer with different size. The CPT, DMT and FVP (Field Velocity Probe) are adopted for in-situ tests. The tests are carried out in the construction site of north container pier of Busan new port, Korea where is accelerating the consolidation settlement using plastic board drains (PBD) and surcharges by crushed gravels. The coefficient of consolidation (C<SUB>h</SUB>) and soil properties are deduced by the laboratory test. The in-site tests are performed after the predrilling the surcharge zone at the point of 90% degree of consolidation. To minimize the penetration effect, the horizontal distance between penetration tests is 3m, the change of the pore pressure is monitored at the fixed depth of 24m. The coefficient of consolidation (C<SUB>h</SUB>) and the t??s are calculated based on the laboratory test and the in-situ data, respectively. The equvalent radi based on the t?? shows that the FVP and the DMT produce the smallest and the greatest equivalent radi, respectively.
