OBJECTIVES The purpose of present study was to analyze the acute effects of exergame on glucose control after glucose ingestion in individuals with pre- and type 2 diabetes. METHODS We enrolled 60 adults with pre-diabetes (Pre-DM, 58.1±9.2 years, n=20) and type 2 diabetes (T2DM, 61.7±9.9 years, n=40). We measured fingertip capillary blood glucose level at 10 min before, 30 min after, 60 min after and 120 min after the 75-g oral glucose tolerance test (OGTT). For the non-exergame control days, participants pursued normal daily activities but refrained from unusual strenuous physical activity. On the exercise days, participants conducted on exergame at 30 min after 75-g OGTT for 30 min. RESULTS Acute exergame reduced the glucose level 60 min, ΔBG30-60 and ΔBG60-120 after glucose ingestion compared to the control trials (p<0.01, p<0.01 and p<0.001 respectively). The cumulative glucose total area under the curve was lower with exergame than under the control condition (p<0.05). CONCLUSION Acute exergame after glucose ingestion can improve hyperglycemia and glycemic excursion.
S-538 The role of local IL6/JAK2/STAT3 signaling in high glucose-induced podocyte hypertrophy

Background: IL6 is an important regulator of cellular hypertrophy through gp130/JAK2/STAT3. We tested the hypothesis that IL6 and its downstream gp130/JAK2/STAT3 signaling pathway were participated in high glucose (HG)-induced podocyte hypertrophy. Methods: Levels of IL6 in media and lysates of podocytes were measured by ELISA. WB were performed to determine protein expression levels of gp130/JAK2/STAT3 among podocytes cultured with normal glucose (NG), NG+mannitol (MN), NG+recombinant IL6 (rIL6), HG, and HG+IL6 neutralizing antibody (IL6NAb). Immunoprecipitation assay (IP) were examined to determine whether gp130 interact with JAK2 in response to HG or IL6. Podocyte hypertrophy was verified using protein/cell counts and flow cytometry. Results IL6 levels were significantly increased in the media and lysates of podocytes cultured in HG compared with the NG groups. Nuclear phospho-STAT3/STAT3 ratio was increased by HG and NG+IL6 treatment and attenuated by addition of IL6NAb to HG media, indicating that nuclear STAT3 activated following JAK2 and cytosolic STAT3 in response to IL6 secreted by HG-stimulated podocytes. IP showed increased phospho-JAK2 recruitment to gp130 in the HG and NG+IL6 groups, and IL6NAb in the HG group significantly abrogated these increases. Podocyte hypertrophy was significantly increased in HG and NG+IL6 conditions compared with NG. This effect was diminished by the addition of IL6NAb to the HG group.Conclusions: IL6 might play a prominent role in local activation of JAK2/STAT3 in podocyte hypertrophy under HG conditions.

A case of reninoma with variant angina

Reninoma is a tumor of the renal juxtaglomerular cell apparatus that causeshypertension and hypokalemia because of hypersecretion of renin. We present acase of a 29-year-old female patient with reninoma and concomitant variant angina. The patient had uncontrolled hypertension and elevated plasma renin activity andaldosterone levels. Imaging studies revealed a mass in the left kidney, which wasfurther confirmed as a renin-producing lesion via selective venous catheterization. During the evaluation, the patient had acute-onset chest pain that was diagnosed asvariant angina after a provocation test. After partial nephrectomy, the plasma reninactivity and plasma aldosterone levels decreased and blood pressure normalized. We report a case of reninoma with variant angina.
G Protein β3 Subunit Polymorphism and Long-Term Prognosis of Functional Dyspepsia

Background/Aims: A link between G protein β3 (GNB3) polymorphism and functional dyspepsia (FD) has been suggested. The aim of this study was to determine the role of GNB3 polymorphism in the long-term prognosis of FD in Koreans. Methods: FD patients and normal healthy controls were recruited from patients who visited our center between December 2006 and June 2007. All of the subjects completed Rome III questionnaires before undergoing upper gastrointestinal endoscopy and colonoscopy. Genomic DNA was extracted for GNB3 genotyping. After 5 years, the subjects were reevaluated using the same questionnaires. Results: GNB3 825T carrier status was significantly related to FD in Koreans (p=0.04). After 5 years, 61.0% of the initial FD patients and 12.2% of the initial normal subjects were diagnosed with FD (odds ratio [OR], 11.7; 95% confidence interval [CI], 4.3 to 31.1; p<0.001). Regardless of the GNB3 genotype (p=0.798), female sex was strongly correlated with FD after 5 years (OR, 3.3; 95% CI, 1.2 to 9.1; p=0.017). Conclusions: The T allele of GNB3 is linked to FD in Koreans but does not predict long-term prognosis. Female sex is related to a higher prevalence of FD after 5 years.
Factors Influencing Glucose Response to Exercise Following Oral Glucose Load

PURPOSE: Exercise can reduce postprandial hyperglycemia by increasing contraction-mediated glucose uptake in skeletal muscles. However, postprandial exercise does not always have similar effects in all patients. This study aimed to analyze the factors influencing glucose response to exercise following oral glucose load. METHODS: We enrolled 169 adults with impaired glucose tolerance (IGT, n=64) and type 2 diabetes (T2D, n=105). We measured fingertip capillary blood glucose level at 30-minute intervals over 2-hour after the 75-g oral glucose ingestion. In addition, a single bout of moderate-intensity aerobic exercise was conducted at 30 minutes after 75-g oral glucose ingestion. We also measured glycated hemoglobin (HbA1c), insulin, insulin resistance (HOMA-IR), β-cell function (HOMA-β), physical activity level questionnaire and anthropometrics. RESULTS: There was a significant difference in glucose concentration between the IGT and T2D groups (p<.001). Additionally, we found that patients with short diabetes duration (≤1 year) had significantly more decrease in glucose concentration than long diabetes duration (≥10 years) (p<.001). A regression analysis showed that waist circumference, visceral fat area, fasting glucose, HbA1c, and HOMA-IR affected glucose response to exercise following oral glucose load in patients with T2D. CONCLUSIONS: Higher volumes of exercise following an oral glucose load may be necessary for individuals with Type 2 Diabetes (T2D) who have a longer disease duration, larger waist circumference, increased visceral fat area, higher HbA1c levels, and elevated HOMA-IR values in order to prevent hyperglycemia.
