Gastrointestinal stromal tumors (GISTs) are common mesenchymal tumors that arise in the wall of the gastrointestinal tract. We report a case of obscure gastrointestinal bleeding due to a GIST of the jejunum successfully documented by videocapsule endoscopy (VCE) and single-balloon enteroscopy (SBE). A 36-year-old man with hematochezia was referred for further evaluation of no evidence of bleeding focus on esophagogastroduodenoscopy and colonoscopy. A VCE showed a suspicious ulcerative hyperemic mass that located in about 1 hour apart from duodenal second portion. SBE revealed a nonbleeding 4×2 cm mass with an ulcer at the proximal jejunum. The patient underwent laparoscopic resection without complication. Histological examination revealed a well circumscribed, dumbbell-shaped firm mass comprised of spindle cells. Immunohistochemical staining for CD 117 was diffusely positive, whereas staining for S-100, CD 34 and MIB-1 was all negative. It was confirmed to be a low-grade GIST at the proximal jejunum.

갑상선기능저하증 이환견에 대한 치료 증례

A 10-year-old, male, poodle dog was referred to the Veterinary Medical Teaching Hospital of Chungnam National University because of continuous cough and cardiomegaly. Weight gain, lethargy, unwillingness to exercise and decreased activity was revealed on history. Obesty and alopecia was also observed on physical examination. Hypercholesterolemia and increased ALP was revealed on blood exam. So, evaluation of thyroid gland function was performed and cTSH concentration was increased. tT4 and fT4 concentrations was decreased. Results of a history, physical examination, blood exam, and thyroid gland fuction test support a diagnosis of hypothyroidism. Initial therapy with sodium levothyroxine was indicated. On 32 days after treatment of hypothyroidism, clinical signs was relieved and cTSH concentration was decreased, tT4 and fT4 concenctrations was increased.

식도암과 위암이 동시에 발생한 원발성 중복암 1예

Double primary cancers are two independently developed cancers in an individual. There have been some reports on double primary cancer since Billroth reported it for the first time in 1879. Double primary cancer of the stomach and esophagus has been revealed a very low incidence worldwide. The incidence of an esophageal cancer with another primary cancer is reported to be 9.5∼27%, but double primary cancers in the esophagus and stomach have been rarely reported to our knowledge. In this study, we present here a case of double primary esophageal and stomach cancer in a 66-year-old man because of progressive dysphagia.

Sulfadimethoxine 투여에 의해 유발된 갑상선 증식성 병변에 나타난 피막 섬유화의 병리학적 특성

Sulfadimethoxine (SM) is one of the anti-thyroid (goitrogenic) agents and has a thyroid tumor promoting effects. In this study, we analyzed the pathology of SM induced thyroid proliferative lesions to see if these lesions showed any distinctive features. Five-week-old male F344 rats were given a single subcutaneous injection of N-bis(2-hydroxypropyl)nitrosamine (DHPN: 2800㎎/㎏, body weight). Starting one week thereafter, SM (1000 ppm in drinking water) was administered for 12 weeks. All animals were autopsied and histopathological observation of the thyroids was perfomed. Thyroid follicular proliferative lesions were induced in all rats treated with SM. In addition, the formation of thick perithyroidal fibrous capsule and pleomorphic follicles within the capsule were observed. These pleomorphic follicles showed invasive figure in some area, especially around follicular adenomas and adenocarcinomas. This confuses adenoma with adenocarcinoma. However, pleomorphic follicles were also observed in the area without adenomas and adenocarcinomas. Therefore, we suggest that it should be careful to diagnose adenoma and adenocarcinoma in the presence of the invasive lesions in surrounding fibrous tissue, especially SM induced thyroid lesion.

교통사고에 의해 발생한 뇌내 좌상 1례

A two years old intact female Minature Schnauzer was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with an intermittent seizure by traffic accident. No specific finding was found by radiography and computer tomography (CT). Hematologic and blood chemical examination revealed that PCV (62.5%), BUN (72.6 mg/dl), CK (1769 U/l), ALT (169 U/l) and ALP (353 U/l) were increased. This dog was recovered by medical treatment.

DENAR교합기 사용시 INTEROCCLUSAL RECORD의 정확도에 관한 연구

This study was performed to investigate the accuracy and availability of the interocclusal recording materials for Denar articulator and compare with the accuracy of the Pantronics. 15dental students in Wonkwang University were paticipated in this study and interocclusal recording materials used in this study were Wax(Modelling wax, Dentrey, England), Corpl wax(Surgident, Columbus Dental, U.S.A.), Impression compound(Pericompound, G-C, Japan)and Impression plaster(Xanthano, Bayer, Germany). The immediate side shift and protrusive condylar guidance were measured from Denar articulator(MarkⅡ,Denar Corp.) and Pantronic(Denar Corp.) The obtained results were as follows: 1. The mean values of left and right immediate side shift were some higher with Impression plaster and Wax, but there were no statistical difference. 2. The mean value of protrusive condylar guidance was higher with Impression compound, but there was no statistical difference. 3. All interocclusal recording materials used in this study were available for interocclusal record.

외식업 종사원과 고객의 관계마케팅에 관한 연구

The objectives of this study are to investigate the factors affecting the relationship marketing activity between employee and customer in food service industry, and the factors to be influenced by the relationship marketing activity. The results offer the suggestion point as follows: First, the customer-oriented activity, friendliness and professionalism of employee will affect the meeting between employee and customer, the relationship between employee and customer. And so the restaurant should pay attention to the feature of employee and should provide a training program which can increase the employee`s ability. Second, the restaurant allows for the feature of customer and should inform the detailed information about the restaurant to the customer when he visits the restaurant. Third, it is important that the interaction factor of the relationship marketing in the relationship-oriented service marketing should be the relationship-oriented activity through the contact frequency that interaction and communication between employee and customer which allows for the quality of conversation, the two-way communication and the contact intensity. Therefore, to make repurchase and word-of-mouth of customer, it is very considerable that needs the relationship marketing strategy based on interaction between employee and customer.

말티즈견에서 발생한 승모판폐쇄부전 1례

A 10 years old, intact male, Maltese dog with clinical signs of labored respiration, exercise intolerance and increasing of coughing was referred to the Veterinary Medical Teaching Hospital of Chungnam National University. Physical examination revealed tachycardia, panting and mild cyanosis. This case was diagnosed as mitral valve insufficiency by clinical signs, ascultation, blood chemistry, radiography and electrocardiography. This dog was treated with ACE inhibitor (enalapril), diuretics (furosemide) and bronchodilator (aminophylline) for 14 days. After treatment, clnical signs were disappeared and also this dog was needed to be continuous monitoring and sodium restricted diet.

재발한 지주막하 포도낭미충증의 Albendazole 치험 1예

뇌실이나 뇌기조에 발생하는 포도낭미충증은 매우 드물며 유구조충 유충의 머리부분이 변성되면서 발생하는 낭성 막의 비정상적인 성장을 특징으로 한다. 이러한 유형의 병변은 지름술 후에도 진행적인 경과를 보이며 아직까지 항기생충제를 이용한 치료 사례들이 적고, 따라서 가장 적절한 치료 기간도 알려져 있지 않다. 본 증례 보고는 재발한 실비우스 틈새의 포도낭미충증 환자에서 4주간의 albendazole과 스테로이드로 효과적으로 치료하였던 예이다. 환자는 praziquantel과 뇌복막지름술로 치료를 받고도 포도상의 뇌실질외형의 병변이 악화되어, 뇌척수액 투과가 좋은 것으로 알려진 albendazole로 치료제를 바꾼 뒤 병변의 수와 크기가 감소였으며 이후 2년 간의 추척기간동안 다른 발작 없이 잘 지내고 있다. 지주막하의 포도낭미충증은 albendazole과 corticosteroid 치료에 잘 반응하는 것으로 보인다. The so-called racemose cysticercosis, a rare variety of neurocysticercosis occurring in ventricles or basal cisterns, is characterized by abnormal growth of cystic membranes with degeneration of Taenia solium heads (scolex). Although lesions of this type are known to follow a progressive course even after ventricular shunting, there are limitations of case series treated with antiparasitic drugs, and the optimal duration of the treatment is not yet known. We report a case of relapsed racemose cysticercosis in the Sylvian fissure, who has been successfully treated with albendazole and adjunct corticosteroid for 4 weeks. The patient had been previously treated with praziquantel and ventricul-operitoneal shunt, and maintained on the anticonvulsant drug for one year, but returned to the hospital due to seizure recurrence. The patient has been well in seizure-free state for the follow-up 2 years after albendazole therapy. The subarachnoid racemose cysticercosis seems to respond well to treatment of corticosteroid along with prolonged albendazole.

서울의 PPNG 발생 빈도(1991-1992)

The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.
