        만성정신질환자의 계속입원치료 심사에 관한 연구

        연구목적: 정신보건법에 근거하여 매월 정기적으로 개최된 충청북도 정신보건심판위원회를 통하여 정신질환자의 계속입원치료 청구 심사의 현황과 문제점을 파악하고 개선사항을 지적함으로써 정신질환자의 효율적인 관리를 유도하고 인권남용의 여지를 차단하고자 한다. 방 법: 충청북도 내의 정신의료기관에서 계속입원치료 심사청구서 및 진단서, 보호의무자의 동의서 등을 제출한 정신질환자들을 대상으로 계속입원치료 청구에 대한 승인 여부를 매달 정기적으로 심사하였다. 1년 6개월여동안 심사한 자료를 정리하여 진단 및 보호의무자, 의료보장별 특성과 불승인률 등을 분석하였다. 결 과: 충청북도 심판위원회에 계속입원치료를 청구한 정신질환자는 21차례의 위원회 동안 총 7,981명이었으며, 진단별로 정신분열병은 80.9%, 알콜중독은 8.1%, 치매 등 기타는 11.0%이었다. 또한 보호의무자는 시장 또는 군수가 29.8%, 부모가 26.9%, 형제가 26.1%, 부부가 6.3%, 자녀가 5.9%, 기타가 5.0%로 나타났으며, 의료보장은 의료보호가 73.0%, 의료보험이 27.0%이었다. 이들 중에서 196명이 계속입원치료를 불승인받아서 전체 불승인률은 2.46%이었으며 정신분열병과 알콜중독 환자의 진단별 불승인률은 각각 0.73%와 17.6%이었다. 결 론: 정신과 병상의 대부분이 장기적으로 입원하는 만성 정신질환자들로 구성되어 있어서 이들에 대한 별도의 대책이 시급하다. 또한 계속입원치료가 불승인되어 퇴원한 환자들의 재활 및 사회적응을 위한 사회적 지지 체계가 부족하며, 특히 환자의 인권보호를 위한 구체적인 기준 등이 필요하다. Objectives: This study was to develop effective managements and to avoid the abuse of human rights in mentally-ill patients. The Mental Health Judgement Board of Chungchongbuk-do province has been held monthly since August, 1997 according to the Mental Health Act. In this study, the procedures and the problems of judging continuing hospitalization of chronic mentally-ill patients were reviewed. Methods: The mentally-ill patients who submitted the request for continuing hospitalization with the certificate of charged doctor and the agreement of caregiver were reviewed by the Mental Health Judgement Board of Chungchongbuk-do province. The analysis of the diagnoses, caregivers, medical care systems, and rejection rate for the patients were done. Results: Total mumber of mentally-ill patients who requested for continuing hospitalization were 7,981 from twenty-one monthly meetings. The diagnostic distributions were 80.9% for schizophrenic patients, 8.1% for alcoholics and 11.0% for others including dementia. As for the caretakers, the rate of majors was 29.8%, parents 26.9%, sibling 26.1%, spouse 6.3%, offspring 5.9%, and others 5.0%. And 73.0% of the patients were on Medicaid and 27.0% were insured. The patients who got the rejection for continuing hospitalization were 196 at the rate of 2.46%. And the rejection rates of schIzophrenic and alcoholic patients were 0.73% and 17.6%, respectively. Conclusions: New policies for decreasing longterm hospitalization of chronic mentally-ill patients are required. And the social support systems for psychiatric rehabilitation and readjustment are presently insufficient for the already discharged mentally-ill. Moreover, the practical guidelines for human rights of patients remains to be suggested.

      • 경도관 동맥색전술을 이용한 비정상 자궁출혈의 치료

        Objective : Our purpose of this study is to evaluate the effectiveness of the transcatheter arterial embolization for the management of abnormal uterine bleeding. Materials and Methods : 14 patients with massive or recurrent uterine bleeding underwent percutaneous transcatheter arterial embolization between February 2003 and September 2004. We reviewed 14 cases of uterine artery or internal iliac artery embolization using gelfoam or PVA particles. Results : Good management of uterine bleeding was achieved in 13 of 14(93%) cases. The cause of abnormal uterine bleeding was myoma(5 case), post D & E bleeding(3), acquired vascular malformation(3), pseudoaneurysm(1), adenomyosis(1), and endometrial hyperplasia(1). Conclusion : Transcatheter artery embolization is an effective and life-saving procedure in massive or recurrent uterine bleeding. Early diagnosis and prompt transcatheter arterial embolization is a useful mangement of uterine bleeding.

        주의력결핍 과잉행동장애 환아에서 뉴로피드백 치료 효과

        Objectives:Considering the significant limitations of current available interventions on attention-deficit/hyperactivity disorder(ADHD), it has become a consensus among clinicians that new treatment options for ADHD are needed. In recent years, Neurofeedback(NF) has been suggested as a promising possibility. This article aims to review studies that have existing evidence and to inform clinicians on the use of NF as an alternative treatment for ADHD. Methods:Website Pubmed was searched in order to get studies that NF used for treatment of ADHD. We had select studies designed randomized controlled trials, peer reviewed. Results:10 studies and 1 consult study reviewed. There are no positive results except 2 studies reported statistically significant improvement of neurocognitive function. All other studies either did not directly test or did not demonstrate significant differences between θ/β training compared to wait list control, placebo control, or active treatment control. The findings of the studies reviewed do not support NF training as a first-line therapy. Conclusion:ADHD patients have treated with NF in many studies reported results statistically not significant improvement of neurocognitive function. But It’s therapeutic value can not be entirely refuse compared to different empirical treatments. In future, more research is needed on issues such as diversified NF according to ADHD subtypes, patient characteristics, and other non-specific treatment effects.

        조선용 C-Mn 강의 제조방법에 따른 미생물 부식 특성 연구

        The MIC(Microbiologically Influenced Corrosion) of lower deck steel plates in double hull VLCC(Very Large Crude Oil Carrier) cargo tanks has been focused because of severe localized corrosion. Recently, ship companies have reported that TMCP(Thermomechanical Control Process) steel plates have been showed more severe localized corrosion than conventional rolled steels. In order to elucidate the MIC resistance of TMCP steels by comparison with conventional rolled steels and normalized steels in environments of double hull VLCC cargo oil tanks, various measurements and corrosion tests were performed such as measurements of polarization curves, immersion tests in bacteria culture medium. All results revealed that three types of steels have almost the same corrosion resistance in bacteria culture medium. Three kinds of steel exhibit almost the same polarization behavior and the corrosion rate. The movement of the open-circuit potential first towards more negative values and later to more positive values is a phenomenon common to all kinds of specimens. This phenomenon resulted from an initial stimulation of the anodes by sulphide ions produced by the bacteria from the reduction of sulphate ions in the medium, followed by the formation of an insoluble partly protective film of ferrous sulphide on the surface of the electrode. The effect of SRB (Sulphate Reducing Bacteria) is clearly showed in the cathodic polarization curve. When the SRB is in a condition of rapid growth, there is a strong cathodic depolarization. However, as the sulphate is depleted and reaction products accumulate, the activity of the bacteria declines and the cathodic polarization curve returns the same form as in the inoculated culture.

        합금원소 첨가가 구조용 강의 해변 내후성에 미치는 영향 연구 (Ⅰ) : Ca, Si의 첨가 효과 Ca and Si Effects

        The objective of this study is to understand the effect of Si, on the corrosion property of the Ca-modified weathering steel and to determine the proper range of such alloying elements to maximize the effect of Ca on the corrosion resistance of the steel. The pH of a thin water film covering the steel was measured to evaluate the effect Ca addition on pH value. The electrochemical impedance spectroscopy (EIS) was employed to evaluate the corrosion resistance of the Ca-modified weathering steel. The characteristics of rust formed on the weathering steels having different alloy chemistry were examined. Analysis on the inclusion morphology and composition showed that the Ca-containing inclusions were mainly composed of Ca-Al-Mn-O-S,and Si was not founded in them except the specimen containing 0.6%Si. In distilled water, the Ca-modified specimens with or without Si showed higher pH values as Ca content was increased. The effect of Ca addition was not greatly affected by the addition of Si when the steel contained a proper Ca content of 50-70 ppm. In the impedance test, the specimens containing higher amounts of Ca showed higher polarization resistance, meaning better corrosion resistance. The analysis on the rust layers formed on the Ca-modified steel surface showed that the dense inner layer was mainly composed of a-FeOOH and y-FeOOH, whereas the loose outer layer was composed of ν-FeOOH and β-FeOOH. Fe₃O_(4) was distributed randomly throughout the rust layer. The specimens containing a high amount of Ca showed a dense and compact morphology, and the specimens containing Si also showed a dense and compact one. The result of Mossbauer analysis showed that more α-FeOOH was formed as Ca content was increased. (Received May 24, 2004)

        합금원소 첨가가 구조용 강의 해변 내후성에 미치는 영향 연구(Ⅱ) : Ni, W, Mo의 첨가 효과 Ni, W and Mo Effects

        The objective of this study is to understand the effect of W and Mo on the corrosion property of the Ca-modified weathering steel and to determine the optimum chemical composition to maximize the corrosion resistance at seaside corrosion environment. The effects of various alloying elements on the weathering steel were evaluated through a series of electrochemical test and analysis of the corrosion product formed on the steel surface. The polarization resistance of Ca-modified weathering steel can be increased by the addition of Ni, W and Mo, thus the corrosion resistance of Ca^(-)modified weathering steel containing Ni, Mo and W is increased at the initial stage of corrosion in a solution with high NaCl concentration. From the measurement on the permeability of Cl^(-) ions, the rust membranes formed from weathering steel of thin plate shape showed the anion-selectivity and the addition of various alloying elements (Ni, W and Mo) contributed to lower the permeability of Cl^(-) ions toward rust membranes. Particularly, Mo and W are more effective than Ni to suppress the permeation of Cl^(-) ions. It has an industrial significance for alloy design of the weathering steel since design of a lower cost weathering steel with higher corrosion resistance can be possible with addition of Mo or W instead of increasing the high cost Ni content to 2% or even higher.         Effect of Scytosiphon lomentaria Ethanol Extracts on Myostatin Activity and Zebrafish Obesity Induced by High Feeding

        생활습관 개선을 통한 근육량향상은 대사 증후군의 위험을 줄이는 것으로 보고되었다. 본 연구에서는 갈조류인 고리매(Scytosiphon lomentaria) 에탄올 추출물이 골격근 성장억제조절 단백질인 마이오스타틴 신호전달을 억제하는지와 고 급식으로 유도된 비만 제브라피시의 대사 항상성에 대한 효과를 확인하였다. 고리매 에탄올 추출물(10 ㎍/ml)은 pGL3- (CAGA) 12-루시퍼라제 분석에서 마이오스타틴(1 nM/ml) 신호를 완전히 차단하였다. 또한 웨스턴 블롯 분석에서 마이오스타틴 신호를 차단하여 Smad2 인산화가 억제되는 것을 확인하였다. 제브라피쉬의 치어에 대한 연구는 고 급식 대조군 그룹의 체내 포도당 농도는 정상 급식 대조군 그룹보다 유의하게 높았지만, 12.5 ㎍의 고리매 에탄올을 처리한 고 급식 그룹과 18.75 ug의 고리매 에탄올로 처리한 고 급식 그룹의 체내 포도당 수준은 정상 급식 대조군 그룹과 유사하였다. 고 급식 그룹의 GLUT2 유전자의 mRNA 발현 수준은 정상 급식 대조군 그룹에 비해 현저히 낮았다. 하지만, 고리매 에탄올 추출물을 처리한 실험군 그룹의 GLUT2 유전자의 발현은 정상 급식 대조군 그룹의 GLUT2 유전자의 발현과 거의 유사하게 나타났다. 그러므로 고리매 에탄올 추출물은 GLUT2유전자의 발현 조절을 통하여 체내 포도당 조절이 가능함을 보여준다. 본 연구의 결과는 고리매 에탄올 추출물이 대사 증후군 치료에 도움을 주는 소재 및 마이오스타틴 억제제로서의 가능성을 시사합니다. Muscle mass improvement through lifestyle modification has been shown to reduce the risk of metabolic syndrome. This study examined the capacity of ethanol extracts of Scytosiphon lomentaria (SLE) to suppress the bioactivity of myostatin, a potent negative regulator of skeletal muscle mass, as well as the effect of SLE treatment on metabolic homeostasis in obese zebrafish induced by high feeding. A total of 10 ㎍/ml SLE completely blocked myostatin (1 nM/ml) signaling in the pGL3-(CAGA)12 luciferase assay and suppressed myostatin-induced Smad2 phosphorylation in the Western blot analysis. In the zebrafish larvae analysis, the whole body glucose concentration of the high feeding control (HFC) group was significantly higher than that of the normal feeding control (NFC) group. However, the glucose levels of the high feeding group treated with 12.5 ㎍ SLE and of the high feeding group treated with 18.75 ug SLE were similar to those of the NFC group. The mRNA expression level of the GLUT2 gene of the HFC group was significantly lower than that of the NFC group. SLE treatment restored the expression of the GLUT2 gene to a level that was close to that of the NFC group, indicating that SLE is capable of regulating glucose levels in zebrafish larvae. The current results highlight the potential of SLE as a natural MSTN inhibitor and supplement that can be used to facilitate the treatment of metabolic syndrome.
