        Reproductive outcomes of retransferring retained embryos in blastocyst transfer cycles

        Objective: To determine the incidence of embryo retention (ER) in the transfer catheter following embryo transfer (ET) in blastocyst transfer and investigate whether retransferring retained embryos has an impact on reproductive outcomes in patients undergoing in vitro fertilization-ET. Methods: We retrospectively analyzed the records of 1,131 blastocyst transfers, which comprised 223 single blastocyst transfer (SBT) and 908 double blastocyst transfer (DBT) cycles. Each SBT and DBT group was classified depending on whether ET was performed without retained embryos in the catheter during the first attempt (without-ER group) or whether any retained embryos were found following ET (ER group) for the purpose of comparing reproductive outcomes in a homogenous population. Results: The overall incidence of finding retained embryos was 2.8% (32/1,131). There were no retained embryos in SBT cycles. In DBT cycles, implantation rates (30.0% vs. 26.6%), positive ${\beta}-hCG$ rates (57.2% vs. 56.2%), clinical pregnancy rates (45.3% vs. 46.9%), and live birth rates (38.9% vs. 43.8%) were not significantly different between the without-ER and ER groups. There were no significant differences in the mean birth weight (g) $2,928.4{\pm}631.8$ vs. $2,948.7{\pm}497.8$ and the mean gestational age at birth ($269.3{\pm}17.2days$ vs. $264.2{\pm}25.7days$). A total of nine cases of congenital birth defects were found in this study population. Eight were observed in the without-ER group and one in the ER group. Conclusion: Our results suggest that retransfer of retained embryos does not have any adverse impact on reproductive outcomes in blastocyst transfer cycles. Furthermore, our results support finding that SBT might be advantageous for decreasing the incidence of retained embryos in catheters.

        Objective: To evaluate whether increased peripheral blood inflammatory immune cell can induce decrease of uterine blood flow in patients with unexplained recurrent spontaneous abortion (RSA). Methods: This study designed prospectively 33 pregnant women between 5 to 7 gestational weeks with a history of unexplained RSA included in this study. 47 normal pregnant women between 5 to 7 gestational weeks without history of infertility and/or RSA included as a control. Peripheral blood natural killer (pbNK) (CD3-/56+) fractions among peripheral blood monocyte (PBMC) were checked by flow cytometry. Uterine color-pulsed Doppler trans-vaginal ultrasound for evaluation of uterine radial artery RI was checked. Uterine radial artery resistance index (RI) compared between study and control group. After then, uterine radial artery RI was compared between high pbNK cell fraction above 12.1% among PBMC and normal pbNK cells fraction below 12.1%. Correlation between pbNK cell fraction to uterine radial artery RI was also evaluated. Results: Uterine radial artery RI in early pregnancy was significantly higher in patients with RSA than that of normal control (0.60 ± 0.14 vs. 0.54 ± 0.12, P = 0.039). Especially, the mean value of uterine radial artery RI in RSA patients with elevated pbNK cells was significantly increased than that of normal control (0.62 ± 0.13 vs. 0.54 ± 0.12, P =0.029). Otherwise, pbNK cell fractions among PBMC displayed strong positive correlation to uterine radial artery RI (Pearson`s correlation coefficient P=0.001, r = 0.667). Conclusion: Increased pbNK cells can evoke decreased uterine blood flow by their pro-inflammatory action on micro vascular structure such as uterine radial artery. This can be a one causative mechanism of inducing spontaneous abortion by increased NK cells. But, larger scaled study is needed for clarify our results.

        The risk factors for herpes zoster in bortezomib treatment in patients with multiple myeloma

        <P><B>Background</B></P><P>Bortezomib has significant activity in treating multiple myeloma (MM). The risk of herpes zoster (HZ) has been reported to increase significantly with bortezomib treatment, but the predisposing factors for HZ are not clear. This study is a retrospective analysis of the relevant risk factors for HZ in Korean MM patients treated with bortezomib.</P><P><B>Methods</B></P><P>Sixty-six patients with refractory or relapsed MM who underwent chemotherapy with bortezomib were included in the study. Prophylactic antiviral drugs were not used for treatment. The following parameters were reviewed: age, gender, stage and type of MM, extent of previous treatment, history of HZ, duration from the time of diagnosis to the time of bortezomib treatment initiation, and absolute lymphocyte counts (ALC) at the time of bortezomib treatment initiation.</P><P><B>Results</B></P><P>The incidence of HZ was 16.7%. There were no intergroup differences between the HZ-positive and the HZ-negative groups with regard to a history of HZ, number of previous treatments, and exposure to steroids before bortezomib treatment. The median duration from the time of MM diagnosis to the time of bortezomib treatment initiation in the HZ-positive group was significantly shorter than that in the HZ-negative group. The median ALC at the time of bortezomib initiation in the HZ-positive group was significantly lower than that in the HZ-negative group.</P><P><B>Conclusion</B></P><P>Bortezomib itself might act as a risk factor for HZ by inhibiting cell-mediated immunity, and patients with low ALC at the time of bortezomib treatment initiation were at greater risk of HZ during bortezomib treatment.</P>

        다양한 부위의 연(Lotus) 열수 추출물의 항혈전 활성

        연(Lotus)의 항혈전 활성을 평가하기 위해, 연잎, 연자방, 연자육, 연자심, 연근 및 우절로부터 각각 열수 추출물을 제조하고, 이의 성분 분석, 항응고, 혈소판 응집저해 및 적혈구 용혈활성을 평가하였다. 연의 부위별 추출효율은 우절>연자심>연근>연잎>연자육>연자방의 순으로 나타났으며, 추출물들의 pH는 5.6~6.5, 산도는 0.06~0.20%로 다양하게 나타났다. Brix의 경우 연자육에서 70%로 가장 높게 나타난 반면, 연잎과 연자방에서는 분석한계(0.1%) 이하로 나타났다. TP 및 TF 함량은 연잎에서 179.7 및 68.5 ㎎/g으로 가장 높은 함량을 나타내었고, 총당은 연자육에서 873.0 ㎎/g, 환원당은 연잎에서 161.4 ㎎/g으로 가장 높게 나타났다. 한편 항응고 활성은 우절, 연잎 및 연자방 추출물에서 우수하였으며, 특히 연자방의 경우 TT, PT 및 aPTT 평가에서 무처리구에 비해 모두 15배 이상 연장된 강력한 활성을 나타내었다. 그러나, 혈소판 응집 저해 활성을 평가한 결과 연자심 추출물에서만 0.25 ㎎/ml 농도에서 무첨가구에 비해 25.1%의 혈소판 응집을 나타내어 강력한 응집저해 활성을 나타내었다. 인간 적혈구에 대한 용혈활성 평가결과, 연자육 추출물에서만 6.1%의 약한 용혈활성을 나타내었고, 나머지 부위 추출물에서는 용혈활성이 나타나지 않았다. 본 연구결과는 연 산업의 부산물인 우절, 연자방, 연자육 및 연자심이 혈전성 질환 예방 및 치료용 식의약품 소재로 개발 가능함을 제시하고 있다. To investigate the anti-thrombotic activities of the lotus (Nelumbo nucifera Gaertner), various hot-water extracts were prepared from the leaf, pod of seed (PS), seed, embryo of seed (ES), root, and the node of root (NR) of the lotus. The highest extraction ratio was found in the NR (20.3%), followed by the seed, root, leaf, ES, and PS. These extracts had pH and acidity levels ranging from 5.6~6.5 and 0.06~0.20%, respectively. The seed extract showed 70% brix, whereas the leaf and PS extracts showed less than 0.1% brix. The highest contents of total polyphenol (179.7 ㎎/g), total flavonoids (161.4 ㎎/g), and reducing sugar (161.4 ㎎/g) were observed in the leaf extract, and the highest total sugar content (873.0 ㎎/g) in the seed extract. Anti-coagulation assay of the extracts of NR, leaf, and PS showed strong activities. In particular, at a concentration of 5 mg/ml, the PS extract had 15-fold extended thrombin, prothrombin, and activated partial thromboplastin times. However, only the ES extract showed activities inhibitory to platelet aggregation, with treatment with 0.25 ㎎/ml of ES extract decreasing platelet aggregation to 25.1%, a reduction comparable to that effected by aspirin. The extracts other than the seed extract showed no hemolysis activities against human RBC at treatments of up to 1 ㎎/ml. These results suggest that the NR, PS, seed, and ES, all byproducts of the lotus agriculture industry, have high potential as novel sources of anti-thrombotic agent.

        Analysis of acupuncture diagnostic decision from the clinical information of a functional dyspepsia patient

        Background: Clinical research in acupuncture has been criticized for not reflecting real-world practice in terms of diagnosis and intervention. This study aimed to collect data on the principles of diagnosis and selection of acupoints from Korean medicine doctors (KMDs) and analyze the patterns and priorities in decision-making. Methods: The study design was based on the data of an actual patient with functional dyspepsia (FD) (according to Rome III criteria) to create simulated patients, and a KMD specialized in gastrointestinal disorders was allocated to collect the clinical information as objectively as possible. Sixty-nine KMDs were recruited to diagnose a simulated patient based on the actual patient's clinical information, in a manner similar to that performed in their clinics. Results: After the diagnostic procedures were completed, the pattern identification, selected acupoints, reasons for choosing them, and importance of symptoms for deciding their diagnoses were documented. The information needed was clearly distinguishable from those routinely asked in western medicine, and information regarding fecal status, abdominal examination, appetite status, pulse diagnosis, and tongue diagnosis were listed as vital. The doctors identified the patient's pattern as “spleen-stomach weakness”, “liver qi depression”, or “food accumulation or phlegm-fluid retention”. The most frequently selected acupoints were CV12, LI4, LR3, ST36, and PC6. Conclusion: There are common acupoints across different patterns, but pattern-specific acupoints were also recommended. These results can provide useful information to design clinical research and education for better clinical performance in acupuncture that reflects real-world practice.

        원인불명의 습관성 유산 환자에서 말초혈액 자연살해세포의 증가가 임신초기 자궁혈류의 변화에 미치는 영향

        목적: 원인불명의 습관성 유산 환자에서 말초혈액 중 비정상적 비율로 증가된 자연살해세포가 자궁으로의 혈류를 감소시키는지에 대해 알아보고자 하였다. 연구방법: 본 연구는 전향적 연구로, 습관성 유산의 기왕력을 갖는 임신 5-7주 사이의 환자 33명을 대상으로 하였으며 습관성 유산의 기왕력이 없는 정상 임신 5-7주 47명을 대조군으로 하였고, 습관성 유산군은 말초혈액 자연살해세포 비율 12.1%를 기준으로 정상(normal) natural killer (NK) group과 증가(elevated) NK group으로 나누어 연구를 진행하였다. 임신 5-7주 사이에 측정한 말초혈액에서 유세포분석기를 이용하여 말초혈액 자연살해세포 분획 및 세포용해 활성도를 측정 후 그 결과를 비교 분석하였고 질식초음파의 Doppler를 이용하여 자궁방 사동맥의 저항을 측정 후 각 군에서 비교하였다. 또한 말초혈액 자연살해세포분획과 자궁방사동맥 저항지수 간의 연관성을 알아보았다. 결과: 자궁방사동맥의 평균 저항지수는 대조군에 비교하여 습관성 유산 환자군에서 통계학적으로 의미 있게 높았고(0.60 ± 0.14 vs. 0.54 ± 0.12, P = 0.039), 말초혈액 자연살해세포의 분획이 12.1% 이상 증가(elevated NK)군에서도 정상대조군에 비해 통계학적으로 유의하게 높게 나타났다(0.62 ± 0.13 vs. 0.54 ± 0.12, P = 0.029). 또한 전체 습관성 유산 환자에서 말초혈액 자연살해세포의 비율과 자궁방사동맥 저항지수 사이에는 양의 상관관계가 나타났고(P = 0.013, r = 0.429), 특히 습관성 유산 환자중 증가(elevated) NK군에서는 더욱 강한양의 상관관계를 보였다(P = 0.001, r=0.667). 결론 증가된 말초혈액의 자연살해세포는 자궁방사동맥과 같은 미세 혈관에 향염증 반응을 일으켜 자궁으로의 혈류를 감소시켜 습관성 유산을 야기한다고 설명할 수 있겠다. 하지만 본 연구의 결론을 확인하기 위해서는 더 큰 규모의 연구뿐 아니라 분자생물학적 기전에 대한 연구도 필요할 것으로 생각된다.         국가 맞춤형 폐자동차 해체시스템 선정 방법에 대한 연구

        폐자동차는 재활용 자원의 주요 원천이며 폐자동차 재활용 과정은 해체, 파쇄 및 ASR 처리 단계로 구분된다. 폐자동차 해체는 재사용 가능 부품을 수집하는 등 매우 중요한 단계인데, 크게 독립형(island type)과 라인형(line type) 으로 구분할 수 있고 다양한 유형의 해체 시스템이 존재한다. 또한 국가별 법적 규제, 폐자동차 발생량, 인구밀도, GNP 등 국가 별 특성이 다르기 때문에 특성에 맞는 적절한 해체 시스템을 체계적으로 선정하는 방법이 필요하다. 본 연구에서는 퍼지이론 및 변형 QFD를 활용하여 국가 특성에 대한 데이터 퍼지화 및 해체시스템 적합도를 평가하는 방법을 개발하였으며, 관련 데이터를 보유한 대표 국가에 대해 적용하고 결과를 평가하였다. The recycling process of ELV consists of three phases: dismantling, shredding and ASR treatment. Dismantling is the collection of reusable parts and the most important phase. The types of dismantling system is diverse and each country has different characteristics. Therefore, the selection of a suitable ELV dismantling system for a target country is dependent on the characteristics of each country. But the characteristics of country data changes every year and is insufficient and ambiguous. In this study, fuzzy inference and modified QFD (Quality function deployment) methods are utilized to solve the problems. The fuzzification of characteristics data for each country, customized rules and decision of modified QFD matrix are developed, which is applied to sample countries.
