        어린이의 사시교정술시 인공호흡수와 심박수와의 상관관계

        Correlation between Respiratory Rates and the Degree of Bradycardia in Strabismus Surgery in Children Jong-Man Kang, M.D., Hee-Soo Kim, M.D., Chong0Sung Kim, M.D. and Myung-Kul Yum, M.D.^* Department of Anesthesiology, College of Medicine, Seoul National University, Seoul, and ^*Department of Pediatrics, College of Medicine, Hanyang University, Seoul, Korea Background: Strabismus surgery is frequent in pediatric ophthalmic surgery. Traction on the extra-ocular muscles can trigger an oculocardiac rdflex. We investigated the correlation between respiratoy rates and the degree of bradycardia to find another management for preventing oculocardiac reflex. Methods: NO premedications were administered. They were induced with thiopental and vecuronium and maintained with O_@-enflurane-N_2O. The tidal volume was 10 ml/kg. Respiratory rate of group 1 (n = 18) was fixed at 24 bpm, group 2 (n = 18) at 20 bpm, and group 3 (n = 19) at 16 bpm. We measured the mean heart rate from intubation to the beginning of surgery and defined this as the basal heart rate. We defined the lowest heart rate after traction of the extraocular muscles as the minimal heart rate. We calculated the correlation coefficient between the respiratory rate and basal heart rate, minimal heart rate, the change of heart rates and E_tCO_2. Results: There was a positive correlation between the respiratory rate and the basal heart rate, and minimal heart rate. There was no correlation between the respiratory rate and the change of heart rate. There was no difference in E_tCo_2 among the 3 groups. Conclusions: Can be increased both basal heart rate in anesthetized patients and minimal heart rate induced by oculocardiac reflex by making the respiratory rate more rapid if there is no significant difference of E_tCO_2. The oculocardiac reflex may be reduced by making respiratory rate more rapid if there is no significant influence on minute ventilation. (Korean J Anesthesiol 2001; 41: 450~454)

        상아질에 적용된 재 습윤제가 미세인장 결합강도에 미치는 영향

        This study investigated that the effect of rewetting agent on dentinal microtensile bond strength (μTBS). Human molars were sectioned to expose the superficial dentin surfaces. Samples were divided into two groups according to type of adhesives-Single Bond (S) and One-Step (0)], and again subdivided into five groups by different dentin surface treatment-dry for 15s (D) , blot dry (BD) or dry for 15s, and rewet with different rewetting agents[distil1ed water (DW), Gluma Desensitizer (GD) and Aqua-Prep (AP)]for 30s. After application of adhesive, composite resin was built up on the bonding surface. Each tooth was sectioned to obtain stick with 1mm^(2) cross sectional area and the μTBS was determined by EZ test. In the S group, the mean μTBS of GD, AP and BD group was significantly higher than that of DW and D group (p < 0.05). In the 0 group, the mean μTBS of AP, GD, BD and DW group was significantly higher than that of D group (p < 0.05). The data suggested that Gluma Desensitizer and Aqua-Prep could be successfully used as rewetting agents, and Distilled water could be acceptable in aceton based adhesive system only.

      • 심층면접 방법의 실제 : 청주 지역사회 권력자들에 대한 심층면접 사례를 바탕으로 With Reference to a Study of Power Elites in Chongju Community

        권력자를 연구하는 방법으로는 양적인 방법(표본조사)과 질적인 방법(심층면접)이 있지만, 표본수가 원천적으로 적을 수 밖에 없는 권력자(특히 지역사회 권력자) 연구에서는 기본적으로 비구조화된질문지를 통한 심층면접을 주로 사용한다.권력자들을심층면접할 때 부딪치는 첫번째 문제는 그들을 쉽게 만날 수 없다는 것이다.이것은 연구의 사활이 걸린 문제이다.이 연구에서는 연구기관이 대학이라는 것이 권력자들을 쉽게 접촉할 수 있게 하였다.다음으로 면접 대상자로 결정된 권력자들에 대하여 사전조사를 철저히 해야 한다.이를 위해 면접 자료 뿐만 아니라 권력자들의 저서,연설,공문서 등 2차 자료도 충분히 숙지한다.그리고 면접자는 면접상황(면접 상황,호의 여부 등)을 상세하게 기록한다. 심층면접을 보다 잘 하기 위해서는 특히 질문 문항에서 체계적인 사전 계획이 필요하다.그리고 연구 주제를 잘 정해야 한다.청주 연구에서도 버스 터미널 이전처럼 청주 지역사회 권력자들의 의사결정 과정을 알아볼 수 있는 주제를 정했는데, 이것에 관여한 응답자가 거의 없어서 결과적으로 성과를 거두지 못했다.기술적 문제로서 지키지 못할 약속을 하지 말아야 한다.이 연구에서는 응답자들에게 녹음 해독을 다시 제시하여 검토받을 것이라고 약속했으나 지키지 못하였다. Although both quantitative(sample survey) and qualitative(interview) methods are available most studies of power elites utilize unstructured interview method as the number of power elites are very few. I would like to mention two problems faced by researchers who try to administer in-depth interviews with power elites. First, it is not easy to mate appointments with power elites. Basically the study can not he carried out without meeting and interviewing them. Secondly, prior to the interview researchers must perform a thorough preparatory study regarding the interviewee(the power elite) including secondary materials such as his/her writings and speeches, relevant public documents as well as interview schedule for the study. The researchers must record in detail interview situations such as general atmosphere and the attitudes of the interviewee about specific topics. Four researchers(Min Kyonghee, Kang Hee-Kyung, Bae Youngmok, and Choi Youngchul Choi) carried out unstructured in-depth interview with 39 power elites out of total 52 power elites in Chongju Community from Feb. 14th through July 17th of 1995. The researchers wanted to find out the following: the existence of core power elites in Chongju, internal structure and the characteristics of this elite group, mechanisms through which power is wielded, decision-making processes among power elites in relation to different subjects. E-mail: kkhee255@trut.chungbuk.ac.kr. seemed to have helped with the first problem mentioned above: the researchers had easy access to power elites. A systematic preparatory planning especially with regard to interview questions is necessary in order to better utilize in-depth interview method. In addition, researchers must be careful in selecting the subject of study. For example, in the study mentioned above we selected the relocation of bus terminal as the subject of our study as it had a communitywide importance. However, not many of the interviewed power elites had anything to do with the issue and the study was not successful in grasping the decision-making process involved in the relocation of bus terminal. Lastly, a technical note should be mentioned: as a researcher one must not make a promise that cannot be kept. In our study researchers promised the power elites that they should be given a chance to review the interview record. But for various reasons the researchers were not able o keep the promise.

        Assessment of In Vitro Assay System for Thyroid HormoneDisruptors Using Rat Pituitary GH3 Cells

        The development of in vitro assays has been recommended to screening and test-ing the potential endocrine disruptors (EDs). These assay systems focus only on identifying thethe thyroid hormone (TH) disruptors. The aim of this study was to evaluate a test system to detectTH disruptors using rat pituitary tumor GH3 cells. The test system is based on the TH-dependentincrease in growth rate. As expected, L-3,5,3-triiodothyronine (T3) markedly induced a morphologicalchange in GH 3 cells from flattened fibroblastic types to rounded or spindle-shaped types. T3 stimu-lated GH3 cell growth in a dose-dependent manner with the maximum growth-stimulating effect9 M. In addition, T3 increased the release of growth hor-mone and prolactin into the medium of the GH3 cells culture. Using this assay system, the TH-dis-rupting activities of bisphenol A (BPA) and its related compounds were examined. BPA,dimethylbisphenol A (DMBPA), and TCI-EP significantly enhanced the growth of GH3 cells in therange of 1 × 10-5M to 1 × 10-6M concentrations. In conclusion, this in vitro assay system might bestandardization before it can be used as a broad-based screening tool.

      • Investigation of the April 2000 forest fire damage in Kang-won Province, Korea, using RADARSAT data

        The series of forest Fires in Kangwon Province in April, 2000, were one of the mast serious ones in the Kang-won Province in recent years. The fire damaged areas are extensive and this paper investigates the utility of the satellite image data, particularly the RADARSAT SAR data. One of the main objectives of this study included evaluation of potential usefulness of the C-band RADARSAT data in the detection of forest fires in mountainous areas. Among several fire damaged sites, the area near Samchuk (city) was selected as the main study area and mufti-temporal RADARSAT(raw) data were acquired and the processed results were comparatively investigated with the IRS-1c data acquired shortly after the fire. To identify the burnt area from the undamaged background in the optical imagery, supervised classification and NDVI processing were carried out and tested with the IRS-1C data. For this experiment, several sets of raw RADARSAT standard and Fine mode (S4, F3, ascending orbit) data were processed and the final image was formed. In addition to the final image, sigma nought (σ°) was catnputed and compared for the burnt area against the undamaged areas. Because of the steep slopes in the study area and subsequent deviation from the flat surface scattering models, the fire scars in the sigma nought (σ°) image were not apparent. However, fire scars are slightly detected in the difference and ratio images obtained from the signal intensity data acquired at different dates (bath before and after fire). Texture analysis of the image data including the burnt areas were carried out using the Multiplicative Markov Random-Field (MMRF) algorithm. The detection of the fire damaged area from both standard mode and fine mode RADARSAT was not as straightforward as in optical data. However, Previous studies with ERS-1 S_AR and RADARSAT data in Indonesia and in the North American Plains indicate that C-band SAR data provide us with an effective tool for studying the forest fire damages. However, the application of C-band SAR data to the analysis of the forest fire areas requires careful processing of the data with realistic scattering models.

        발효 온도-시간 조합이 배추김치의 품질 특성에 미치는 영향

        In order to examine the effect of the combination of fermented temperature and time on Baechu kimchi in a kimchi refrigerator, Baechu kimchi was fermented at four different modes of the fermentation temperature and time for 16 weeks and analyzed the properties of Baechu kimchi. The pH, Baechu kimchi fermented at 20℃ for 24 hours/stored at -1℃, decreased rapidly during first week and then decreased very slowly. The hardness and the chewiness of Baechu kimchi fermented at high temperature were higher and the values were decreased when the fermentation continued. In sensory evaluation, carbonated flavor in Baechu kimchi fermented at 20℃ for 24 hours/stored at -1℃ was the best after 4 weeks, and Baechu kimchi fermented at 5℃ for 3 days or 6 days/stored at -1℃ was the best after 8 weeks. The scores for sourness were the highest on 8 weeks and 12 weeks in kimchi fermented at 20℃ for 24hours/stored at -1℃ and 5℃ for 3 days or 6 days/stored at -1℃, respectively. Total microbial count was increased as the temperature of fermentation increased. Counts of Leuconostoc spp. reached to the highest after 6 days and counts of Lactobacillus spp. seached to the highest after 5 days in kimchi fermeted at 20℃ for 24 hours/stored at -1℃. From these results, it was concluded that it required 4 weeks to eat most edible Baechu kimchi in kimchi refrigerator 20℃ for 24 hours/stored at -1℃, and 8 weeks to eat edible Baeche kimchi refrigerator fermented at 5℃ for 3 days or 6 days/stored at -1℃.

        발효 온도와 시간 조합을 달리한 총각김치의 품질 특성

        To determine the conditions of the fermentation and storage for Chonggak kimchi refrigerator, prepared Chonggak kimchi took into kimchi refrigerators which were controlled at four different modes of the fermented temperature and time, and fermented and kept for 16 weeks. The pH in Chonggak kimchi fermented at 20℃ for 24 hours/stored at 01℃ dropped greater than all of kimchi fermented at other combinations, and the changes of pH at any combinations were not greater than those in Baechu kimchi, because pH in Chonggak kimchi did not dropped below 4.5. Acidities in Chonggak kimchi were greatly increased at higher temperature. The acidity in Chonggak kimchi during the first week of fermentation was lower than that in Baechu kimchi and then it was rather higher becuase of the addition of waxy rice paste. In texture, puncture force of Chonggak kimchi was decreased slowly until 8 weeks of fermentation and then did not changed much and the highest values showed in Chonggak kimchi stored directly at -1℃ without any fermentation. In sensory evaluation, the score for the carbonated flavor and the sourness were the highest in Chonggak kimchi fermented at 20℃ for 24 hours/stored at -1℃, but the lowest in Chonggak kimchi stored directly at -1℃ without any fermentation because of some undesirable flavors. The lowest hardness showed in Chonggak kimchi fermented at highest temperature and the best hardness was in Chonggak kimchi fermented at 5℃ for 3 days or 6 days/ stored at -1℃. The appearance was the best in Chonggak kimchi fermented at 20℃ for 24 hours/stored at -1℃ and the worst was in Chonggak kimchi stored directly at -1℃ without any fermentation. The overall acceptability of Chonggak kimchi fermented at 20℃ for 24 hours/stored at -1℃ was good after 4 weeks of fermentation, but in Chonggak kimchi fermented at 5℃ for 3 days or 6 days/stored at -1℃ it was food after 6 weeks. Total microbial counts in most of Chonggak kimchi were reached to a maximum number within 7 days, and then decreased similarly at all modes. Leuconostoc spp. and Lactobacillus spp. increased to maximum number of 1.48 10^(9) and 5.62 0^(9), respecively, in Chonggak kimchi fermented for 7 days. Yeast counts showed a increasing trend not depends on fermenting temperature and they were lower counts than those in Baschu kimchi. Waxy rice paste which added to Chonggak kimchi resulted in increasement of glucose as a carbon source and stimulated to reproduce the microbes in Chonggak kimchi.

One of the most important constraints in Yakut is connected with syllabic structure. Its syllabic structure is based on CV. So the constraint of "NoGeminates" is strictly applied. And the harmony between features is also considered important. Yakut has the strict constraint on the phonotactical relations. In Yakut a feature [constricted glottis] works more vigorously than [voiced].         조기 발병한 정신증적 장애에서 자기공명영상을 이용한 측뇌실에 대한 구조적인 예비연구

        연구목적:측뇌실의 확장은 성인기의 정신분열병에서 잘 확립되는 소견이지만, 조기에 발병한 정신증적 장애에서는 아직 논란이 있다. 본 연구에서는 자기공명영상을 이용한 후향적인 방법을 통하여 지금까지 비교적 연구가 많이 시행되지 않았던 조기발병한 정신분열증·양극성 장애 환아들을 대상으로 측뇌실의 구조적인 이상을 연구하여, 이러한 뇌의 구조적 이상이 병의 초기부터 시작되어 정상과는 다른 경과로 진행되는 것인가를 알아보고자 하였다 방법:정신분열병과 양극성 장애의 DSM-Ⅲ-R 진단 기준에 부합하는 14명의 환아(평균 연령 14.0세,표준편차 2.1)와 26명의 두통 대조군에서 뇌자기공명 영상상을 후향적으로 모집하였다. 분석을 위하여 측뇌실이 가장 잘 보이는 축 영상(axial image)를 선택하였다. 뇌자기공명영상상은 평판형 스캐너로 읽어들여서 NIH IMAGE 프로그램을 통하여 분석하였다. 결과:환자군에서는 측뇌실 크기, 측뇌실확장지수가 연령의 증가에 따라 함께 증가하는 상관관계를 나타내었으나, 대조군에서는 연령과 측뇌실 크기, 측뇌실확장지수 등이 유의한 상관관계를 보이지 않았다. 환자군에서는 정상군에서 보이는 좌우 측뇌실 비대칭성이 소실된 결과를 보였다. 그리고 통계적으로 유의한 정도는 아니었으나 환자군이 대조군에 비해 측뇌실과 측뇌실확장지수가 큰 경향을 보였다. 결론:정신증적 환자에서의 뇌의 구조적 이상은 병의 비교적 초기 경과에서부터 시작되며, 정상과는 다소 다른 경과를 취하여 진행한다는 것을 시사하는 결과라 할 수 있다. Objective : The enlargement of the lateral ventricle is well established finding in adult-onset schizophrenia, but in the patients with early-onset psychotic disorder(EOPD) is still controversal. To identify structural abnormalities of the lateral ventricle in patients with EOPD and difference of the brain developmental course in EOPD group, planimetrical assessment of the lateral ventricle was conducted by means of retrospective quantitative neuroimaging. Methods : The brain MR images of 14 patients with EOPD(mean age 14.0±2.1 years) who met the DSM-Ⅲ-R diagnostic criteria for schizophrenia and bipolar disorder and 24 control subjects with simple headache(mean age 10.0±2.1 years) were collected. Single best view for the lateral ventricle, one axial slice was selected for analysis. MR images were redigitalized with flatbed scanner and data were analyzed with NIH IMAGE software. Results : In the patients with EOPD, there were significant correlations between aging and lateral ventricular sizes and between aging and ventricular brain ratio(VBR). The statistical significance of normal asymmetry of the lateral ventricle was not observer for the EOPD group. The EOPD group had larger lateral ventricular sizes and VBR than control group, but these results were not statistically significant. Conclusion : These findings suggest that the brain abnormalities in patients with EOPD progress from the earlier course of the disorders and the course of brain development in EOPD group is different from control group.
