        일 대학병원 주차장 유료화에 따른 주차장 이용실태 조사

        Background : As parking problem caused by increasing owner-driver and patients concentration to a general hospital is becoming one of the dissatisfactions in medical care. It is time that a general hospital should solve the parking problem in a desirable way. The purpose of this survey is to let the clients understand the basic motivation of the pay parking and develop the better parking system. Methods : Clients of a tertiary care hospital in Chon-ju were surveyed by means of a questionnaire. All in all, 193, subjects answered the questionnaire. Results : In relation to the previous experiences, 39.6% of the subjects experienced inconvenience with confuse parking lot and the shortage of parking space. Under the were more than those who didn't 62.7% of the subjects answered that the y could find the parking lot easily. 33.2% of the subjects mentioned that it was not easy to drive in the parking area ; The reasons were pointed out the shortage of space, disordered parking, and insufficient guide, 12.8% of the subjects satisfied with the current administering system of parking lot. The outpatients were more affirmative than the admitted patients about the charging system. As for the parking fee, 64% of the subjects answered that it is expensive, and 89.5% of the subjects answered that it is expensive, and 89.5% of the subjects thought imposing of parking fee is irrational. Conclusion : To say as a whole, th basic purpose of the charging system are more of less accepted. However, the management details like parking facilities and payment method are dissatisfactory, so it is necessary to improve the management system. It is also noted that the inpatients showed more negative attitude than the outpatients with the charging system. To secure a more convenient parking, the parking system should be considered in relation to the information service, kind guidance, improve facilities, personnel cooperation, fix outpatient scheduling system. etc.

        생태계서비스 기반 국립공원 생태복지 개념정립 및 정책방향 설정

        생태복지는 인간의 복지와 건강한 생태계의 상호관계성에 대한 국제적 담론에서 시작되었다. 본 연구는 국립공원이 생태복지의 최대 실현가능 지역이라는 점에서 국립공원에 적합한 ‘국립공원 생태복지’ 개념을 정립하고 정책방향을 설정함으로써 향후 국립공원에서의 생태복지 실현을 도모하기 위한 기초자료를 제공하는데 그 목적이 있다. 이를 위해 사전워크숍과 총 2회에 걸친 델파이 조사를 수행하였다. 본 연구를 통해 ‘국립공원 생태복지’는 국립공원에서 생태계의 지속가능성과 생물다양성을 유지하며, ‘국립공원 인간복지’와 ‘국립공원 생태계복지’의 조화를 지향하는 복지로 정의되었다. 2차 델파이 결과 모든 항목에서 평균 4.00 이상, 내용 타당도(CVR) 0.37 이상, 변이계수(CV)0.5 미만으로 타당한 내용으로 확인되었다. 국립공원 생태복지의 정책방향을 설정하기 위해 생태복지의 바탕이 되는 생태계서비스의 요소들 중 국립공원에 적합한 주요 요소를 도출한 결과 공급서비스에서 먹이·식량, 물, 유전자원 3항목, 조절서비스와 기반(서식지)서비스의 모든 항목, 문화서비스에서 미적정보, 휴양관광, 심신치유, 지식체계ㆍ교육적가치 4항목이 최종 도출되었다. 또한 본 연구에서는 국립공원 생태복지의 개념과 향후 국립공원 생태복지 정책이 어떻게 나아가야할지 방향성을 제시하였다. 그러나 이를 실현하기 위해서는 향후 이에 대한 구체적인 단위사업에 대한 실행계획이 수반되어야 하며 이를 위한 추가적인 연구가 필요할 것이다. The concept of Eco-welfare emerged in the process of discovering the relationship between a healthy ecosystem and human wellbeing. The objective of this study is to offer basic data for eco-welfare policy realization by conceptualizing the ‘National Park Eco-welfare’ since national parks are appropriate places for eco-welfare to be optimally implemented. A pre-workshop and two rounds of Delphi surveys were conducted to determine the concept of the ‘National Park Eco-welfare ’ and the main elements to be included in ecosystem service. Through this study, the concept of ‘National Park Eco-welfare’ was defined as efforts to conserve the biodiversity and sustainability of the ecosystem and create harmony between ‘National Park Ecosystem Welfare’ and ‘National Park Human Welfare’. To establish the direction of the National Park Eco-welfare policy, we identified the main elements of ecosystem services which are appropriate for the National Park. These are comprised of 3 elements (food, fresh water and genetic resources) from supply service, all elements from regulating service and supporting/habitat service and 4 elements (aesthetic information, recreation-based ecotourism, healing, and knowledge systems with educational values) from culture service. In this study, the concept of National Park Eco-welfare was established, and its policy objectives and scope were suggested. However, further studies are necessary to develop action plans areand thereby realize the policy.

        중국 물류단지 육성정책과 시사점에 관한 연구 -상해물류단지를 중심으로-

        중국은 개혁개방 이후 급속한 경제성장을 이룩하였으며 이제는 더 이상 세계의 공장이 아닌 세계의 시장으로 주목받고 있다. 급속한 경제성장의 배경에는 물류산업이 뒷받침하고 있으나 중국 물류산업의 발전수준은 아직까지 초기성장단계라고 할 수 있다. 중국 정부는 물류산업의 발전을 위해 물류단지를 핵심요인으로 인식하여 발전을 추구하고 있지만 정책과 지원체계가 일관되지 않기 때문에 발전의 많은 어려움이 존재한다. 물류단지란 물류 공동화와 운영의 효율성을 위해 물리적으로 물류시설과 물류기업들을 집중시키는 지역을 의미한다. 상해는 중국에서 물류단지가 가장 활발하게 운영되는 지역이며, 상해에는 외고교, 푸동공항, 심수항, 서북종합 물류단지와 계획 중인 서남종합 물류단지까지 총 5개의 물류단지가 있다. 대부분의 물류단지는 「10,5 상해 현대물류산업 발전」에서 구축이 제기되었고 푸동공항 물류단지와 심수항, 외고교 물류단지로 이어지는 "3항3구" 가 형성되어 있다. 최근 상해 지유무역시범지역의 구축으로 인해 상해 물류단지의 많은 발전 가능성뿐만 아니라 항만과 공항의 연계작용 또한 활발할 것으로 예상된다. 따라서 본 연구에서는 중국 물류단지의 육성정책과 상해 물류단지의 사례 분석을 통해 국내 항만과 상해로의 진출을 모색하는 기업들에게 시사점을 제공해주고자 한다. Since the country reopened, China has been achieving rapid economic growth. They are emerging as a huge market in the world. Although the logistics industry has lead to growth, the development level of Chinese is still in its early stages. The government in China regards Logistics Park as a key factor for the development of China``s logistics industry. There are many difficulties, however, because of the inconsistent government policies and support systems. Logistics Park is an area that has gathered logistics facilities and firms for logistics cooperation and efficiency. Shanghai manages its logistics parks very aggressively and there are five logistics parks. They are operating four logistics parks such as Waigaoqiao, Pudong International Airport, Yangshan Deep-water Port, North-West Comprehensive logistics park and South-West Comprehensive logistics park is under contemplation. Most logistics parks were planned in the 10th Five-Year Plan for Modern Logistics Industry Development of Shanghai. And there were formed "Three Port Three Park" by connecting Pudong International Airport logistics park, Yangshan Deep-water Port logistics park and North-West Comprehensive logistics park. They are expected to grow remarkably as they designate a Pilot Free Trade Zone and to actively interact between port and airport. The purpose of this paper is to provide suggestions for ports in Korea and firms that want to enter into Shanghai. This paper will provide assistance through the analysis of China``s development policies and case studied of Logistics Park in Shanghai.

        Insulin resistance mediates high-fat diet-induced pulmonary fibrosis and airway hyperresponsiveness through the TGF-β1 pathway

        <▼1><P>Prior studies have reported the presence of lung fibrosis and enhanced airway hyperresponsiveness (AHR) in mice with high-fat-diet (HFD)-induced obesity. This study evaluated the role of TGF-β1 in HFD-induced AHR and lung fibrosis in a murine model. We generated HFD-induced obesity mice and performed glucose and insulin tolerance tests. HFD mice with or without ovalbumin sensitization and challenge were also treated with an anti-TGF-β1 neutralizing antibody. AHR to methacholine, inflammatory cells in the bronchoalveolar lavage fluid (BALF), and histological features were evaluated. Insulin was intranasally administered to normal diet (ND) mice, and in vitro insulin stimulation of BEAS-2b cells was performed. HFD-induced obesity mice had increased insulin resistance, enhanced AHR, peribronchial and perivascular fibrosis, and increased numbers of macrophages in the BALF. However, they did not have meaningful eosinophilic or neutrophilic inflammation in the lungs compared with ND mice. The HFD enhanced TGF-β1 expression in the bronchial epithelium, but we found no differences in the expression of interleukin (IL)−4 or IL-5 in lung homogenates. Administration of the anti-TGF-β1 antibody attenuated HFD-induced AHR and lung fibrosis. It also attenuated goblet cell hyperplasia, but did not affect the AHR and inflammatory cell infiltration induced by OVA challenge. The intranasal administration of insulin enhanced TGF-β1 expression in the bronchial epithelium and lung fibrosis. Stimulating BEAS-2b cells with insulin also increased TGF-β1 production by 24 h. We concluded that HFD-induced obesity-associated insulin resistance enhances TGF-β1 expression in the bronchial epithelium, which may play an important role in the development of lung fibrosis and AHR in obesity.</P></▼1><▼2><P><B>Obesity: A trigger for asthma onset</B></P><P>Insulin resistance may be an important causative factor underlying the increased risk of asthma and other respiratory issues in obese individuals. Obesity doubles the likelihood of developing asthma, with symptoms that are more difficult to control than in non-obese patients. The connection between these conditions is poorly understood, but researchers led by Jung-Won Park, Yonsei University College of Medicine, Seoul, South Korea, have identified a potential mechanism. They demonstrated that a signaling molecule called TGF-β1 contributes to airway sensitivity and tissue scarring in a mouse model of diet-induced obesity. Subsequent experiments showed that treatment with insulin also gives rise to increased TGF-β1 production in the mouse lung. Since insulin resistance is a common feature of obesity, resulting in abnormally high levels of circulating insulin, this could also account for the increased risk of respiratory problems.</P></▼2>

      • 사찰의 문화재관람료 법적 연구

        국립공원의 입장료가 폐지되어 수년이 지났지만 국립공원 내에 있는 사찰의 문화재관람료는 계속 징수하고 있어 문화재관람을 원하지 않은 공원탐방객의 불만과 함께 관람료에 대한 타당성의 논쟁이 여전히 계속되고 있다. 관람료징수의 입장에서는 국립공원을 입장 할 때는 사찰의 문화재도 함께 관람한다는 전제가 포함되어 있다는 것이다. 문화재를 둘러싼 주변의 환경까지도 역사문화 환경에 포함된 개념이기 때문에 문화재를 관람할 의사가 없다고 해도 사찰문화재를 향유하고 있다고 보아서 관람료징수는 정당하다는 주장이다. 반면, 관람료징수의 부당성 측면에서는 관람료징수와 관련된 문화재보호법과 자연공원법이 헌법적 가치인 국민의 기본권 즉, 인간의 존엄과 가치ㆍ행복추구권, 인간다운생활을 할 권리, 재산권과 사적자치권을 침해하는 위헌성을 안고 있고 포괄위임금지원칙과 법률유보원칙의 위헌성도 제기되고 있다. 그동안의 관람료징수에 대한 관련 법제도의 과정과 분쟁의 사례를 살펴보고 법적 타당성을 검토하였다. 합리적 대안으로서 국민의 기본권이 침해당하는 위헌의 소지를 없애고 사찰의 문화재관람료 사용에 대한 투명성, 징수구역의 재설정, 사찰의 비영리법인화, 관람료의 사전승인제도 등을 검토하여 명확한 관련법제ㆍ개정의 시급함을 제시하고자 하였다. 사찰의 문화재는 소중한 우리의 문화유산이며 공공의 재산이다. 따라서 국민이 누릴 수 있는 문화재향유권의 영역에서 사찰문화재 관람료징수문제가 검토되어야 할 것이다. Several years have passed since the admission fee for national park was abolished. However, the admission fee for cultural properties in Buddhist temple is being continuously collected. Thus, the argument of validity on admission is still being continued along with dissatisfaction in park visitors who don’t want to see the cultural properties. From the position of collecting admission fee, it is said to be necessarily included the proposition as saying that even the cultural properties of Buddhist temple are viewed together when entering the national park. It is a concept that even the environment of surrounding the cultural properties is involved in the environment of historical culture, thereby being considered to enjoy the cultural properties of Buddhist temple even if having no intention of seeing cultural properties, resulting in asserting that collection of admission is righteous. On the other hand, from the aspect of iniquity in collecting admission fee, there is the violation of the constitution that infringes on people’s basic right, which is the constitutional value in the Cultural Properties Protection Law and the Natural Park Law related to collecting admission, namely, on the Human Dignity and the Right of Pursuing Happiness, the right of a Life Worthy of Human Beings, property right and private autonomy. Even the violation of the constitution against the non comprehensive delegation doctrine and the Reservation of Laws is being raised.36) It examined the process of legal system and a case of dispute in relation to collecting admission fee in the meantime, and inquired into legal validity. As a reasonable alternative, the aim was to suggest the urgency of the definite enactment and the revision of the relevant law by removing the possibility of violating the constitution that people’s basic right is infringed and by examining the transparency on the use of cultural-property admission in Buddhist temple, the re-establishment of collection section, making a Buddhist temple noncommercial corporation, and the prior approval system of admission. The cultural properties of Buddhist temple are our precious cultural heritages and public assets. Accordingly, there will be a need of examining a problem of collecting admission fee for cultural properties of Buddhist temple in the sphere of the Enjoyment Right of Cultural Properties that can be enjoyed by people.

        지리산국립공원 심원계곡 생태경관 복원공사지역 식생 변화

        본 연구는 지리산국립공원 심원계곡 생태경관 복원공사지역의 모니터링 결과를 분석하여 체계적인 복원관리를 도모할 수 있는 기초자료를 확보하는데 그 목적이 있다. 복원사업이 완료된 2017년도에 식생 모니터링을 위한 조사구 12개소와 대조구 4개소를 설치하였고, 2017년과 2020년의 상대우점치, 종다양도지수, 유사도 등의 변화를 분석하였다. 복원사업 시 수목이 식재되었던 조사구는 종다양도지수가 0∼1.4552로 나타났고, 대조구와의 유사도지수에서는 1개 조사구가 1.32%로 분석된 것을 제외하고 모두 0%로 나타났다. 조사구의 종다양도지수 및 유사도지수가 매우 낮은 것은 복원사업 시 식재된 수목이 고사하거나 멧돼지 피해, 유수에 의한 침식으로 유실되는 등의 영향으로 판단되었다. 반면 복원사업 시 수목이 식재되지 않은 미식재 조사구는 종다양도지수가 0.9538∼2.3222로 나타났고, 유사도지수는 최대 8.33%까지 분석되었다. 국립공원 생태경관 복원공사지역의 복원 공법 개발을 위한 장기 모니터링과, 미식재 조사구의 천이과정을 분석하는 지속적인 연구가 필요하다. This study aims to obtain basic data for systematic restoration by analyzing the monitoring results of the Shimwon Valley Ecological Landscape Restoration Project area in Jirisan National Park. In 2017, when the restoration project was completed, 12 monitoring plots and 4 control plots were installed for vegetation monitoring, and changes in the relative dominance, species diversity index and similarity between 2017 and 2020 were analyzed. The species diversity index of the surveyed areas where trees were planted during the restoration project was 0-1.4552, and the similarity index with the control group was 0% except for one survey area at 1.32%. The very low species diversity index and similarity index in the survey areas were attributed to the loss of trees planted during the restoration project due to death, damage by wild boars, or erosion by running water. On the other hand, the species diversity index was 0.9538-2.3222 in the monitoring plot where no tree was planted, and the similarity index was analyzed to be as high as 8.33%. It is necessary to continue the long-term monitoring for the development of ecological landscape restoration methods in the national park and analysis of the succession in monitoring plots where no trees were planted.

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

      • 한국에서의 간호중재분류체계(NIC)구조 타당성 검증

        The purpose of this study was to validate the NIC taxonomy in Korea. A sample of 10 nurse experts who were head nurses at a university affiliated hospital and had some knowledge and experience with research method and NIC rated, the NIC taxonomy twice using five criteria: clarity; homogeneity; inclusiveness; mutual exclusiveness; theory neutral. The NIC Taxonomy Evaluation questionnaire developed by the Iowa Intervention project team was used for data collection. The instrument was translation and back-translation. The results were as follows: 1. About fifty-eight percent of the respondents rated the domains as either quite characteristic or very characteristic on all criteria and about seventy -one percent of the respondents rated the classes as either quite characteristic or very characteristic on all criteria. 2. The criterion of theory neutral received the highest ratings and the criteria of clarity and inclusiveness received the lowest ratings on domains. 3. The criterion of theory neutral received the highest ratings and mutual exclusiveness received the lowest ratings on classes. 4. The Physiological: Complex domain received the highest ratings and the Family domain received the lowest ratings. These findings suggest that Nursing Interventions Classification System sensitive to Korean culture need to be developed.

Average allele number per primer pair was 3.36. Genetic similarities for every pair of cultivars ranged from 0.42 to 0.97, with 0.69 of overall average. Korean cultivars were divided into two major groups based on microsatellite DNA polymorphisms. Group I consisted of relatively old cultivars developed until 1970s, and group II contained the recent cultivars developed during 1980s and 1990s. Amongst old elite cultivars/lines, ‘Yukseung 3’, ‘Norin 12’ and ‘Norin 72’ contributed most to the genetic background of cultivars belonging to group I, and ‘Norin 4’, ‘Norin 12’, ‘Norin 43’ and ‘Norin 72’ to group II, respectively. The phylogenetic relationship of Korean wheat cultivars was in accordance with the genealogical data of each cultivar. The genetic background of each cultivar was assessed from the point of breeding and germplasm management such as variety identification and duplicated accessions for assisting in developing a system for the registration of new variety based on the molecular characterization in future.
