A 23-year-old female patient was diagnosed with congenital long QT syndrome, discovered when she visited our cardiac department due to chest discomfort, because she had a prolonged QTc interval on ECG, history of syncopal attacks and seizures several times every year, and ingestion of anticonvulsants for several yeam. It is well known that the long QT syndrome is associated with sudden death secondary to ventricular tachyarrhythmia or fibrillation at a young age. Moreover cardiac artests during induction, maintenance, and awakening of anesthesia of these patients have been reported, and may be due to asymmetrical adrenergic stimuli in the heart, especially in the unrecognised case. This case report describes the anesthetic management of a patient with congenital long QT syndrome, who presented for cervicothoracic sympathectomy for a more permanent control of life-threatening ventricular arrhytbmias, and reviews the related literature.

Background/Aims: Cluster differentiation 44 standard isoform (CD44s) is a transmembrane glycoprotein. CD44s is a known prognostic factor in various cancers, due to its involvement in tumor cell growth, invasion and metastasis. Its prognostic role, however, is debated because it can be a positive or negative prognostic factor depending on tumor type and is still an ambiguous prognostic indicator in other cancers, especially hepatocellular carcinoma (HCC). We investigated the relationship between CD44s expression and survival in HCC patients. Methods: A total of 260 HCC samples were collected to generate a tissue microarray. Staining of the arrays with a primary mouse CD44s monoclonal antibody was followed by evaluation of the relationship between CD44s expression and tumor differentiation. The effect of CD44s expression on patient survival was analyzed. Results: CD44s protein expression correlated with histological grade (most and worst Edmondson grade) of the HCC (p=0.029 and p=0.039, respectively) and adversely affected the disease free survival period based on univariate and multivariate analyses (p=0.038 and p=0.077, respectively). Conclusions: High CD44s protein expression correlates with shorter disease free survival and poorly differentiated HCC. CD44stargeted therapy may be effi cacious for HCC treatment in the future. Objective: Decompression sickness (DCS) is one of the most important diseases in underwater medicine. However, we do not know about the pathogenesis and related factors of DCS, therefore prevention or treatment are difficult. Nowadays, inflammatory process has been known as the major pathogenesis in DCS, though, only few proteins are concerned with provocation factors. We tried to evaluate the expression of some inflammatory factors, especially Cyclooxygenase-2(COX-2) and Heat shock protein-70 (HSP-70), in DCS animal model. Methods: SD rats (n=15) were randomized into two groups. Control group (n=5) was safely decompressed after pressurization (175ft. 6.2ATA) for 10 hours. Experimental group (n=10) was abruptly decompressed to atmospheric pressure. All animals were euthanized, and blood and lung were collected for analysis of CRP and histologic evaluation such as degree of inflammation with immunohistochemical analysis for COX-2 and HSP-70 proteins. Results: Dehydrated group showed tendency of increasing level of CRP. The number of animals showing high serum level was observed in 7 out of 10 experimental group (70%). In the other hand, increased CRP level was found in two out of 5 control group (40%). Immunohistochemical analysis has been resulted that the status of COX-2 and HSP-70 expressions were higher in lung of experimental group than of control group as 40% and 50%, respectively. Conclusion: The two proteins showed elevated expression levels in experimental group than in control ones. Then we may suggest the relationship between inflammatory factors and DCS.

Purpose. The aim of this study was to evaluate the reliability of implant stability measuring devices depending on the location of the implant and the position of the patient. Materials and Methods. Six implants were installed in different dentate sextants of six artificial bone models. Implant stability was measured in three conditions of the bone model (without mounting on a phantom head, mounted on a phantom head in supine position, and mounted on a phantom head in upright position). A resonance frequency analysis device (Osstell) and two damping capacity analysis devices (Periotest and Anycheck) were used to measure implant stability. The values measured outside the phantom head were treated as controls, and the values inside the phantom head were compared using an independent t-test. Results. Osstell showed different results in two of the six divisions in both the supine and upright positions compared to outside of the mouth (P < .05). Periotest showed different results in all six parts in the supine position and in five parts in the upright position compared to outside of the mouth (P < .05). While Anycheck showed different results in five areas in the supine position compared to outside of the mouth, it showed different results in only one area in the upright position (P < .05). Conclusion. In the difficult implant position for the operator to access, the implant stability measuring devices show less reliability. The accessibility of implant is greatly affected in the order of Osstell, Anycheck, and Periotest.
