        가토를 이용한 PYROST(Porous Hydroxyapatite)의 골 재생에 관한 실험적 연구

        PYROST is a biocompatible porous matrix implantable as a bone graft substitute. This biomaterial has been used in a bariety of experimental settings over the past years and more recently has been used in clinical applications for the filling of bone defects. its use in human may help to eleminate the morbidity the morbidity of autogenous bone graft donor sites and resorption of autogenous bone graft. We studied the new bone regeneration within a PYROST block which were placed end to end in a subperiosteal pocket on the nasal dorsum of 1.8-2.0 kg New Zealand white rabbit. En block specimen were harvested at 4.8 and 12 weeks post onlay. These were then decalcified. imbedded and sectioned in the sagittal plane. For the confimation of bone regeneration within a PYROST we used weighing the PYROST and light microscope, point-counting technique and fluorescent microscope. The results obtained were summarized as follow : 1. There was no volume change of PYROST 2. The activation of osteoblasts and bone matrix were found of 4 weeks and matrix of lamellar pattern was found 12 weeks post onlay. 3. The quantity of the regenerated new bone within a PYROST was increased at 12 weeks (32 ±2.1%) than 4 weeks(15 ±1.8%).

      • 지속적 외래 복막투석 환자에서 복막염과 출구감염의 임상상 및 위험인자에 관한 임상적 고찰

        지속적 외래복막투석(CAPD)은 전세계적으로 말기 신부전 환자에서 널리 시행되고 있는 신대체 요법으로 지속적으로 증가하는 추세이다. 하지만 복막투석에 대한 이해, 수술 기법의 발달, 연결 기구와 도관에 대한 끊임없는 개발에도 불구하고 복막염과 도관 관련 합병증 등은 아직도 중요한 문제로 남아 있으며 복막투석의 실패 원인의 대부분을 차지하고 있으나 복막염과 출구감염의 위험 인자에 대한 보고는 미흡한 실정이다. 이에 저자들은 1994년 4월부터 1998년 12월까지 동국대학교 경주병원 신장내과에서 CAPD를 시행한 말기 신부전증 환자 51명을 대상으로 복막염과 출구감염의 임상상 및 위험인자에 대한 후향적 조사를 시행하였다. 복막염의 발생빈도는 환자당 0.73회/년 이었고 57.3%에서 원인균이 배양되었으며 동정된 균으로는 Staphylococcus aureus(20.4%), Staphylococcus epidemidis(14.9%), Steptococcus species(11.2%) 순 이었고 전체적으로 그람양성 균주가 68%, 복합 균주가 배양된 경우가 17.1% 였다. 복막염의 치료 결과는 항생제로만 치유된 경우가 83.4%, 도관제거 14.8%, 사망 1.8%였고 외래 치료가 44.4%, 입원하여 치료한 경우가 55.6% 였다. 위험인자에 대한 비교에서는 50세 이하환자에서 0.88 회/년, 51세 이상에서 1.21 회/년으로 고령군에서 더 높았으며(p=0.043) 활동성이 있는 군과 없는 군은 각각 0.78회/년, 1.21 회/년으로 활동성이 없는 군에서 더 높았으며(p=0.044) 직업이 없는 군에서 1.12 회/년, 직업이 있는 군에서 0.93 회/년으로 직없이 없는 군에서 발생빈도가 더 높았다(p=0.041). 또한 당뇨군에서는 연평균 1.22 회/년, 비당뇨군에서는 0.99 회/년으로 당뇨군에서 더 발생빈도가 높았고( p=0.039), 출구 감염이 있는 군에서 1.19 회/년,없는 군에서 9.98 회/년으로 출구감염이 있는 군에서 더 발생빈도가 높았다(p=0.047). 그 외 성별, 학력, 거주지, 투석액의 종류 및 도관 종류에서는 통계적 의의가 없었다. 출구감염은 환자당 0.37 회/년의 발생빈도 및 88.6%의 원인균 배양율을 보였으며, 동정된 균으로는 Staphylococcus aureus(34.3%), Staphylococcus epidermidis(17.1%), Sterptococcus species (8.6%) 순 이었고 전체 63개의 도관중 21개(33.3%) 제거되었으며 가장 많은 원인은 복막염 8예(38.1%) 와 출구감염 6예(28.6%)였다. 출구감염에 영향을 주는 인자에 대한 분석에서는 농촌에 거주하는 경우, 당뇨병이 있는 경우, Tenckhoff 카테터를 사용하는 경우에 다소 빈도가 높은 것으로 나타났으나 통계학적 유의성은 없었고 그 외에 나이, 성별, 활동성, 직업의 유무, 학력과도 연관성이 없었다. 이상의 결과로 CAPD 시행 시 환자선정에 있어 나이, 당뇨병 유무, 거주지, 직업 등의 인자를 특히 고려하여 시행해야 할 것으로 생각된다. Objectives : CAPD has established itself as an effective therapeutic modality as hemodialysis or renal transplantation in the treatment of patients with end stage renal disease. Although much progress of technology of CAPD has been made, peritonitis or catheter-related complications are still important problem until now. Therefore, we investigated about risk factors which are related to peritonitis and exit infection in CAPD patients. Methods : We analysed retrospectively the clinical results of 51 patients on CAPD at Dongguk University Kyoung-ju Hospital from April 1994 to December 1998. Results : 1) There were 31 male and 20 female patients who are 28 to 79 age-old(mean age 55.4±11 years). 2) Underlying disorders of CAPD patients were diabetic nephropathy(33.3%), hypertensive nephrosclerosis(27.5%) and glomerulonephritis(17.7%) 3) The incidence of peritonitis was 0.73/year/patient and peritonitis free interval period was 9.3±1.4 months. The isolation rate of microorganisms from patients with peritonitis was 57.3% and the order of frequency of isolated organims was Staphylococcus aureus, Staphylococcus epiermidis, Steptococcus species, Pseudomonas species and Acinetobacter species. The peritonitis was treated with antibiotics only(83.4%), catheter removal(14.8%) and death(1.8%) 4) The incidence of peritonitis was higher in patients with older age(>50yr), DM, exit site infection and without occupation, physical activity according to univariate analysis (p<0.05). 5) The incidence of exit site infection was 0.37/year/patient and the order of frequency of isolated organisms was Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus species, and Pseudomonas species. The statistical analysis for risk factor of exit site infection had no significant value. The exit site infection was treated with antibiotics only(45.7), external cuff externalization(8.6%), external cuff removal(28.6%), and catheter removal(14.8%). 6) 21 of catheters(33.3%) were removed. The peritonitis and exit site infection or tunnel infection were main causes of catheter removal. The catheter survival rate was 82%, 73%, 68% and 50% at one, two, three and four years, respectively. Mean survival time of all catheters was 39.3±1.4 months. 7) The causes of death were most frequently cardiac disease(28.4%), followed by vascular disease(14.4%), malignancy(14.4%) and infections(14.4%). The patient survival rate was 92%, 82%, 78% and 70% at one, two, three and four years, respectively. Mean survival time of all patients was 61.2±1.8 months. Conclusion : Although CAPD is an effective therapeutic modality in the treatment of patients with end stage renal disease, CAPD peritonitis and exit site infection are the most important pitfalls of CAPD. From the above results, we can know that the risk factors significantly affecting the incidence of peritonitis were old age, no occupation, no physical activity, DM, and exit site infection. Therefore, we most consider the risk factors affecting the incidence of peritonitis and exit site infection when we choose the renal replace therapy in end stage renal failure.

        This study was conducted to investigated the major characteristics of genetically modified rice of "Milyang 204" originated from Dongjinbyeo compared to a non-transgenic rice varieties Dongjinbyeo and Junambyeo. Basta resistant transgenic rice lines carrying bar gene produced by the Yeongnam Agricultural Research Institute were evaluated for their agronomic characters. The transgenic Japonica rice of "Milyang 204" showed inferior phenotypic traits compared to a non-transgenic rice variety Dongjinbyeo and Junambyeo. On the basis of UPOV (Union Internationale Pour la Protaection des Obtentions Vegetables) and NSMO(National Seed Management Office) the transgenic "Milyang 204" showed difference in some traits out of some agronomic traits, such as leaf color, angle of flag leaf, number of spikelets, culm length, white core and white belly compared to the nontransgenic varieties rice.

      • Fe-C-Cr-Mo-V계 공구강의 미세조직과 건식 긁힘 마멸 거동 (마모재 : Al₂O₃)

        High carbon- high chromium tool steels display a number of advantages in abr asive wear applications. They contain moderat e amounts of chromium that produces massive chromium- rich carbides in an austenitic matrix of sufficient hardenability to be transformed to martensit e with relatively simple heat treatment . In this study the role of vanadium- rich carbides with high hardness as well as chromium- rich carbides in improving abrasive wear resistance was investigat ed using a series of Fe- C- Cr -Mo- V system alloys with varying carbide volume fr action. The abr asive wear resistance of the alloys against Al₂O₃ harder than chromium- rich carbides but softer than vanadium- rich carbides was measured with a dry abrasive rubber wheel abr asion test. The abr asive wear resist ance was affected by a volume fraction and variety of carbide because of a differance of hardness between carbide and abrasive. As a result, The specimen with the most volume fraction of vanadium- rich carbide harder than Al2O3 had the mot excellent abrasive wear resistance.

        Many studies revealed the neuroprotective, cardioprotective, and chemopreventive actions of dietary flavonoids. The plausible mechanistic interpretation of the various effects of flavonoids was concentrated on the anti-oxidant or free radical-scavenging properties of these phytochernicals, both in model systems and under in vivo conditions. While there has been a major focus on the anti-oxidant properties. there is an emerging view that flavonoids and their in vivo metabolites. do not act as conventional hydrogen-donating anti-oxidants. but they may exert regulatory functions in cells through actions at protein kinase or lipid kinase signaling pathways. Flavonoids and more recently their metabolites. have been reported to act at phosphoinositide 3-kinase(PI 3-kinase). Akt/protein kinase B(Akt/PKB), protein kinase C (PKC), mitogen activated protein kinase(MAP kinase), and various tyrosine kinases signaling cascades. Inhibitory or stimulatory actions at these pathways are likely to affect cellular function profoundly by altering the phosphorylation state of target molecules and by modulating gene expression. A clear understanding of the mechanisms of action of flavonoids, either as anti-oxidants or modulators of cellular signaling pathways, and the influence of their metabolism on these properties are key to the evaluation of these potent biomolecules as anti-cancer agents, cardio-protectants, and inhibitors of neurodegeneration.

        참치 뼈에서 추출한 hydroxyapatite를 출발물질로 여러 가지 세라믹 복합체를 제조하여 인공체액에서의 hydroxyapatite 복합체간의 화학결합을 조사하였다. hydroxyapatite 복합체들은 인공체액에서 4주 후부터 화학 결합성질을 나타내었다. 결합강도는 bioglass가 가장 강하게 나타났으며, 조성에 따른 의존성은 관찰할 수 없었다. 인공체액에서 복합체들은 이들의 경계면에서 불균일 핵생성 및 성장에 의해 화학적 결합으로 이루어졌다. Chemical bonding was investigated in the simulated body fluid of several selected hydroxyapatite-containing composites. The hydroxyapatite-containing composites chemically bonded with each other in the simulated body fluid after 4 weeks. Bioglass was strongly bonded in the simulated body fluid, but bonding strength was not depended on composition. Their composite bodies were chemically bonded by heterogeneous nucleation and growth at the interfaces of the specimens in the simulated body fluid.

      • 梓木이 腦血流 및 血壓에 미치는 影響

        The purpose of this study is to measure the changes of regional cerebral blood flow (rCBF) and blood pressure (BP) in rats, following the intravenous injection of Catalpa Ovata Water Extract (COWE). The measurement was continually monitored by laser-doppler flowmeter (Transonic Instrument, USA) and pressure tranducer(Grass, USA) in anesthetized adult Sprague-Dawley rats through the data acquisition system composed of MacLab and Macintosh computer. The result of this experiment was as following : COWE increased the changes of rCBF and BP significantly. After pretreatment with propranolol and indomethacin, the changes of rCBF were inhibited significantly, but not by L-NNA and methylene blue. Also, after pretreatment with propranolol, the changes of BP were inhibited significantly, but not by L-NNA, methylene blue and indomethacin. According to above result, it is considered that Catalpa Ovata increased the rCBF and BP, related to adrenergic β-receptor.

        열공형과 비열공형 피질하 혈관성 치매에서 위험인자의 차이에 관한 비교 연구

        Backgrounds and Objectives: Vascular dementia is a group of dementing disoders arising from various stroke syndrome. Among these. subcortical ischemic vascular dementia (SIVD) is regarded as a relatively distinct clinical entity. However, MRI patterns of SIVD are not homogenous. In some patients, lacunes are dominant, and in others, subcortical white matter changes are. This study was designed to compare risk factor profiles between SIVD with and without multiple lacunes. Methods: We divided 47 subjects (22 males, mean age. 68 years) recruited from VADAPET (Multicenter Trial For Evaluation Of The Changes In the PET Images Of Subcortical Vascular Dementia Patient) study into two groups one with more than 5 lacunes in deep gray matter (lacune group) and the other with 5 or less(non-lacune group) Clinical characteristics and laboratory findings of two groups were compared. Results: Nineteen of 47 patients (40%) belonged to the lacune group. The lacune and non-lacune groups d d not differ in the following variables: age, hypertension, diabetes mellitus, hyperlipidemia heart disease, history of stroke or TIA, history of trauma or major surgery, family history of hypertension stroke, or dementia, age at diagnosis of dementia, body mass index, white blood cell count, ESR, CRP, fibrinogen, hemoglobin A1C, total cholesterol. LDL cholesterol creatinine, proteinuria, glucosuria, and microhematuria. However, male sex, smoking alcohol. hemoglobin, and HDL cholesterol were possibly associated more with lacune group SIVD than with non-lacune group (p<0 1) Multivariate analyses revealed that smoking, hemoglobin, and HDL cholesterol were independent predictors of SIVD with multiple lacunes Conclusion: Our study suggests that SIVD with multiple lacunes may be significantly different in smoking habits hemoglobin, and HDL cholesterol from SIVD without multiple lacunes.
