Background: This study was conducted to analyze the current problems in completing death certificates and to identify the correct method for completing death certificates Methods: We reviewed 262 death certificates in three hospitals from March 1 to April 30, 2000, and 119 death certificates in one hospital from March 1 to 31, 2000. We identified major and minor errors and analyzed and compared them retrospectively. Results: A total of 381 death certificates were reviewed: 59 in Seoul National University Hospital,101 in Ewha Woman's University Hospital, and 102 in Gachon Medical College Hospital, which has no education program for completing death certificates in postgraduate training, and 119 in Samsung Medical Center which has an education program for completing death certificates. 358 certificates(94.0%) had at least one error. There were only 23 death certificates(6.0%) without an error. In 182 cases(47.8%), there was one major error. In 321 death certificates(84.3%), there were more than two errors. A comparison of Samsung Medical Center with the other hospitals showed that the number of total errors was statistically different(p=0.001). Conclusion: There were few death certificates without an error in this study. In a hospital which has postgraduate training in completing death certificates, there are fewer errors than in other hospitals which have no training course. Emergency physicians actually certify many deaths, so they must know the correct method of completing death certificates for statistics on morbidity and mortality.

Eighty- six patients with spontaneous pneumothorax were treated surgically at the department of thoracic and cardiovascular surgery Chungnam National University Hospital during period from Oct. 1993 till Dec. 1997. ratio between male and female was 8 : 1, their age ranged from 15 years to 76 years old(average, 26.3). The surgical indications were recurrent pneumothorax, continuous air leakage, bullae on chest x-ray film, previous contralatera pneumothorax and hemopneumothorx. The methods of thoracotomy were axillary approach(41 cases), posterolateral incision(21cases), anterolateral approach and VATS(22 cases). Post operative complications were air leakage, wound infection, bleeding, pleural effusion, brachial plexus injury and ptosis. The brachial injury and ptosis were transient and recovered completely three weeks after surgery. There was no mortality.

This experiment was carried out to enhance plantlet conversion and ex vitro survival of encapsulated somatic embryos of Siberian ginseng. Cotyledonary somatic embryos were encapsulated with 3.0% sodium alginate and 96% of conversion rate in terms of plantlet with well-developed epicotyl marked when the encapsulated embryos were placed on perlite soils wetted with sucrose solution as for carbon source. However, post-germinative growth of encapsulated embryos was suppressed in case of sucrose did not added. Instead of sucrose alone, the addition of both sucrose and starch to the sodium alginate enhanced the post-germinative growth of the embryos. In sodium alginate matrix with 2% sucrose, the survival rate of the encapsulated embryos was more than twice (23.5%) that of ones without sucrose (10.0%). Embryos encapsulated with both 2% sucrose and 1% starch showed the highest percentage (42.1%) of survival rate was shown. In analysis of Iodine staining and starch content in the sodium alginate matrix, the starch component was decomposed when the embryos started to germinte. This result indicated that the carbohydrate treatments (starch and sucrose) in the encapsulation matrix enhanced the survival rate of post-germinative growth of encapsulated embryos in Siberian ginseng.

<P>Whether the metabolic syndrome (MetS) has prognostic value for coronary artery disease (CAD) beyond its individual components is controversial. We compared the relationship between the number of MetS components and CAD severity as assessed by angiography in non-diabetic and diabetic subjects. We consecutively enrolled 527 patients who underwent their first coronary angiography. Patients were divided into four groups according to the number of MetS components: 0/1, 2, 3, and 4/5. A coronary atherosclerosis score was used to quantify the extent of atherosclerotic involvement. The relationship between the MetS score and angiographic CAD severity or clinical presentation was compared between non-diabetic and diabetic subjects. Individuals with the MetS (n = 327) had a higher prevalence of CAD (60% vs 32%, <I>P</I> < 0.001), multi-vessel disease (34% vs 16%, <I>P</I> < 0.001), and acute coronary syndromes (49% vs 26%, <I>P</I> < 0.001) than those without the MetS. In the non-diabetic group, atherosclerosis score increased with the MetS score (1.0 ± 2.1, 2.0 ± 2.9, 2.8 ± 2.9, and 3.6 ± 3.9, <I>P</I> < 0.001) whereas there was no significant difference in the diabetic group (0.5 ± 1.0, 5.2 ± 4.7, 4.2 ± 2.9, and 4.4 ± 3.5, <I>P</I> = 0.102). The MetS score is related to CAD severity in non-diabetic patients but the association between the MetS score and angiographic CAD severity may be obscured in the presence of diabetes.</P>

Glioblastoma (GBM) is a disease without any definite cure. Numerous approaches have been tested in efforts to conquer this brain disease, but patients invariably experience recurrence or develop resistance to treatment. New surgical tools, carefully chosen samples, and experimental methods are enabling discoveries at single-cell resolution. The present article reviews the cell-of-origin of isocitrate dehydrogenase (IDH)-wildtype GBM, beginning with the historical background for focusing on cellular origin and introducing the cancer genesis patterned on firework. The authors also review mutations associated with the senescence process in cells of the subventricular zone (SVZ), and biological validation of somatic mutations in a mouse SVZ model. Understanding GBM would facilitate research on the origin of other cancers and may catalyze the development of new management approaches or treatments against IDH-wildtype GBM.

Rabies is an ancient disease of humans and animals which is still endemic in many parts of the world. Rabies had not been occured for 8 years from 1985 to 1992. But one dog in Kangwon province in 1993 and many dogs and cattle in Kyunggi and Kangwon province in 1994 were identified to be infected with rabies virus. Wild animals especially including raccoons and badgers which are distributed widely in above regions, therefore they were suspected to be the source of rabies. Two wild raccoons were captured from Kyunggi province and examined routinely by rabies diagnosis techniques. They showed all positive results to the rabies test such as histopathological findgings, indirect fluorescent antibody test, mose inoculation test and transmission electron microscopy. In the histopathological examinations Negri body were observed frequently from the neuronal cytoplasm in hippocampus as well as the Purkinje cells in cerebellum. The bullet-shaped rabies viral particles were observed from not only neurons in hippocampus but also acinar cells in mandibular salivary glands. The raccoon was confirmed to be one of the possible source of rabies in Korea.
Tumor Mesenchymal Stem-Like Cell as a Prognostic Marker in Primary Glioblastoma

<P>The isolation from brain tumors of tumor mesenchymal stem-like cells (tMSLCs) suggests that these cells play a role in creating a microenvironment for tumor initiation and progression. The clinical characteristics of patients with primary glioblastoma (pGBM) positive for tMSLCs have not been determined. This study analyzed samples from 82 patients with pGBM who had undergone tumor removal, pathological diagnosis, and isolation of tMSLC from April 2009 to October 2014. Survival, extent of resection, molecular markers, and tMSLC culture results were statistically evaluated. Median overall survival was 18.6 months, 15.0 months in tMSLC-positive patients and 29.5 months in tMSLC-negative patients (<I>P</I> = 0.014). Multivariate cox regression model showed isolation of tMSLC (OR = 2.5, 95% CI = 1.1~5.6, <I>P</I> = 0.021) showed poor outcome while larger extent of resection (OR = 0.5, 95% CI = 0.2~0.8, <I>P</I> = 0.011) has association with better outcome. The presence of tMSLCs isolated from the specimen of pGBM is associated with the survival of patient.</P>
중증 급성췌장염 환자에서 급성신부전증의 발생빈도와 임상경과에 대한 연구

Purpose: Although acute renal failure (ARF) commonly develops in patients with severe acute pancreatitis (SAP), the impact of ARF on disease severity is rarely reported in Korea. This study was performed to compare the clinical findings, morbidity and mortality between SAP patients with and without ARF. Methods: We retrospectively evaluated the medical records of 102 patients with SAP between January 2001 and June 2008 in 3 hospitals. We investigated the incidence and clinical course of ARF in SAP patients. Then, we compared morbidity and mortality between the patients with ARF and normal renal function (NRF). Results: Of the total 102 SAP patients, ARF was observed in 39 patients (38.2%). The peak serum creatinine level in ARF patients was 4.5±2.3 mg/dL. Eight of the 39 ARF patients (20.5%) received hemodialysis and ten patients (25.6%) died. When compared to NRF patiens, ARF patients (n=39) had higher incidence of dyspnea (17.9% vs 3.2%, p=0.011), loss of consciousness (17.9% vs 1.6%, p=0.003), and APACHE II scores more than 8 (92.3% vs 0%, p<0.001). The ARF group had also higher incidences of sepsis (35.9% vs 7.9%, p<0.001), multiorgan failure (15.4% vs 0%, p=0.001), respiratory failure (28.2% vs 4.7%, p=0.001) and mortality (25.6% vs 3.2%, p=0.001). Multivariate analysis demonstrated thrombocytopenia, hemoconcentration, and high LDH as independent risk factors of ARF in SAP patients. Conclusion: The incidence of ARF was high (38.2%) and ARF patients showed higher morbidity and mortality, compared to NRF patients. We suggest that early management of ARF should be performed for reducing the mortality in SAP patients.
