        This study was designed to clarify histological changes in the experimental rabbit ureter after unilateral prartial or complete ureteral obstruction. A total of 20 adult white rabbits were divided into 4 subgroups in the partial obstruction followed by 2 weeks, partial obstruction followed by 4 weeks, complete obstruction followed by 2 weeks, complete obstruction followed by 4 weeks. The normal control group consisted of 5 adult white rabbits. The H & E stain. light microscopy were used. Abnormal histological fingings were showed in ipsilateral ureter of all subgroups; dilatation of ureter, epithelial cell atrophy or loss, interstitial fibrosis, interstitial inflammation. These findings were most severe in the complete-obstructed ureter for 4 weeks. There were no significant changes in contralateral ureter of all subgroups. Despite of progressive histological changes in ipsilateral ureter no abnormal findings were showed in contralateral ureter. We suggest that long term experimental investigation for histological changes in ipsilateral ureter and compensatory changes in contralateral ureter should be done.

        Gospel Insights for Happiness in the Eucharistic Context: An Experiential Approach to the Christian Eucharist

        This thesis aims to provide a method for a Eucharistic theology to support the meaning of happiness in the Gospel context. The Eucharist has been regarded as providing basic insights into the Christian faith. The Eucharist provides a special space where the community prays, discerns and interprets the meaning in the Paschal mystery which eventually invites them to experience happiness. This thesis will provide a holistic and experiential vision of the Eucharist through the New Testaments description. Forgiveness, Healing, Unity of the Community, Intellectual Grasp of the Kingdom of God, Service, and Transcendence. The thesis presumes that forgiveness from and by God is the fundamental human experience of grace which enables humans to grasp the meaning of happiness and salvation. This experience of forgiveness may expand to various dimensions of human experiences: Communal dimension of unity; Intellectual dimension of for grasping the true; Encounter with the transcendence; Healing the wounded memories, and the Spirit of service as the gospel meaning of leadership. This thesis aims to provide some basic biblical references to how these experiences have developed and deepened in the early Christian traditions.

        Clinical Features and Treatment Efficacy in CDKL5 Mutation-Related Epileptic Encephalopathy in the Infant

        Purpose: Mutations in the cyclin-dependent kinase-like 5 (CDKL5) gene are associated with epileptic encephalopathy and severe cognitive impairment. We aim to characterize the association between this gene and treatment efficacy. Methods: We retrospectively analyzed 10 patients who were treated at Severance Children s Hospital for epileptic encephalopathy who were subsequently diagnosed with a CDKL5 mutation using next-generation sequencing. Results: Electroencephalography (EEG) results showed generalized pattern abnormalities in 60% (6/10) of patients with CDKL5 mutations. We analyzed the effects of three treatments, namely antiepileptic drugs (AEDs), ketogenic diet (KD), and steroids. A more than 50% reduction in seizures was observed in 12% (1/8) of patients treated with clobazam. KD treatment proved ineffective in most cases. In addition, a more than 50% reduction in seizures was observed in 57% (4/7) of patients treated with steroids. EEG analysis of patients treated effectively with steroids revealed that 75% (3/4) showed hypsarrhythmia and 25% (1/4) showed focal epileptiform. Conclusion: In this study, as in other studies, AEDs and KD did not effectively control seizures in most patients with a CDKL5 mutation. However, steroid therapy reduced the frequency of seizures in patients who also exhibited hypsarrhythmia. This suggests that steroid treatment is helpful in cases of hypsarrhythmia with CDKL5 mutations.

      • Could Cannabidiol be a Treatment Option for Intractable Childhood and Adolescent Epilepsy?

        <P>Epilepsy is an important disease that affects brain function, particularly in those under 3 years old. Uncontrolled seizures can affect cognitive function and quality of life. For these reasons, many trials have been conducted to investigate treatments for pediatric epilepsy. Currently, many antiepileptic drugs are available for the treatment of epilepsy, but cases of intractable epilepsy continue to exist. In the past, cannabis has been tested as a potential treatment of intractable epilepsy. Since 2013, 10 epilepsy centers in America have conducted research regarding the efficacy of cannabis to treat epilepsy. Cannabis has many components, including cannabidiol (CBD) and Δ<SUP>9</SUP>-tetrahydrocannabinol (THC). THC has psychoactive properties exerted through its binding of the cannabinoid receptor (CBR) whereas CBD is a CBR antagonist. The inhibition of epilepsy by CBD may therefore be caused by various mechanisms, although the detailed mechanisms of CBD actions have not yet been well defined. In most studies, trial doses of CBD were 2–5 mg/kg/day. Several such studies have shown that CBD does have efficacy for treatment of epilepsy. Reported adverse effects of CBD were mostly mild, including drowsiness, diarrhea, and decreased appetite. Severe adverse reactions requiring treatment, such as status epilepticus, have also been reported but it is not clear that this is related to CBD. Furthermore, many previous studies have been limited by an open-label or survey design. In future, double-blind, controlled trials are required and the use of CBD to treat other neurological problems should also be investigated.</P>

        Cannabidiol for Treating Lennox-Gastaut Syndrome and Dravet Syndrome in Korea

        Background: For the first time in Korea, we aimed to study the efficacy and safety of cannabidiol (CBD), which is emerging as a new alternative in treating epileptic encephalopathies. Methods: This study was conducted retrospectively with patients between the ages of 2–18 years diagnosed with Lennox-Gastaut syndrome (LGS) or Dravet syndrome (DS) were enrolled from March to October 2019, who visited outpatient unit at 3 and 6 months to evaluate medication efficacy and safety based on caregiver reporting. Additional evaluations, such as electroencephalogram and blood tests, were conducted at each period also. CBD was administered orally at a starting dose of 5 mg/kg/day, and was maintained at 10 mg/kg/day. Results: We analyzed 34 patients in the LGS group and 10 patients in the DS group between the ages of 1.2–15.8 years. In the 3-month evaluation, the overall reduction of seizure frequency in the LGS group was 52.9% (>50% reduction in 32.3% of the cases), and 29.4% in the 6-month evaluation (more than 50% reduction in 20.6%). In DS group, the reduction of seizure frequency by more than 50% was 30% and 20% in the 3-month and 6-month evaluation, respectively. Good outcomes were defined as the reduction of seizure frequency by more than 50% and similar results were observed in both LGS and DS groups. Adverse events were reported in 36.3% of total patients of which most common adverse events were gastrointestinal problems. However, no life-threatening adverse event was reported in both LGS and DS during the observation period. Conclusion: In this first Korean study, CBD was safe and tolerable for use and could be expected to potentially reduce the seizure frequency in pediatric patients with LGS or DS.

        How the Christian Sacrament of the Anointing of the Sick Can Help to Release Anxiety

        This article introduces the Christian Rite of the Anointing of the Sick and aims to see if this rite can help sick people and their acquaintances to release their anxiety and invite them to experience healing in the context of Christian faith. The second chapter examines the meaning of healing and its connection with anointing. The thesis analyzes how Jesus’ healing ministry was succeeded in the Church as a rite to anoint the sick. The third chapter of this article introduces the contemporary Roman Catholic rite for the Anointing of the Sick as an example of the Church’s healing ministry. This article summarizes several concepts to correctly understand the meaning of healing in the Christian context. ‘Holism’ is the key word to express the Christian understanding of healing. Finally, in the concluding remarks, this article recommends if the rite has any meaning for contemporary society. As the individualism has developed, many sick people or elderly people have separated from the main stream of society and have been marginalized. This article argues that the Christian Rite of Anointing has insights to re-integrate the fragmentized contemporary society.

        Fiscal Sustainability and Its Implication for Fiscal Policy in Korea

        A sharp rise in government debt in Korea has followed over years the Asian financial crisis. This paper aims to assess the fiscal performance and test whether the current level of government debt is sustainable for a severely attacked country, namely, Korea. Under the intertemporal budget constraint model, the study tests for fiscal sustainability and examines whether there was any discernible change in the behaviour of government debt following the crisis. It applies the well-known test procedure by Hamilton and Flavin and Trehan and Walsh to the Korean case under study. In a different context, the study also applies the procedure of Campbell and Shiller to the tests of intertemporal budget balance. Their method also allows us to assess whether there was any discernible change in the behavior of government debt following the crisis. This task may not be accomplished with the use of unit roots or co integration tests because there is only a short sample span since the crisis. The situation is particularly acute for Korea in which only a yearly data are readily available. Empirical analysis indicates that the levels of government debt are not sustainable in Korea. It also shows that the crisis contributes significantly to push the government debt in excess of its sustainable level. This draws a particular policy attention for fiscal consolidation.

        Treatment Response to Acquired Aphasia with Seizures and Prognosis through Electroencephalogram on Cognitive Function

        Purpose: Acquired epileptic aphasia (AEA) accompanied by electroencephalogram (EEG) abnormality is a rare disease; therefore, there are few studies investigating the prognostic factors and treatment efficacy. We aimed to determine the therapeutic effects and prognostic factors for clinical seizure and neuropsychological function in acquired aphasia patients. Methods: We retrospectively studied cases of AEA diagnosed at Severance Children’s Hospital from January 2013 to October 2017. We evaluated the efficacy of antiepileptic drugs, steroids, and ketogenic diets (KD) in treating acquired aphasia. The EEG patterns and prognostic factors were predicted by the background EEG and frequency of spike and wave during sleep (SWS). Results: The study analyzed 20 patients, 11 male and 9 female, with AEA. Aphasia most commonly occurred at 4 years of age, and clinical seizure was most likely to occur between 2 and 4 years of age and focal seizures were the most common seizure type. KD was shown to be the best treatment for clinical seizure in AEA patients. Patients with normal EEG background showed better responses to clinical seizure treatment and improvements in neuropsychological function. Conclusion: KD and steroids generate the best therapeutic effects for clinical seizure in AEA patients. Improvements in neuropsychological function in AEA patients may be related to the EEG background and the SWS patterns. Additionally, the results suggest that the response of clinical seizure to antiepileptic drugs may also be related to the EEG background. However, the current study had some limitations and further research is needed.
