        관절가동술이 만성 뇌졸중 환자 발목관절의 즉각적인 기능에 미치는 영향: 관절가동범위, 족저압과 보행능력

        본 연구는 만성 뇌졸중 환자의 마비측 발목관절에 관절가동술을 적용하여 즉각적인 관절가동범위와 족저압과 보행에 미치는 영향에 대해 알아보고자 실시하였다. 대상자는 선정기준에 맞는 만성 뇌졸중 환자 30명을 무작위 배정하여 관절가동술군 15명, 관절가동범위운동군 15명으로 배정하였다. 대상자들은 중재 전 관절가동범위, 족저압, 10m 걷기 검사를 측정하고 각 군별 15분간 관절가동술과 관절가동범위운동을 적용 후 재측정하였다. 그 결과, 관절가동술군이 중재 전후 발등굽힘에서 유의한 차이가 있었고(p <.05), 정적 족저압에서는 정적 마비측 발 표면적에서 유의한 차이를 보였다(p <.05). 하지만 정적 마비측 발표면적과 동적 마비측 발표면적에서 유의한 차이를을 보이지 않았다. 또한 보행능력에서 중재 전후 10m 걷기 검사에서 유의한 차이를 보이지 않았다(p> .05). 결론적으로 관절가동술군은 1회의 치료 후 발등굽힘 가동범위와 정적 마비측 발표면적에서 즉각적으로 유의한 차이가 있었으며, 이러한 결과는 만성 뇌졸중 환자의 발목에 관절가동술을 적용하는 것은 발목의 즉각적인 발등굽힘 가동범위를 증가시키고, 정적 마비측 발표면적을 개선하는 데 도움이 될 수 있음을 확인하였다. The purpose of this study was to investigate immediate range of joint movement, plantar pressure, and gait after applying joint mobilization to paraplegic ankle joints in chronic stroke patients. Thirty chronic stroke patients who met the selection criteria were assigned equally to a joint mobilization group or a passive range of motion group. Subjects underwent joint range of motion and plantar pressure testing and a 10m walking test before intervention, and these tests were repeated after 15 minutes of joint mobilization. A significant difference in dorsiflexion was observed after intervention (p <.05). Plantar pressures were significantly different in static affected side foot surface after intervention(p <.05) but not significantly different in static affected side foot surface or dynamic affected side foot surface after intervention (p >.05). Ten-meter walking test results were not significantly different after intervention (p >.05). In conclusion, a significant difference in dorsiflexion and static affected side foot surface of the group of joint mobilization groups, These results indicate that the application of joint mobilization to the ankles of chronic stroke patients may help increase immediate the range of ankle motion and improve foot surface.

        Neuromuscular 트레이닝이 여자육상 투척선수의 동적 균형능력과 하지 관절 움직임에 미치는 영향

        This study was to investigate the effects of neuromuscular training on dynamic balance ability and joint motions in the throwers. The participants were divided into two groups: exercise group (n=14) and non exercise group (n=14). The neuromuscular training program for 12 weeks. The results of this study were as follows: First, As for the dynamic balance abilities, there were significantly increased in Up, Down, Left, Right in the exercise group. Second, As for the joint motions abilities, there were significantly increased in Overlap Comparison, left ankle ROM, right ankle ROM, left knee ROM, right nnee ROM in the exercise group. As conclusions, this study confirmed that the neuromuscular training could improve the dynamic balance ability and joint motions abilities in the throwers. However, there is a need for more specific exercise programs to be developed for the continual improvement of athletic performance in athletics along with further studies to confirm the physiological benefits of those programs.
