      • 가미태음조위탕 복용 후 감량성공군과 감량실패군의 초기 특성 비교 : 과체중 및 비만자를 대상으로

        Objectives The purpose of this study was to compare the initial characteristics of successful and failed weight loss groups after taking Gamitaeeumjowi-tang. Methods From January, 2017 to May, 2019, the subjects were 14,548 overweight and obese people (body mass index [BMI]≥23 kg/m2) who had taken Gamitaeeumjowi-tang for more than 1 month. After about 1 month, after setting the top 25% or more of the weight loss as a successful weight loss group and less than the bottom 25% as a weight loss failure group, the continuous variables among the initial characteristics were compared and analyzed by independent t-test and categorical variables by χ2-test. Results In the successful group, initial BMI, initial weight, skeletal muscle mass, male ratio, binge eating, and nighttime eating were significantly higher. In the failed group, age, light eating, diet drug experience over 3 months, hypothyroidism, and gastritis were significantly higher. Conclusions In this study, we found out the differences of the several initial characteristics between successful and failed weight loss group after taking Gamitaeeumjowi-tang, and these results may be useful as a reference for providing prognosis of weight loss.

        『의휘(宜彙)』의 침구법 연구

        Objectives “Uihwi” is a book of Korean medicine written under the pseudonym of Geumlisanin in the late Joseon Dynasty. At that time, it was published by collecting famous prescriptions of experience across the country, and it can be said to be an important data in that it gives a glimpse of oriental medicine at the end of the Joseon Dynasty, but there are not many studies on “Uihwi”. This paper attempted to examine the characteristics of oriental medicine at the end of the Joseon Dynasty. Methods Using the text of “Uihwi” as the subject of research, comparing to other medical texts, and to examine the position of acupuncture in it. Results Thus, the following conclusions were obtained. “Uihwi” was mainly edited by citing medical books published in Joseon, and is also a comprehensive medical book that combines the experiences of himself and other medical professionals. In addition, it can be seen that medicine at that time was very practical by presenting uncomplicated prescriptions. In the case of acupuncture, an acupuncture method that synthesizes the clinical achievements of Joseon was presented. Conclusions The practicality was emphasized by reducing the theoretical explanation, and based on this practicality, efforts were made to aggregate medicine in the late Joseon Dynasty.

        With aging, skeletal muscle mass and its function decrease, and the changes lead to increased risk of falls, fractures, long-term care institutionalization, mortality, and even cardiovascular and metabolic diseases. Sarcopenia has been defined as low muscle mass together with low muscle strength and/or low physical performance. In 2019, Asian Working Group for Sarcopenia(AWGS) published a consensus paper about diagnosis and management of sarcopenia. The AWGS 2019 proposed algorithm starting with measuring SARC-F questionnaire or calf circumference (cut-off, <34 cm in men, <33 cm in women) and then measuring handgrip strength (cut-off, <28 kg in men, <18 kg women) or 5-time chair stand test (≥12 sec), and then ‘possible sarcopenia’ can be defined. Low appendicular skeletal muscle mass and either low muscle strength (handgrip strength) or low physical performance (gait speed or 5-time chair stand test) is used to diagnose sarcopenia. For management of sarcopenia, high protein nutrition and resistance exercise are highly recommended, and vitamin D may increase muscle strength, particularly in vitamin D deficiency.

        기공의 한의학적 이해와 활용

        Objectives This study aims to provide information about history, theories about postures, and utilization of Qigong in Korean Medicine. Methods Qigong and Daoyin need distinction in aspects of both in historical and medical pratice in Korean Medicine. Therefore, we should distinguish the history of Qigong and Daoyin, and learn clinical use of Qigong. Results The modern Qigong has started in early 1900s. Qigong could be utilized in various disease theoretically, whereas clinical study of Qigong focuses on musculoskeletal disease. Patients who are new to Qigong need education with adequate intensity and level of difficulty. Conclusions Qigong will be one of the mainstream medicine if Qi is measured in using scientific method.

        비만치료에서 수면장애, 불면증의 관리

        Department of Neuropsychiatry, College of Korean Medicine, Kyung Hee University Obesity and sleep may differ in terms of causes and effects, but they are highly related to each other, so sleep deprivation causes obesity, and obesity causes insomnia and other sleep disorders. Just as obesity management is necessary for the treatment of insomnia, sleep management is necessary in the treatment of obesity. In general sleep hygiene, circadian rhythm should be managed by activating daily life during the daytime, and in the clinical field of Korean Medicine, sleep management should be performed to help treat obesity by considering the treatment method of Korean Medicine. In addition, the choice of herbal medicine requires a prescription that considers both obesity and sleep.

        노화의 개념, 진단 및 관리

        Anti-aging medicine is defined as ‘the newest clinical specialty, the optimum of wellness and longevity, and employs extensive therapies and treatment in the preventive health care field far beyond just cholesterol testing and mammogram, by American Academy of Anti-aging Medicine. Aging theories is dived into two main theories, genetic control theory and acquired theory. Though many aging theories have been developed, any single theory is not universally accepted. Aging is defined as a time-related decline in physiological functions and changes in morphology. Aging is usually evaluated by questionnaires (AGHDA), self-test, aging biomarkers tests, and biological age measurements. Biological age measurement is usually considered as the index to evaluate the overall health status and aging degree in clinical practice. The goal of aging management is not only to treat age-related diseases, but to improve the diverse function of body and improve quality of life. Five major methods for aging management are the elimination of risk factors, management of chronic diseases, caloric restriction with nutritional balance, medium-intensity regular exercise, and stress management. Many other methods, such as, anti-oxidant therapy, hormone replacement, smart drugs, immune boosters, detoxification, and alternative medicine have been tried for aging management.

        한의학에서 딥러닝의 뜻밖의 역할: 딥러닝의 과학으로 한의학 이해하기

        Deep learning is revolutionizing in many scientific fields today. When it comes to Traditional Korean Medicine(TKM), it is commonly expected that deep learning will be able to assist TKM doctors to diagnose and prescribe treatments. We believe, however, there is another way to revolutionize TKM using deep learning. Mathematically, deep learning is a universal approximation function and can be a powerful model that explains cognitive processes in the brain. Since all of the decision-making processes in TKM are cognitive processes in the TKM doctors’ brain, they could also be modeled and explained using deep learning framework. As the science of deep learning advances, we will be able to better understand TKM through deep learning framework.

        현대 생체시계 연구를 바탕으로 재조명한 <황제내경 소문 사기조신대론>의 계절에 따른 생활양식

        Objectives In <Hwangjenaegyeong Somun Sagijosindaelon>, the climate change according to the four seasons is explained, and how to manage health by adjusting the lifestyle is recorded. Since the concepts of yin-yang and the four seasons are being re-examined in modern times with the discovery of biological clock genes, we attempted to scientifically reinterpret the classic text. Methods We reinterpreted the classic text and used keywords such as “circadian clock,” “circadian gene,” “seasonal variation,” “metabolism,” “environment,” and “climate” to search PubMed and Google Scholar for relevant articles. References in relevant articles were also considered. Results In mammals, the suprachiasmatic nucleus and the autonomic nervous system regulate seasonal variation in physiological responses in response to changes in the length of the external light cycle. Since most people now live in indoor environments with air conditioning and lighting, seasonal circadian rhythms have decreased in comparison to 100 years ago. Disrupting the circadian rhythm is equivalent to attacking the body from within by disrupting the order in the body. To restore and fine-tune the circadian rhythm and prevent serious diseases before the body's dysfunction becomes chronic, healthy eating, proper sleep, and adequate physical activity are required. Conclusions With the recent focus on global warming and environmental pollution, interest in climate change-related health issues is growing. Global cooperation will be required, and the effects of climate change on ecosystems and environments, as well as their impact on people's health, must be continuously monitored to protect the global environment and public health.

        정상 체중 비만 성인의 체지방 감량을 위한 한약 처방의 활용: 후향적 차트 리뷰

        Objectives Normal weight obesity (NWO) is defined as a normal body weight according to Body Mass Index (BMI) but with a high body fat percentage. NWO is associated with metabolic abnormalities and cardiovascular risk factors, thereby emphasizing the importance of body fat loss. The objective of this study is to evaluate changes in weight and body fat with NWO who were prescribed Korean herbal medicine. Methods A retrospective chart review was conducted for subjects with NWO, within the normal weight range but with over 30% body fat percentage. These subjects were prescribed herbal medicine for a 12-week between March 2021 and May 2023. Weight (kg), body fat mass (kg), body fat percentage (%) and % Fat Free Mass (FFM) loss/weight loss (%FFML) were assessed before and after the period. Reported adverse events were evaluated for severity, causality, and system-organ classes. Results A total of 42 subjects were included (mean±SD, weight 57.38±3.45 kg, body fat percentage 33.65±2.12%). The average weight loss rate was 6.99±3.22% with a reduction of 4.08±1.95 kg, while the body fat loss rate averaged 18.00± 0.08% with a decrease of 3.54±1.73 kg. The average of %FFML was 21.89±0.16%. Most adverse events reported were mild (90.9%) in severity, and the likelihood of events being 'Unlikely' increased from 48.8% to 64.3% as the program progressed. Conclusions The administration of Korean herbal medicine can be considered an appropriate method to alleviate normal weight obesity, without causing serious adverse reactions. Further well-designed controlled trials and clinical studies with the analysis of hematological indicators are needed to validate its efficacy.

        안마의자를 활용하는 마사지가 성인 남녀의 자율신경균형도에 미치는 즉각적 영향: 전향적 관찰연구

        Objectives The aim of this study was to examine the immediate impact of a massage chair intervention on homeostasis and the balance of the autonomic nervous system in individuals aged 20 to 64. Methods This study was a single-center, prospective, single-arm, investigator-initiated observational study. Non-underweight adults without significant health concerns underwent a 40-minute massage chair intervention. Measurements of heart rate variability, functional near-infrared spectroscopy, and vital signs were conducted before and after the intervention. Additionally, participants completed the Mibyeong questionnaire and a satisfaction survey. Results A 40-minute massage chair session led to a notable decrease in average heart rate and low frequency to high frequency (LF/HF) ratio, coupled with an increase in root mean square of the successive differences (RMSSD) and standard deviation of NN interval (SDNN). No adverse reactions were noted. Subjects with elevated sympathetic nerve activity exhibited a reduction in LF/HF ratio, whereas those with heightened parasympathetic activity demonstrated an increase in LF/HF ratio post-intervention. Conclusions The findings indicate that the massage chair intervention has the potential to activate the parasympathetic nervous system, promote blood pressure homeostasis and autonomic nervous system balance. However, further research over a more extended period is crucial to validate these conclusions.
