      • Prevalence of Local Recurrence of Colorectal Cancer at the Iranian Cancer Institute

        Background: Although a great deal of progress has been made in the management of colorectal cancer in terms of neoadjuvant modalities, surgical techniques and adjuvant therapies, the recurrence of tumors remains an enigmatic complication in patients. A better understanding of colorectal cancer and of factors that lead to recurrence of disease can provide helpful information for designing more effective screening and surveillance methods. Aim: To investigate the factors that may lead to local recurrence of colorectal cancers. Materials and Methods: The current retrospective case study evaluated 617 patients admitted to the Iranian Cancer Institute (the largest referral cancer center in the country) from 1995 to 2009 with confirmed colorectal cancer. Patients with distant metastasis, or with pathology other than adenocarcinoma and no follow-up, were excluded (175 patients). The remainder (442) included 294 (66.5%) with rectal cancer and 148 (33.5%) with colon cancer. The median duration of follow-up was 26 months. Results: The total rate of recurrence was 17.4%, comprising 19.6% and 16.3% recurrence rates in colon and rectal cancer, respectively. Conclusions: Recurrence of colorectal cancer was significantly correlated to tumor grade (p<0.008).

      • Anatomical Distribution of Colorectal Carcinoma in Iran: A Retrospective 15-yr Study to Evaluate Rightward Shift

        Background: Although more than two third of colorectal cancers are localized on the left side, recent studies suggest a right ward shift in anatomical distribution with increase in proximal colon cancers. The aim of the present study was to determine the anatomical distribution of colorectal cancer in a referral center over a 15 year period. Method: Records of patients who underwent colectomy in the Cancer Institute of Iran from 1994 to 2009 were retrieved. Data including anatomical localization, year of diagnosis, patient age and gender, tumor histology and differentiation, and disease stage were extracted. Tumors located from the cecum to the distal transverse colon were classified as right side and those occurring from the splenic flexure to the descending colon as left-sided. Cancer of rectum and recto-sigmoid junction were considered as rectal cancers. Results: A total of 442 patients including 220 (49/8%) men and 222 (50/2%) women with mean age 53 were included. Most patients were in stage II &III (47.1% and 33% respectively). There were 157 (35.5 %) colon cancers and 285 (64.5%) rectal cancers. 43.3% of the colon cancers were right sided and 56.7% were left sided. There was no statistically significant increase in right sided cancer during the period of the study. There were no significant differences in age at diagnosis, gender, grade and stage of tumor between the right and the left sided cancers. Conclusion: No proximal shift over time was identified in our study.

      • Epidemiology of Female Reproductive Cancers in Iran: Results of the Gholestan Population-Based Cancer Registry

        Background: Malignancies of the female reproductive tract are estimated to be the third most common group of cancers in women. Objectives: We here aimed to present their epidemiological features in Golestan province located in Northeast of Iran. Materials and Methods: Data on primary female reproductive cancers diagnosed between 2004-2010 were obtained from Golestan Population-based Cancer Registry (GPCR). CanReg-4 and SPSS software were used for data entry and analysis. Age standardized incidence rates (ASR) (per 100,000 person-years) were calculated using the world standard population. Poisson regression analysis was used to compare incidence rates. P-values of less than 0.05 were considered as significant. Results: A total of 6,064 cancer cases were registered in Golestan females in the GPCR during 2004-2010, of which 652 cases (11%) were female reproductive cancers. Cancers of the ovary (ASR=6.03) and cervix (ASR=4.97) were the most common. We found significant higher rates in females living in cities than in villages. Our results showed a rapid increase in age specific incidence rates of female reproductive cancers at the age of 30 years. Conclusions: We found significant higher rates of female reproductive cancers among residents of cities than villages. Differences in the prevalence of risk factors including reproductive behavior between the two populations may partly explain such diversity. Our results also showed a rapid increase in incidence rates of these cancers in young age females. Further studies are warranted to determine risk factors of female reproductive cancers in our population.

        Incorporation of Oxidized Pectin to Reinforce Collagen/Konjac Glucomannan Hydrogels Designed for Tissue Engineering Applications

        Injectable hydrogels have been widely studied for the use as scaffolds or as carriers of therapeutic agents including proteins, cells, drugs, and bioactive molecules in the disease treatment and the regeneration and repair of tissues. Collagenbased hydrogels and pectin that mimic the extracellular matrix are generally applied to maintain the balance between physical and biofunctional features. In this study, preparation and characterization of novel hydrogels based on oxidized-konjac glucomannan (OKG), collagen, and oxidized-pectin (OP) have been successfully developed. Fabricated hydrogels were evaluated for their morphological, degradation, thermal stability, and mechanical properties. The physicochemical properties of OKG/ collagen hydrogels without pectin (HG) or with pectin (HGP) and oxidized-pectin (HGOP) were studied. The results presented that with the addition of the OP content, the hydrogels degradation behavior and mechanical properties were improved. In addition, HGOP hydrogels enhanced the cell proliferation without any cytotoxic effect on mouse embryonic fibroblast (NIH-3T3) cells. These achieved results suggested that the developed hydrogel could be a suitable candidate to be applied for tissue engineering scaffolds.

      • Machine learning-based techniques to facilitate the production of stone nano powder-reinforced manufactured-sand concrete

        This study aims to examine four machine learning (ML)-based models for their potential to estimate the splitting tensile strength (STS) of manufactured sand concrete (MSC). The ML models were trained and tested based on 310 experimental data points. Stone nanopowder content (SNPC), curing age (CA), and water-to-cement (W/C) ratio were also studied for their impacts on the STS of MSC. According to the results, the support vector regression (SVR) model had the highest correlation with experimental data. Still, all of the optimized ML models showed promise in estimating the STS of MSC. Both ML and laboratory results showed that MSC with 10% SNPC improved the STS of MSC.

      • Prognostic Value of Lymph Node Ratios in Node Positive Rectal Cancer Treated with Preoperative Chemoradiation

        Background: To investigate the impact of the lymph node ratio (LNR) on the prognosis of patients with locally advanced rectal cancer undergoing pre-operative chemoradiation. Methods: Clinicopathologic and follow up data of 128 patients with stage III rectal cancer who underwent curative resection from 1996 to 2007 were reviewed. The patients were divided into two groups according to the lymph node ratio: LNR ${\leq}$ 0.2 (n=28), and >0.2 (n=100). Kaplan-Meier and the Cox proportional hazard regression models were used to evaluate the prognostic effects according to LNR. Results: Median numbers of lymph nodes examined and lymph nodes involved by tumour were 10.3 (range 2-28) and 5.8 (range 1-25), respectively, and the median LNR was 0.5 (range, 0-1.6). The 5-year survival rate significantly differed by LNR (${\leq}$ 0.2, 69%; >0.2, 19%; Log-rank p value < 0.001). LNR was also a significant prognostic factor of survival adjusted for age, sex, post-operative chemotherapy, total number of examined lymph nodes, metastasis and local recurrence (${\leq}$ 0.2, HR=1; >0.2, HR=4.8, 95%CI=2.1-11.1) and a significant predictor of local recurrence and distant metastasis during follow-up independently of total number of examined lymph node. Conclusions: Total number of examined lymph nodes and LNR were significant prognostic factors for survival in patients with stage III rectal cancer undergoing pre-operative chemoradiotherapy.

      • Reinforced concrete structures with damped seismic buckling-restrained bracing optimization using multi-objective evolutionary niching ChOA

        The paper contrasts conventional seismic design with a design that incorporates buckling-restrained bracing in three-dimensional reinforced concrete buildings (BRBs). The suboptimal structures may be found using the multi-objective chimp optimization algorithm (MEN-ChOA). Given the constraints and dimensions, ChOA suffers from a slow convergence rate and tends to become stuck in local minima. Therefore, the ChOA is improved by niching and evolutionary operators to overcome the aforementioned problems. In addition, a new technique is presented to compute seismic and dead loads that include all of a structure's parts in an algorithm for three-dimensional frame design rather than only using structural elements. The performance of the constructed multi-objective model is evaluated using 12 standard multi-objective benchmarks proposed in IEEE congress on evolutionary computation. Second, MEN-ChOA is employed in constructing several reinforced concrete structures by the Mexico City building code. The variety of Pareto optimum fronts of these criteria enables a thorough performance examination of the MEN-ChOA. The results also reveal that BRB frames with comparable structural performance to conventional moment-resistant reinforced concrete framed buildings are more cost-effective when reinforced concrete building height rises. Structural performance and building cost may improve by using a nature-inspired strategy based on MENChOA in structural design work.

      • Applicability of Radioguided Occult Lesion Localization for Non-Palpable Benign Breast Lesions, Comparison with Wire Localization, a Clinical Trial

        Background: This study was designed to compare radioguided versus routine wire localization of nonpalpable non-malignant breast lesions in terms of efficacy for complete excision, ease of use, time saving, and cosmetic outcome. Materials and Methods: Patients with non-palpable breast masses and non-malignant core biopsy results who were candidates for complete surgical lumpectomy were enrolled and randomly assigned to radioguided or wire localization groups. Radiologic, surgical, and pathologic data were collected and analyzed to determine the difficulty and duration of each procedure, ease of use, accuracy, and cosmetic outcomes. Results: This prospective randomized study included 60 patients, randomly divided into wire guided localization (WGL) or radioguided occult lesion localization (ROLL) groups. The mean duration of localization under ultrasound guidance was shorter in the ROLL group (14.4 min) than in the WGL group (16.5 min) (p<0.001). The ROLL method was significantly easier for radiologists (p=0.0001). The mean duration of the surgical procedure was 22.6 min (${\pm}10.3min$) for ROLL and 23.6 min (${\pm}9.6min$) for WGL (p=0.6), a non-significant difference. Radiography of the surgical specimens showed 100% lesion excision with clear margins, as proved by pathologic examination, with both techniques. The surgical specimens were slightly heavier in the ROLL group, but the difference was not significant (p=0.06). Conclusions: The ROLL technique provides effective, fast, and simple localization and excision of non-palpable non-malignant breast lesions.

      • Predicting the splitting tensile strength of manufactured-sand concrete containing stone nano-powder through advanced machine learning techniques

        The extensive utilization of concrete has given rise to environmental concerns, specifically concerning the depletion of river sand. To address this issue, waste deposits can provide manufactured-sand (MS) as a substitute for river sand. The objective of this study is to explore the application of machine learning techniques to facilitate the production of manufactured-sand concrete (MSC) containing stone nano-powder through estimating the splitting tensile strength (STS) containing compressive strength of cement (CSC), tensile strength of cement (TSC), curing age (CA), maximum size of the crushed stone (Dmax), stone nano-powder content (SNC), fineness modulus of sand (FMS), water to cement ratio (W/C), sand ratio (SR), and slump (S). To achieve this goal, a total of 310 data points, encompassing nine influential factors affecting the mechanical properties of MSC, are collected through laboratory tests. Subsequently, the gathered dataset is divided into two subsets, one for training and the other for testing; comprising 90% (280 samples) and 10% (30 samples) of the total data, respectively. By employing the generated dataset, novel models were developed for evaluating the STS of MSC in relation to the nine input features. The analysis results revealed significant correlations between the CSC and the curing age CA with STS. Moreover, when delving into sensitivity analysis using an empirical model, it becomes apparent that parameters such as the FMS and the W/C exert minimal influence on the STS. We employed various loss functions to gauge the effectiveness and precision of our methodologies. Impressively, the outcomes of our devised models exhibited commendable accuracy and reliability, with all models displaying an R-squared value surpassing 0.75 and loss function values approaching insignificance. To further refine the estimation of STS for engineering endeavors, we also developed a user-friendly graphical interface for our machine learning models. These proposed models present a practical alternative to laborious, expensive, and complex laboratory techniques, thereby simplifying the production of mortar specimens.
