        특발성 척추측만증 학생의 정적균형, 체간 근력 및 유연성에 관한 연구

        The purpose of this study was to find the difference on static balance(stability), muscular strength(flexion and extension) and flexibility of trunk in adolescent idiopathic scoliosis through comparing normal students. Fifty-one adolescent subject were participated in this study(scoliosis group; twenty-five, normal group; twenty-six). Data were analyzed with independent t-test using SPSS program. The results showed that there was no significant difference in static balance ability(stability) and also in flexor(60º/sec, 120º/sec), extensor(60º/sec) and flexibility(flexion, extension) of trunk between the two groups. But, there are statistically significant difference in trunk extensor power at 120º/sec. From this result, we have recognized the characteristics of adolescent idiopathic scoliosis patients, and we believe that these findings may lead us to treat the scoliosis patients in a way we have not tried before

        회전근개 전층파열 환자의 등속성 외전 운동 시 각속도에 따른 견관절 주위근 활성도 변화

        This study investigated the differential muscular firing patterns of the shoulder muscles in patients with full-thickness rotator cuff tear. Eleven patients with unilateral rotator cuff tear were recruited. During isokinetic exercises, the muscular activities of upper trapezius, serratus anterior and lateral deltoid were recorded by surface EMG. Subjects performed isokinetic shoulder abduction of 60°/sec, and 180°/sec. The mean EMG amplitude of root mean square (RMS) was calculated to a percentage of maximal activity for each muscle. In the rotator cuff tear sides, the muscular activities of lateral deltoid in abduction were increased in 180°/sec compared to 60°/sec. And the EMG activities of upper trapezius and serratus anterior in the lesion side were significantly decreased compared to sound side in abduction of 180°/sec. But, in the case of lateral deltoid, the EMG activities were not different between sound and lesion sides in abduction of 180°/sec.

        급성 아킬레스건 파열의 봉합술 후 조기 체중 부하 및 족관절 관절 운동의 임상적 결과 및 근력 평가

        목적: 본 연구는 급성 아킬레스건 파열의 수술 후 조기 재활치료를 시행한 환자의 결과를 알아보고 조기 재활치료의 효과 및 기능 호전 정도를 평가하고자 하였다. 대상 및 방법: 2007년 3월부터 2009년 8월까지 아킬레스건 파열로 본원을 방문하였던 52예 중 수술 후 12개월 이상 추시가 가능하였던 11예를 대상으로 하였다. 최종 추시 기간은 평균 18.2개월이었다. 환자는 수술 후 조기 전 체중 부하 및 관절 운동을 시행하였다. 최종 추시에서 임상적 결과를 평가하였고 근력 측정기를 이용하여 족저굴곡근의 등속성 및 등척성 최대 우력을 측정하였으며, 근지구력을 측정하여 결과를 평가하였다. 결과: 임상적 결과는 Arner-Lindholm 평가 결과에서 우수 4예, 양호 7예였으며 AOFAS 족관절-후족부 척도는 평균 88.9점이었다. 최종 추시 시 검사한 족저굴곡근의 등속성 근력 검사에서 최대 우력은 각속도에 따라 정상측의 평균 92.2% (30°/sec), 97.0% (120°/sec)로 확인되었다. 등척성 근력 검사상 최대 우력은 측정 시 족관절의 굴곡자세에 따라 정상측의 평균 89.4% (족배굴곡 10도), 84.4% (중립 자세), 84.0% (족저굴곡 20도)로 회복되었다. 근지구력 검사 결과는 정상측 62.4%, 환측 59.2%로 정상측과 아킬레스건 파열측 간의 족저굴곡근의 지구력 차이를 보이지 않았다(p=0.79). 결론: 급성 아킬레스건 파열의 수술적 봉합 후 조기 체중 부하 및 빠른 족관절 운동을 시행한 치료 방법은 평균 18개월 추시상 만족할 만한 근력 및 근지구력의 회복을 보였으며 우수한 임상적 결과를 보였다. Purpose: The purpose of this study was to evaluate the clinical results of early functional treatment after surgical repair of acute Achilles tendon rupture and to evaluate the isokinetic and isometric concentric plantar flexion peak torque and muscle endurance. We wanted to provide objective results of the functional improvement and the effect of early rehabilitation. Materials and Methods: On a retrospective basis, we studied 52 cases of acute Achilles tendon rupture who visited our clinic between March 2007 and August 2009. Eleven patients (9 male, 2 female) were available for the follow-up more than 12 months and their mean final follow-up duration was 18.2 (12 to 39) months. We performed early weight-bearing and ankle exercise after surgical repair of acute Achilles tendon rupture. At final follow-up, patients were evaluated with clinical and functional examination using Amer-Lindholm scale and American Foot and Ankle Society (AOFAS) Ankle-Hindfoot scale and patients" subjective satisfaction. In addition, the isokinetic and isometric concentric plantar flexion peak torque and muscle endurance were evaluated. Results: Evaluating the clinical results using the Amer-Lindholm scale, we had 4 excellent cases and 7 good cases. The average AOFAS score was 88.9 (68 to 100) points at final follow-up. The patients" subjective satisfaction was excellent in 5 cases and good in 6 cases. The isokinetic concentric plantar flexion peak torque was restored to 92.2% (30°/sec) and 97.0% (120°/sec) in relation to the intact side at final follow-up. The isometric concentric plantar flexion peak torque was restored to 89.4% at 10° dorsiflexion, 84.4% at neutral, and 84.0% at 20° plantar flexion of the ankle position in relation to the intact side. The muscle endurance of ankle plantar flexor was 62.37% for the intact side and 59.16% for the injured side that there was no difference between the intact and injured side (p=0.79). Conclusion: The clinical results and the satisfactory restoration of muscle power and endurance support early full weight bearing and exercise as an acceptable form of rehabilitation.
