이 글에서는 지방의료원이 지역 공공병원으로서 기능과 역할을 충실히 수행하는 데 필요한 지방의료원의 특성에 적합한 지불보상체계와 재정 지원 방안을 제시하고자 한다. 이를 위해 관련 법 및 정부 계획에 제시된 지방의료원의 기능과 역할을 고찰하고, 지방의료원의 현황 및 문제점을 살펴보았다. 지방의료원 지불보상체계 및 재정 지원 개선을 위해 고려할 주요 사항은 다음과 같다. 우선, 회계연도별 적자가 발생하지 않는 지불보상체계 및 재정 지원 방안을 마련해야 한다. 또한, 필수의료 부문에 대해 시설, 장비 확충을 위한 자본 비용뿐만 아니라 의사, 간호사 등 필요 인력에 대한 인건비 등의 경상비를 지원할 수 있도록 재정 운용 원칙을 개선해야 하며, 지방의료원 지불보상체계 및 재정 지원 방안을 마련할 때 지역별, 기관별 상황을 고려해야 한다. This paper aimed to propose reimbursement schemes and financial support plans to provide local public hospitals with sufficient compensation for appropriate delivery of health care services and population health projects so that they can fulfill their missions as local public hospitals. I reviewed major roles, current status, and characteristics of local public hospitals as well as the reimbursement and financial support system for local public hospitals in Korea. My suggestions to improve reimbursement and financial support system for public hospitals in Korea include 1) developing and implementing customized models not only to cope with needs in communities but also to consider the current status and characteristics of local public hospitals, 2) securing sufficient compensation for essential healthcare services as well as population health projects, 3) expanding financial support to fund operating costs and labor costs for local public hospitals.

헬스 리터러시는 자가 건강관리, 의료이용에 대한 접근성, 의료서비스 제공자와 환자 간 상호작용 등에 영향을 미치며 건강 행동, 의료이용 행태, 건강 결과의 주요 영향요인으로 작용한다. 이 글에서는 2023년도 상반기에 공개될 2021년도 2기 한국의료패널 2차 조사의 헬스 리터러시 부가조사를 소개하고 주요 결과 및 정책적 시사점을 제시하고자 한다. 분석 결과 19세 이상 성인의 절반 정도가 건강정보에 접근하여 필요한 정보를 획득하고, 이를 이해하고, 적합한 정보인지 판단해 활용하는 데 어려움을 겪고 있으며, 사회경제적으로 취약한 경우 이러한 헬스 리터러시 수준이 더욱 취약한 것으로 나타났다. 헬스 리터러시 증진을 위한 다각적인 노력과 정책 수립이 필요한 것으로 판단되며, 의료이용 및 의료비 지출과 함께 의료이용에 영향을 미치는 다양한 요인을 포함하는 한국의료패널의 헬스 리터러시 부가조사 자료가 관련 연구와 정책 수립의 근거 자료로 널리 활용될 것을 기대한다. Health literacy influences self-management of health, access to health care use, and interaction between health care providers and patients. It is also a major influencing factor in health behavior, health care use patterns, and health outcomes. This paper aims to introduce and analyze the health literacy survey conducted in 2021 as a supplement to the Korea Health Panel Survey. The results of the supplemental survey indicate that almost half of persons aged 19 and over find it difficult to access, acquire, interpret, and use the information and services they require to make informed health decisions and take appropriate actions. Furthermore, health literacy levels are even worse among vulnerable socioeconomic groups. To establish policies to promote health literacy, it is necessary to monitor health literacy levels among Koreans. We hope that the supplemental survey of the Korea Health Panel Survey, a nationally representative survey of health care utilization, expenditures, and comprehensive influencing factors such as socioeconomic characteristics, comorbidities, and health behaviors, will be widely used to provide empirical evidence for researchers and policy-makers.
