한국 중고령자의 근로와 건강 관계 연구에 대한 방법론적 고찰

대부분의 선진국과 마찬가지로 한국은 인구 고령화가 중요한 사회적 문제로 떠오르고 있다. 그에 따라 중고령자의 근로가 새롭게 주목받고 있으나, 이런 논의는 주로 경제학적 관점에서만 이루어지고 있으며 보건학적 접근은 아직 부족하다. 본 연구는 한국에서 중고령자의 근로가 그들의 건강에 어떤 영향을 미치는지에 대한 선행 연구들의 결과를 종합하고, 연구 방법 측면에서 고찰하는 것을 목적으로 한다. 이를 위해 2000년 이후 국내외 학술지에 출판된 관련 문헌들을 모두 검색하였고, 본 연구에서 설정한 선정 기준에 적합한 문헌 15편을 분석하였다. 대다수의 문헌에서 한국 중고령자의 근로는 다양한 변수로 측정한 건강에 긍정적인 영향이 있다고 보고되었으나, 일부 문헌에서는 유의한 관계가 없거나, 하위 집단별로 유의성이 다르게 나타나기도 하였다. 연구 방법 측면에서 고찰했을 때, 많은 연구에서 인과성 측면의 내생성을 다루기 위한 연구 설계와 분석 방법의 적용에 제한이 있었다. 분석 대상 15편 중 7편의 문헌이 횡단면으로 설계되어 있었으며, 내생성을 다루기 위해 준실험적 연구 방법을 사용한 연구는 3편에 그쳤다. 본 고찰의 결과 한국에서 중고령자의 근로와 그들의 건강 사이의 관계에 대한 근거는 인과성 측면에서 아직 미흡하며, 이에 대한 보건 관점(population health perspective)의 보다 엄밀한 연구 설계와 분석 방법을 적용한 연구가 필요하다. Rapid population aging is a major social concern in Korea, and accordingly, the value of and needs for prolonged work by older adults have been debated. Such debates, however, have been mainly discussed from an economic perspective and rarely from a population health perspective. The aim of this study is to systematically review studies of how work affects health among Korean middle-aged and older adults. In particular, we did a critical review focusing on study designs and analytic approaches. Based on our selection criteria, a total of fifteen studies were selected and reviewed. A majority of the studies reported a positive relationship between work and health among middle-aged and older adults; however, some found no statistically significant relationship, and others found the relationship may differ by sex. In terms of methods, only a few studies addressed the endogeneity issues between work and health, and their approaches still did not fully address the issues. Our review suggests further studies with a population health perspective are needed on the relationship between work and health among middle-aged and older adults. In the future studies, the causal impact of work on health needs to be more carefully investigated with methodological rigor and the unique social context of Korea around the topic of interest should also be considered.
