Objective: This study aimed to identify regional structural vulnerability typologies at the eup/myeon/dong level in Gyeongsangbuk-do, examine individual-level health vulnerability latent classes at the si/gun level, and analyze the associations between the two factors as well as changes across two time points.
Methods: Data from the Korean Community Health Survey(KCHS) for 2015 and 2023 were used, with the study population comprising adults aged 19 years or older residing in Gyeongsangbuk-do(2015: N=13,957; 2023: N=14,399). Regional structural vulnerability typologies were derived by cross-classifying z-standardized population-structure and healthcare-vulnerability indices using demographic indicators and healthcare resource indicators across 322 eup/myeon/dong units in Gyeongsangbuk-do. At the individual level, latent class analysis(LCA) was conducted incorporating five dimensions of health vulnerability: physical health, mental health, health behavior, healthcare utilization, and social health. Since KCHS data do not identify respondents' residences at the eup/myeon/dong level and are only available at the si/gun level, the structural vulnerability typologies derived at the eup/myeon/dong level were re-aggregated into representative typologies at the si/gun level for linkage with individual-level data. Differences in the distribution of health vulnerability latent classes by regional typology were examined using cross-tabulation analysis, and factors associated with health vulnerability were investigated using binary and multinomial logistic regression analyses.
Results: Four regional structural vulnerability types were identified: stable, population-vulnerable, healthcare-vulnerable, and complex-vulnerable. Between 2015 and 2023, 199 of the 322 eup/myeon/dong units(61.80%) maintained the same typology, while the proportions of stable and complex-vulnerable types increased and those of population-vulnerable and healthcare-vulnerable types decreased. At the individual level, three latent classes were derived: complex health vulnerability type(Class 1), partial health vulnerability type(Class 2), and generally healthy type(Class 3). Significant differences in the distribution of health vulnerability latent classes were observed across regional structural vulnerability typologies(p〈.001). Regression analyses revealed that national basic livelihood security recipient status was most strongly associated with health vulnerability, while cohabitation with a spouse and higher educational attainment served as protective factors. In 2023, the influence of regional variables related to demographic structure and healthcare supply was more pronounced compared to 2015.
Conclusions: Regional health inequalities represent a complex phenomenon shaped by the interaction between individual socioeconomic vulnerability and regional structural conditions, and analysis at the eup/myeon/dong level holds important significance in revealing intra-regional heterogeneity that cannot be captured by si/gun-level averages alone. Future regional health policies should advance toward a multilevel, precision-targeted approach that considers both regional typology and individual health vulnerability profiles. This requires prioritizing resources for complex-vulnerable areas while simultaneously establishing a precise health equity monitoring system capable of identifying latent vulnerable populations even within stable regions.
