Purpose: To evaluate the association between body mass index (BMI) and recurrence after corrective surgery for epiblepharon in pediatric patients and to improve the prediction of surgical prognosis. Methods: A retrospective chart review was conducted on 78 patients who underwent reoperation for recurrence after primary corrective surgery for epiblepharon at our institution between March 2006 and December 2024. Two comparison groups were used: 33 children who underwent reoperation after failed primary eyelid surgery for non-epiblepharon conditions, and a single-surgery group of 3,735 children with no recurrence after primary correction. BMI, BMI z-scores adjusted for sex and age, and their changes between primary surgery and reoperation were compared. Results: At primary surgery, mean BMI and BMI z-scores did not differ significantly between groups. At reoperation, the mean BMI (19.13 ± 3.27 kg/m²) and BMI z-score (1.06 ± 1.21) of the epiblepharon reoperation group were significantly higher than those of the comparison group (all p〈0.001). The increases in BMI and BMI z-score between primary surgery and reoperation were also significantly greater in the epiblepharon group (all p〈0.001). Compared with the single-surgery group, the reoperation group had significantly higher median BMI and BMI z-score at reoperation (p〈0.001). A higher BMI at primary surgery was significantly correlated with a shorter interval to reoperation (R²=0.07, p=0.019). Conclusions: Pediatric obesity and a rapid postoperative increase in BMI and BMI z- score are significant risk factors for epiblepharon recurrence. When planning treatment, clinicians should consider systemic growth and weight changes in addition to local eyelid findings, along with weight-control education and meticulous follow-up.
