Peripherally inserted central catheters are widely used in patients who require long-term and repeated venous access; however, various catheter-related complications during the dwell period can lead to unplanned early removal, which in turn results in interruption of treatment continuity, additional invasiveness from reinsertion, prolonged hospitalization, and increased medical costs. Unplanned removal of a peripherally inserted central catheter is a complex event that cannot be explained by any single factor, and most prior predictive studies have been designed as binary classification predicting whether an event occurs at a specific time point, thereby failing to adequately reflect time-varying risk and censoring. Accordingly, this study aimed to dynamically predict the risk of unplanned removal due to catheter complications over time using survival-analysis-based deep learning models, and to identify the contribution direction and relative importance of key risk factors through explainable artificial intelligence, thereby proposing an interpretable clinical decision support model.
This study used retrospective electronic medical record cohorts from two institutions. The internal cohort comprised 843 cases of peripherally inserted central catheter insertion at an institution in Incheon, of which 43 were events, and was divided into training, validation, and test sets at a ratio of 8:1:1. The external validation cohort comprised 218 cases from an institution in Seoul, of which 18 were events. The Cox proportional hazards model, a standard statistical model in survival analysis, was set as the reference model, and DeepSurv, DeepPAMM, CoxTime, and DeepHit, which progressively relax the assumptions of linearity, proportional hazards, and semiparametric structure, were comparatively evaluated. Class imbalance in the training data was corrected using an oversampling technique, and model performance was assessed using the C-index, IBS, and time-dependent AUC at 7, 14, and 30 days. For model interpretation, SHAP was applied to derive the contribution and directionality of each variable, which were then compared with the hazard ratios from the Cox proportional hazards model.
In the internal cohort, DeepPAMM showed the best discriminative performance with a C-index of 0.74 and an AUC of 0.87 at 14 days, while CoxTime demonstrated a strength in short-term risk prediction with an AUC of 0.87 at 7 days. In contrast, in the external cohort, DeepPAMM and DeepHit showed large errors from 40 days onward, whereas the relatively simple CoxPH and CoxTime maintained stable performance across all time points. SHAP analysis identified catheter insertion length, catheter diameter, history of prior peripherally inserted central catheter insertion, and catheterization site as key risk factors, and the tendency for the contribution of catheter insertion length to rise to the top as time elapsed was consistently observed in both the internal and external cohorts. In addition, a nonlinear interaction between two variables was identified, in which a higher risk of removal was predicted as catheter insertion length deviated from the appropriate range and became excessively long in older patients. In the univariate Cox analysis, none of the variables in the internal cohort were statistically significant, whereas in the external cohort only catheterization site was identified as a significant factor, with a hazard ratio of 0.25 and a p-value of 0.002, indicating a significantly lower risk of removal in the right-side insertion group compared with the left-side group.
By dynamically predicting the risk of unplanned removal of a peripherally inserted central catheter over time and presenting the basis for these predictions through SHAP-based interpretation, this study demonstrated the potential of a survival-analysis-based deep learning model as an interpretable clinical decision support model that can support early identification of high-risk patients and preventive intervention. Furthermore, the univariate statistical analysis showed that the linear contribution of individual variables alone is insufficient to fully explain removal risk, supporting the need for survival-analysis-based deep learning models that learn nonlinear interactions among variables. However, limitations remain regarding data scale, model generalizability, and validation under diverse clinical conditions; therefore, future studies are intended to expand multicenter data, secure model generalization performance through transfer learning, and conduct subgroup analyses considering disease severity and total parenteral nutrition status.
