    기후변화는 인위적인 온실가스 배출로 인해 심화되고 있는 가장 시급하고 장기적인 범지구적 과제이다. 이는 단순한 환경 문제를 넘어 공중보건의 중대한 위협요인으로, 응급의료 수요에도 직접적인 영향을 미친다. 본 연구는 기상학적 요인이 응급 환자 수와 특정 질환에 발생에 영향을 미치는 요인을 규명하기 위한 후향적 단면 연구이다.
    연구자료는 국립중앙의료원의 국가응급의료진료정보망(National Emergency Department Information System, NEDIS) 과 기상청의 기상 데이터를 기반으로 활용하였다. 2020년 1월부터 2024년 12월까지 5년간 경기도 내 권역응급의료센터와 지역응급의료센터로 내원한 환자 중 변경된 중증도 분류 결과가 ‘Level 1: 소생’,’Level 2: 긴급’,’Level 3: 응급’에 부합하는 환자로 최종 분석대상은 3,235,690명이었다.
    수집된 자료는 IBM SPSS version 25.0 Statistic을 이용하여 기술통계, Pearson`s correlation coefficient, 카이 제곱 검정, 다중 선형 회귀분석, 로지스틱 회귀분석을 실시하였고 다중 집단 비교의 경우 Bonferroni 보정 방법을 이용한 사후분석을 수행하였다.
    전체 내원 환자의 과반수가 50대 이상의 중∙ 노년층이었으며, 주말과 월요일에 내원이 집중되는 과밀화 현상이 확인되었다. 특히 호흡기계 환자는 전체 환자 중 차지하는 비율은 적었으나, 입원률이 88.6%로 비호흡기 환자(34.8%) 대비 압도적으로 높아 응급의료 자원 소모가 큰 질환군임이 확인되었다.
    다중 선형 회귀분석 결과(설명력 R2=0.693), 기상 요인은 전체 환자 수에 유의한 영향을 미쳤다. 평균기온, 일교차, 평균 상대습도, 합계 일사량, 적설량의 증가는 응급실 내원 환자 수의 유의미한 증가(p〈0.05)를 이끈 반면 일 강수량, 및 미세먼지 농도의 상승은 외부 활동 제약으로 인해 응급실 내원 환자 수를 감소시키는 부적(-)상관관계(p〈0.05)를 보였다.
    전체 환자 수의 감소 추세와 달리 질환 및 의도성에 따라 환경적 위험 요인은 뚜렷하게 양극화되었다. 한랭한 환경에서는 순환기계 및 호흡기계 환자가 증가한 반면, 고온 환경에서는 감염성 질환, 비의도적 사고 및 폭력/타살(남성 우위)이 급증하였다. 또한, 고농도 미세먼지와 극심한 일교차 환경은 정신 및 행동장애 환자와 자해/자살 시도군(여성 우위)의 발생을 가장 크게 높이는 유발 요인으로 작용하였다.
    이상의 연구 결과를 통하여 기상 및 대기 환경의 변화가 응급환자의 전체 규모를 증감시킬 뿐만 아니라, 환자가 가진 질환의 종류나 손상의 원인(사고, 폭력, 자해 등)에 따라 각기 다른 유형의 응급 상황을 촉발한다는 사실을 확인하였다. 비가 오거나 미세먼지가 심한 날 전체 내원 환자수가 감소하더라도 순환기계 응급환자나 자해/자살과 같은 고위험군의 유입은 오히려 증가할 수 있다. 따라서, 단순한 환자 수 예측을 넘어 기상 데이터와 응급의료망을 연계하여 특정 중증환자(중환자실, 정신 응급 개입 등)의 수요를 선제적으로 대비하는 감시체계의 마련을 고려하여야 할 것으로 사료된다.

    Climate change is the most urgent and long-term global challenge, exacerbated by anthropogenic greenhouse gas emissions. It goes beyond a mere environmental issue to become a major threat to public health, directly affecting the demand for emergency medical care. This study is a retrospective cross-sectional study designed to investigate the impact of meteorological factors on the number of emergency patients and the incidence of specific diseases.
    The study utilized data from the National Emergency Department Information System (NEDIS) at the National Medical Center and meteorological data from the Korea Meteorological Administration. The study included patients who visited regional emergency medical centers and local emergency medical centers in Gyeonggi Province from January 2020 to December 2024, with revised severity classification results corresponding to “Level 1: Resuscitation,” “Level 2: Urgent,” or “Level 3: Emergency.” The final analysis included 3,235,690 patients.
    The collected data were analyzed using IBM SPSS Statistics version 25.0 to perform descriptive statistics, Pearson’s correlation coefficient analysis, Chi-square test, multiple linear regression analysis, logistic regression analysis, and post-hoc analysis using the Bonferroni correction method for comparisons across multiple groups.
    More than half of all patients were middle-aged or elderly (aged 50 or older), and a concentration of visits was observed on weekends and Mondays. In particular, although respiratory patients accounted for a small proportion of the total patient population, their hospitalization rate was 88.6%, which was overwhelmingly higher than that of non-respiratory patients (34.8%), confirming that they constitute a disease group that consumes significant emergency medical resources.
    The results of multiple linear regression analysis (coefficient of determination R2 = 0.693) showed that meteorological factors had a significant impact on the total number of patients. Increases in average temperature, daily temperature range, average relative humidity, total solar radiation, and snowfall led to a significant increase in the number of emergency department visits, whereas increases in daily precipitation, and fine dust concentration showed a negative correlation, reducing the number of emergency department visits due to restrictions on outdoor activities.
    Contrary to the overall downward trend in patient numbers, environmental risk factors were distinctly polarized depending on the type of illness and whether the incident was intentional or unintentional. In cold environments, the number of patients with circulatory and respiratory system disorders increased, whereas in hot environments, there was a sharp rise in infectious diseases, unintentional accidents, and violence/homicide (predominantly among males). Furthermore, high concentrations of fine dust and environments with extreme daily temperature fluctuations acted as the primary triggers for the highest increases in patients with mental and behavioral disorders and those who attempted self-harm or suicide (predominantly women).
    Based on these findings, it was confirmed that changes in weather and atmospheric conditions not only affect the overall volume of emergency patients but also trigger different types of emergencies depending on the patient’s specific condition or the cause of injury (accidents, violence, self-harm, etc.). Even if the total number of patients decreases on rainy days or days with high fine dust levels, the influx of high-risk groups, such as cardiovascular emergency patients or those involved in self-harm/suicide, may actually increase.
    Therefore, it appears necessary to consider establishing a surveillance system that goes beyond simple patient count predictions by linking weather data with the emergency medical network to proactively prepare for the demand for specific critically ill patients (e.g., those requiring intensive care unit admission or psychiatric emergency intervention).
