The purpose of this study was to examine the effects of a reminiscence program on ego integrity and depression among rural female elderly living alone. Nine participants aged 70 or older who used village community halls in a rural area (G-gun, North Gyeongsang Province) were selected. The K-MMSE~2 Brief Version was used as a screening criterion for participant selection and as a cognitive stability monitoring tool, not as a dependent variable. The program was structured around Butler's (1963) Life Review theory, using various activities and symbolic media (e.g., yarn, miniature crafts) suited to elderly women with low literacy, physical limitations, and patriarchal life histories, so that the cognitive restructuring and positive reframing that naturally arise during reminiscence could take place. Six sessions (90 minutes each, once weekly) were conducted by the researcher and a local assistant. Depression and ego integrity were measured before and after the program using the Korean version of the short-form Geriatric Depression Scale (GDS-K, 15 items) and an 8-item Ego Integrity Scale, and the data were analyzed using paired t-tests along with qualitative content analysis. Results showed that GDS-K depression scores decreased significantly from a pretest mean of 9.67 to a posttest mean of 2.67 (t=13.28, p〈.001), and ego integrity scores increased significantly from 21.78 to 33.67 (t=-10.82, p〈.001). Qualitative analysis revealed stepwise processes of externalizing past pain, reconstructing negative cognitions, and agentically integrating life narratives. However, participants in this study were limited to elderly individuals who were relatively cognitively intact and able to attend all six sessions consistently; the findings should therefore be interpreted as reflecting effects observed under these conditions rather than being generalized to rural elderly women as a whole. These findings suggest that a reminiscence-based approach may help reduce depression and enhance ego integrity among rural elderly women for whom verbal cognitive behavioral therapy is largely inaccessible, providing exploratory foundational data for community-based psychological support programs for rural elderly populations.
