Parkinson’s disease (PD) is associated not only with motor symptoms but also with impairments in various aspects of communication. However, studies examining communication difficulties in individuals with PD from a multidimensional perspective remain limited in Korea. In particular, little is known about the specific communication difficulties experienced in real-life communication situations and the factors associated with such difficulties. Therefore, this study aimed to investigate communication difficulties in individuals with PD from a multidimensional perspective, including physical function, emotional and psychological factors, real-life communication, communication abilities, and quality of life. In addition, this study examined patterns of communication difficulties within specific communication situations and explored differences according to individual characteristics.
Study 1 was conducted to investigate the overall characteristics of communication difficulties in individuals with PD and included 35 individuals with PD and 35 caregivers. Data were analyzed using repeated-measures analysis of variance, one-way analysis of variance, paired t-test, correlation analysis, multiple regression analysis, and content analysis. The results showed that participants reported relatively greater difficulties in respiration, word retrieval and fluency, and voice and phonation. Overall communication difficulties tended to increase with disease severity. No significant differences were found between patients’ and caregivers’ perceptions of communication difficulties. All communication domains were significantly associated with quality of life, with nonverbal communication showing the strongest association. In real-life communication, participants reported greater difficulties in telephone conversations, asking and answering questions in public places, using public transportation, and exchanging information. Content analysis further revealed that communication difficulties were influenced not only by linguistic abilities but also by contextual factors such as time pressure, environmental demands, and interactional burden.
Study 2 was conducted to examine detailed patterns of communication difficulties in real-life communication situations and differences according to individual characteristics in 130 individuals with PD. Data were analyzed using repeated-measures analysis of variance, one-way analysis of variance, correlation analysis, and multiple regression analysis. The results indicated that the level of difficulty varied across specific communication tasks even within the same communication situation, demonstrating the situation-specific nature of communication difficulties in PD. In addition, overall communication difficulties tended to increase with longer disease duration, greater disease severity, and older age. Factors associated with communication difficulties differed across situations, with prosody emerging as the most consistently related factor.
The findings of this study demonstrate that communication difficulties in individuals with PD represent a multidimensional phenomenon that extends beyond speech and voice impairments and is closely related to quality of life and communicative participation. Furthermore, this study highlights the importance of understanding communication difficulties within real-life contexts and suggests that communication assessment and intervention should consider both situation-specific communication demands and interactional characteristics.
