Adolescents with autism spectrum disorder (ASD) may experience difficulties in emotional awareness and expression, sensory awareness, and the use of emotion regulation strategies. Therefore, concrete and structured emotion regulation support programs that consider the characteristics of adolescents with ASD are needed. Although some studies have reported mindfulness-based interventions for adolescents with ASD, research on the development of programs that integrate mindfulness and art therapy for the purpose of supporting emotion regulation remains limited. Accordingly, this study aimed to develop a mindfulness-based art therapy program to support emotion regulation in adolescents with ASD and to explore the applicability of the developed program.
The study was conducted through the following stages: planning, needs analysis, preliminary program development, pilot implementation, final program development and application, and evaluation. In this process, previous studies were reviewed, and a needs analysis was conducted with adolescents with ASD, caregivers, and experts. Based on these findings, a preliminary program was developed, followed by expert validity verification, pilot implementation, and subsequent revision and supplementation. The final program consisted of eight individual intervention sessions, conducted once a week for 60 minutes per session. Considering the characteristics of the participants, the program included visual structuring, sensory-based activities, choice-based responses, concrete activity instructions, repeated practice, and home practice.
The final program was implemented with two adolescents with ASD who had difficulties in emotion regulation, did not have an intellectual disability, and had a full-scale IQ of 70 or above. The Korean version of the Child and Adolescent Mindfulness Measure (K-CAMM) and the Difficulties in Emotion Regulation Scale for Adolescents (DERS) were administered at pretest, posttest, and follow-up. In addition, session responses, artwork, activity sheets, home practice materials, and caregiver interviews were analyzed to examine changes in mindfulness and emotion regulation, as well as patterns of sensory, emotional, and cognitive awareness and emotional expression.
The two participants who took part in the final program showed different patterns in the self-report measures. However, session responses, artwork and activity sheets, home practice materials, and caregiver interviews indicated that both participants became more able to notice and express sensations, emotions, and thoughts. They were also observed attempting emotion regulation strategies in emotionally difficult situations, such as pausing before reacting, choosing appropriate behaviors, stopping and breathing, waiting, and selecting methods that suited them. Caregiver interviews also reported possible improvements in patience and concentration, more concrete emotional expression, reduced emotional reactivity, and changes in regulation-related behaviors.
These findings suggest the potential applicability of the mindfulness-based art therapy program in clinical and educational settings. However, because this study was based on a small number of cases, there are limitations in generalizing the results. Further research with more participants and continued program refinement is needed.
